The last annual compliance visit was conducted 3/25/25. Upon arrival, I was greeted by Wanda Johnson, Administrator. I stated the purpose of my visit and completed a walkthrough of the licensed spaces. There were twelve children present aged 4 months to four years old. There are a total of 14 children enrolled. Children were observed playing with blocks on a table. During block play, they engaged in group time with the teacher as they sang their good morning song, reviewed ABC’s, counted to 20 and named the color of the blocks. Infants and toddlers were observed eating breakfast, being bottle fed, and playing with a variety of toys of their choice. Interactions observed were positive and caring in nature. Some of the required items were posted, including emergency numbers and menus. However, activity plans were not current. A sanitation inspection was completed on 8/25/25, the facility earned a Superior status. I monitored documented outdoor inspections, fire and emergency drills. I monitored the following child care requirements today; Supervision, Capacity, • CPR / First Aid, • CBC Qualification • Storage of Hazardous Products • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation There were 2 items of non-compliance observed during today’s visit and the corresponding violations were cited. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. There was not a activity plan posted for children 0-1 years old. It was also not available for review. GS 110-91(12); .0508(a) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. Teesha L. did not have current IT'S SIDS training. She was the only teacher in the space for infants and toddlers. .01102 (f) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 9/17/25, describing how and when the violation(s) were corrected. Please remember your letter should include facility name, facility ID#, and original signature. The letter can be emailed to cherissa.alston@dhhs.nc.gov. Today we discussed lesson plans and making sure they are posted with or Friday evenings or first thing Monday morning to maintain compliance. I also advised you to have teachers date lesson plans each week. If teachers would like to recycle lesson plans have them make a copy of the plan before writing the date on them. I reminded you to make sure that disenrolled children are removed from the allergy list. The allergy list should be reviewed each month and updated as needed. Reminders ABCMS Portal The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Additional information and instructions are available in the ABCMS Provider Portal Guide. This training and roster needs to be completed immediately. Lead and Asbestos Contamination Some programs have only completed the process for lead testing in water. Testing includes lead in water, lead based paint, and asbestos. This needs to be completed immediately. As a reminder, child care facilities must test the water every three years for lead contamination. Please keep the results on file for review. Look for an invitation for an upcoming Director’s meeting in September. Thank you for your time today. This program currently operates with a Three Star License issued 4/9/20 earning 4 points in Education, 4 points in Program Standards and 1 point in Quality for enhanced policies and infrastructure of parent involvement. Today, I met with Ms. Wanda Johnson, the center administrator. I conducted a partial assessment of the child care requirements and completed a walkthrough of the facility and outdoor areas. There were 16 children present at the center. I observed the children engaged in teacher-led activities as well as completing their bathroom routines. In the infant room, I saw one child resting in a crib and four children dancing to music and playing with toys. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions; CPR and FA certification. The sanitation inspection was completed July 2, 2024 with a "Superior" classification with 4 Demerits. Outdoor inspections were documented and current. Emergency drills were documented and current. During today's visit, one violation of child care requirements was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The outdoor play area for children ages 2-12 had several issues. The plastic lining underneath the mulch was exposed, and grass was growing through the mulch. On the blue and red playhouse, a wooden board was broken. The yellow handrails on the climbing structure were loose. 10A NCAC 09 .0601(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before October 15, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If I do not receive your letter by October 15, 2024, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. REVIEW OF ENVIRONMENT RATING SCALE ASSESSMENT: Today, we thoroughly reviewed the recently completed ERS (Environmental Rating Scale) reports. The ECERS-R assessment conducted on August 22, 2024 scored 4.64 points; the ITERS-R assessment on August 15, 2024 scored 3.71 points. We discussed the specific items that scored 4 or below on the assessments and areas of licensing concerns. We also reviewed your corrections for those items. The Hold Harmless provisions have been extended, and star-rated license assessments for licensed child care providers will now be conducted only upon provider request, until the QRIS rulemaking process has been completed. Today you stated that you do not wish to proceed with the Rated License Process at this time. Thank you for your cooperation during today’s visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
