Arintha Latimer, training coordinator, accompanied me during today’s visit. Upon our arrival, we were greeted by Ms. M. Gammon provider/owner. I informed Ms. Gammon the purpose of today’s visit. Ms. Gammon gave me the necessary paperwork we needed to complete today’s visit. I observed and documented the attendance. Two (2) children were present during today’s visit. They were observed arriving and playing with available toys Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions CPR and FA Certification One (1) violation was cited during today’s visit. Violation Number Comment Rule 2057 The family child care home was not free of asbestos hazards as defined in G.S. 130A-444(2) and 10A NCAC 09 .0102(4). The provider did not complete the required testing to ensure the home is free from asbestos hazards. 10A NCAC 09 .1702(2) The above violation must be corrected immediately, and a letter must be emailed to me no later than March 2,2026 stating how the one item was corrected. My email address is wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by March 2,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Gammon that her in-service training hours, CPR and First Aid trainings by specific dates. Your cat vaccination is due April14,2026. Please refer to your verification form. Also, we discussed the requirements you need to ensure you are maintaining requirements with minimum requirements. Rated License : We discussed with Ms. Gammons the Rated license assessment process for the hold harmless, is now over. The new QRIS has been adopted effective July 1,2025. We reviewed the QRIS with Ms. Gammons. After the review ,Ms. Gammons stated educed to a that she wants to reduce to One Star rated license because she wants to do the new requirements since she is close to retiring. I informed Ms. Gammons that she will need to send me a letter in writing with her signature stating that she wants to reduce her Two Star license to One star. Once the violation had been corrected and I receive your letter then I can submit the paperwork to reflect your request. Clean Classrooms for Carolina Kids This facility completed the lead water testing on November 22, 2022. The facility has enrolled lead paint and asbestos testing but has not completed the training. A checklist was used to note the requirements I monitored today. Cherissa Alston, a childcare consultant accompanied me during today’s visit. This program currently operates with a Two Star License, issued 10/21/19 earning 1 point in Education, 2 points in Program Standards and 1 point in Quality for infant capacity restricted to no more than two children under one year of age. Upon our arrival, we were greeted by Ms. M. Gammons provider/owner. We informed Ms. Gammon the purpose of today’s visit. Ms. Gammons gave us the necessary paperwork we needed to complete today’s visit. WE observed and documented the attendance. Two (2) children present during today’s visit. They were observed playing with available toys. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. All required items were posted. The space contained the required materials. All medication and hazardous items were out of the children's reach. I monitored the area of the home used by the children. I was unable to monitor the outdoor area due to heat advisory. I reviewed all children's files and the provider's file. Today Ms. Gammons stated that she did not provide transportation. To verify compliance with specific requirements, you were given a Verification of Required Information for Operator and Additional Caregivers form and FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form to complete. During today’s visit, we completed the forms. Your signature on these forms will serve as verification the information provided is accurate and complete. Any violations observed today were discussed with Ms. Gammons and documented on the Visit summary. There were seven (7) violations cited during today’s visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. There was a can of Febreze accessible to the children and it was not locked storage. .1719 (a)(7) 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The playground inspections were not available for review. 10A NCAC .1721(e)(5)(A-F) 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. The monthly fire drills were not available to review. .1721 (e )(2) 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The provider did not have vaccinations for a cat. .1719(b)(1) 927 Operator did not follow the written feeding schedule for each child under 15 months of age. The provider did not updated the written feeding schedule for an infant. .1706(i) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. A copy of the shaken baby syndrome and abusive head trauma policy was not given to two parents at time of enrollment. .1726(b)&(c) 2048 Products that are labeled “keep out of reach of children” with an additional warning(s) on the label, were not kept in locked storage while children were in care. There was cat Chow and insecticide accessible to the children. 10A NCAC 09 .1719(a)(7) The above violations must be corrected immediately, and a letter must be email to me no later than September 13,2024 stating how each item was corrected. If I do not receive your letter by September 13,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Gammons that she needs in-service training hours, health safety trainings by September 7,2024. If you do not receive the 10.5 hours by this date, then I will add a violation for not completing in-service training hours. Please refer to the verification form. You need to email the training certificates once staff has completed the training hours. I discussed with Ms. Gammons she needs to complete her, and her husband’s criminal records check by October 24,2024, CPR and First Aid Trainings January 31,2025. Please refer to verification form. You need to email the criminal letters and training certificates once you have has completed the training hours. Today we discussed the cat that you are providing food. You stated that this cat does not come into your house, and it is not your cat. I explained to you that the fact that you have food in your house and a food bowl at the back door then you will need to have the required vaccinations for this cat. You will need to notify the parents about this cat and where the cat will be located while children are in care. Technical Assistance: I observed today that two children had permission to use sunscreen and diaper on an old form. I explained to Ms. Gammons that I will send her copy the new medication and topical ointment forms. I also reviewed with her the required document that she needs to have in her file. I discussed with Ms. Gammons about her activity plans. Ms. Gammons stated that she was a little confused and that she was working with the family childcare specialist, Ms. Ryan Farrington. I told Ms. Gammons that I would contact Ms. Farrington and set up a visit with her to come and help you with your activity plans. Rated License Assessment: I also discussed with Ms. Gammons the Rated license assessment process. The hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. Thank you for your time during today's visit. This family child care home currently operates with a Two Star license, effective October 21, 2019, earning 1 point in Education, 2 points in Program, and 1 Quality point, programmatic option met for serving no more than two infants under the age of one year. This family child care home is licensed to operate on first and second shift. Upon my arrival I was greeted by you, Mona Gammons, the family child care home provider. There were three children present. I observed two children participating in free choice activities in the primary space. I observed one infant napping in a separate room. A partial assessment of child care requirements was monitored. I reviewed two children's files. Today you stated that you do not transport the children enrolled in the family child care home at this time. The children play in the front yard and on the back deck. During today's visit two violations of the child care requirements were cited. The violations were corrected during the visit. A compliance letter is not required. Violation Number Comment Rule 726 Children were sleeping or napping in a room with a closed door between the child and the supervising staff. The infant was sleeping in room with the sliding door closed. .1711(a)(4)(D) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. A bottle of gout cleaner was stored on a shelf within the reach of where the two children were playing. .1719(a)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. TA: 10A NCAC 09 .1711 SUPERVISION OF CHILDREN IN A FAMILY CHILD CARE HOME: (D) for children who are sleeping or napping, the operator shall not be required to visually supervise them but shall be able to hear and respond without delay to them. Children shall not sleep or nap in a room with a closed door between the children and the operator. The operator shall be on the same level of the home where children are sleeping or napping. Today we discussed the infant sleeping in a separate room with a closed door. The door is a sliding pocket door that closes automatically. I suggested you place the infant in one of the two approved bedrooms without the sliding door or in the primary space. You stated that the infant is a light sleeper and is easily awoken by noise when the other children are playing in the primary space. You stated that you prefer the room closer to the primary space so you can respond to the infant’s needs quickly. You stated you prefer to use a door stopper to prevent the door from closing.
