Carolyn Stover, owner/operator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Stover and one additional staff member. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 1845 The child care provider and/or household member who incurred pending charges, indictments, or convictions since their last qualification letter was issued by the Division, did not notify the operator of these charges within five business days or before returning to work. The operator did not notify the Division of these pending charges, indictments, or convictions within one business day of being notified by the child care provider or household member. The operator was criminally charged with communicating threats on 11/5/25, and did not report the new criminal charge to the Division of Child Development and Early Education within 24 hours, as required by the rule. G.S. 110-90.2 & .2703(m) Violations must be corrected immediately. Within one week, by 11/24/25, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at scott.mason@dhhs.nc.gov. A checklist was used to note the requirements I monitored today. I also monitored health and safety requirements. This program currently operates with a Three Star License, issued 10/3/18 earning 4 points in Education, 2 points in Program and 1 point in Quality for serving no more than two infants under age one. Upon my arrival I was greeted at the door by the owner/operator Ms. Carolyn Stover. You invited me into the home. One child was present. I observed the child playing in an Jumperoo Bouncer, having tummy time and being held. During today's visit, no children were present. There are 11 children enrolled. I reviewed the operator's file and a sampling of children files. A satisfactory fire inspection was conducted on October 23, 2023. The last documented fire drill was conducted on 4/8/24. The last documented shelter in place drill was conducted on 1/10/24. To verify compliance with specific requirements, you were given a Verification of Required Information for Operator and Additional Caregivers form and FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form to complete. During today’s visit, you completed and sign the forms. Your signature on these forms will serve as verification the information provided is accurate and complete. One violation of child care requirements was cited. Violation Number Comment Rule 709 Equipment and toys were not in good repair and developmentally appropriate. The red outdoor climber was missing two wooden steps. 10 A NCAC 09.1720(a)(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before April 29, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date each violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. Please email the compliance letter to: sharon.y.williams@dhhs.nc.gov. Your facility is in Cohort One. Your facility’s preparation year began on July 1, 2023 and will end on June 30, 2024. Your reassessment year which will be between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. Today we discussed Indicator Item 15 using Books from the Family Child Care Home Environment Rating Scale Workbook. You stated that this is the indicator item you will use for your three month self assessment. I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year: • Begin the FCCERS-R self-study process now since it must occur over a 3-month time period prior to your rated license reassessment • Consider requesting an FCCERS-R assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways • ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality • If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment • Assessment scores can be saved to use during the reassessment year If you have questions, please contact me.
