A checklist was used to note the requirements I monitored today. This facility currently operates with Four Star license issued on July 8,2021. Upon my arrival to the facility, I was greeted by K. Greggs, the director. Ms. Greggs informed me that Ms. V. Lea, owner/co director, was not here today. I informed her the purpose of my visit. Ms. Greggs gave me the necessary paperwork and assisted me during my visit today. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Three (3) children were present during today’s visit. I observed the children playing on the floor with available toys, playing in centers, washing their hands in preparation for lunch, eating lunch and napping. A sanitation inspection was conducted on April 14,2025. This facility received a superior rating with six (6) demerits. A fire inspection was conducted on April 23,2025. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as August 18,2025. Ms. Greggs informed me there were no children on any type of medication and she did not provide transportation services therefore; I did not monitor medication or transportation requirements during this visit. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Monthly playground inspections were available for the months September 2024 to December 2024. .0605(q) 1043 All staff records, except financial records, were not made available for review. Two staff records were not available for review. G.S. 110-91( 9) The above violations must be corrected immediately, and a letter must be emailed to me no later than September 1,2025 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by September 1 ,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Greggs, that the staff members need in-service training hours, It-SIDS, health and safety training, CPR and First Aid trainings by specific dates. Please refer to the previous staff/training worksheet. Today, I discussed with Ms. Greggs her hours and when she works at this facility since I was informed that she works in a classroom at the Kernersville location. I explained to Ms. Greggs that she will need to be at this location as the director for hours at least 30 hours a week. Ms. Greggs says that she opens in Kernersville then she comes to this location. I spoke with Ms. Lea on the phone, and I informed her that I will need to know who the director at the Greensboro facility. I informed Ms. Lea that Ms. Greggs has been working in a class room at your Kernersville location. Ms. Lea informed me that Ms. Greggs is the director at the Greensboro location and she co-director at this location. I informed Ms. Lea thank you for clarifying this information. Rated License: I discussed with Ms. Greggs the Rated license assessment process for the hold harmless, is now over. The new QRIS has been adopted effective July 1,2025. I have explained to Ms. Greggs new process that will be explained in detail at the upcoming directors’ meeting in September 23,2025. Beginning on February 1, 2025, all child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. You can visit ncrlap.org for any upcoming training courses for ITERS-3 and ECERS-3. ABCMS Provider Portal North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Additional information and instructions are available in this ABCMS Provider Portal Guide. Once you have been notified that access has been granted, log into the provider portal to locate the unique provider code for the facility. Applicants and employees will use this code to create a connecting application to be associated with your facility. Clean Classrooms for Carolina Kids This facility completed the lead water on January 25,2025. The facility has also completed lead paint and asbestos testing however you are in mitigation for the lead paint. I spoke You need to send me the updated information regarding the date the contractor has completed the work and the date scheduled to have the completed work inspected. I need you to contact me by August 22, 2025. These tests should a be completed every three years. A checklist was used to note the requirements I monitored today. This facility currently operates with Four Star license issued on July 8,2021. Upon my arrival to the facility, I was greeted by Ms. T. Melvin one of the lead teachers. She informed me that Ms. V. Lea, owner/director was not here at the facility. Ms. Melvin contacted Ms. Lea and informed her of my arrival. Ms. Melvin informed me that Ms. Lea was on her way to the facility. Ms. Lea arrived along with Ms. K. Greggs, / the co-director. I introduced myself and informed them the purpose of my visit. Ms. Lea gave me the necessary paperwork and assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Six(6) children were present during today’s visit. I observed the children playing on the floor with available toys, playing in centers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch and napping. A sanitation inspection was conducted on July 23,2024. This facility received a superior rating with fifteen (15) demerits. A fire inspection was conducted on April 4,2024. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Violation Number Comment Rule 468 When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. There was not enough materials in the block area. .0510(d)(1) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The mulch was measured less than one inch under the climbing structure. .0605(j) 721 All equipment and furnishings were not in good repair. The fence is broken at the top in two areas. Also there is a gap at the bottom of the fence in two places on the playground. there gaps are entanglement and entrapment hazards. G.S. 110-91(6); .0601(b) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. The grass was overgrown in several places on the playground as well as weeds were growing threw the mulch. 15A NCAC 18A .2832(a) The above violations must be corrected immediately, and a letter must be email to me no later than September 6,2024 stating how each item was corrected. Rated License Assessment: I also discussed with Ms. Lea the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. Technical Assistance: Today I discussed with Ms. Lea the incident log. Ms. Lea informed me that she thought the incident logs need to be completed only when a child received medical attention from the doctor. I explained to Ms. Lea that once you complete the incident form it must be add to incident log. We also discussed purchasing more materials in the event the toys need to be cleaned you will have materials available when your toys are bein cleaned. Thank you for your time during today's visit.
