Name of Operation: MILESTONES SCHOOL OF ACHIEVEMENT Facility ID: 41002546 Consultant: MARY MCMILLAN Operation Type: Center Case Number: Visit Date: 12/16/2025 Number Present: 157 Completed Date: 12/16/2025 Age: From 0 To 5 Total Minutes: 195 Time In: 12:00 PM Time Out: 03:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period visit. Today I met with Jennifer Doggett, Administrator. This is the third temporary visit conducted. The following limited items were monitored during today’s visit: Staff/Child Ratio, Supervision, approved space, permit posted and restrictions met, 12 new staff files, ITS-SIDS trainings, criminal background checks, CPR/FA training and maltreatment training. Upon arrival the children were observed having teacher directed circle time, transitions to eat lunch, being served lunch and playing. The following violations were documented during today’s visit: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. This could not be found for 1 new staff member. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months, for one new staff member this information could not be found. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). 1 staff members qualification letter expired 12/10/25 and no renewal process had been started. G.S. 110-90.2(b) & .2703(n)&(o) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. There was no orientation verification for 9 new staff members. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. 2 new staff members did not have first aid within their 90 days of hire. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. 2 new staff members did not have their CPR within their first 90 days of hire. .1102(d) 1757 A valid qualification letter was not on file and available to review at the facility, for 2 new staff members. G.S. 110-90.2(b) & (d) & .2703(e) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. There was no training on file for 3 new staff members within their first 90 days. .1102(g) We discussed the following items of consultation during today's visit: -Pathway to the stars information for the Curriculum and Instructional Quality Pathway and what is required for forms and to be in place prior to January 10, 2026 when I will need the application with all forms for Pathway #2. Staff education must meet the star level that you wish to achieve: Lead Teachers by 50% and then 50% of all other positions. -I looked up multiple staff's education in WORKS that Mrs. Doggett had questions about. The following technical assistance items were discussed during today's visit: -Staff trainings and onboarding process for DCDEE requirements: You stated that you understood that staff must have CPR/FA and Child Maltreatment training within their first 90 days, but that you were told by your management to have them do other items first. These are now violations since you have multiple staff that do not have these required trainings within their first 90 days. -You could not find one staff member's medical statement and TB information, which you think may have been misfiled. -Criminal Background Check (CBC) letters must be on file upon their start date, to verify that they have it completed. This was corrected during the visit. -You must have everyone re-new their CBC before the expiration date of their last letter. They can do this up to 6 months before their letter expires. -Proof of orientation must be available on file for all staff. You stated that you are aware of this and with the new process for onboarding you are not sure what is in the system that you now use for this. We discussed that your violations must be corrected immediately and you must submit a compliance letter, stating how they are corrected by 12/29/25. Please email me this letter at mary.mcmillan@dhhs.nc.gov and let me know if you have any questions. All violations but me corrected before your rated license paperwork can be processed. In order to have time to process your rated license paperwork, all items must be received by 1/10/26. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
