The last annual compliance visit was on 4/22/24. The facility has a compliance history of 95%. Upon arrival, I was greeted by Mrs. Harris-Thompson. I stated the purpose of the visit and you invited me inside. I complete a walkthrough of the licensed spaces. There was one toddler and six preschool children present. The toddler was observed crawling around playing with a variety of age appropriate toys. An infant arrived during the visit. The preschool children were observed drawing at the table, engaged in group time; singing songs, reciting the alphabet, and counting in Spanish. They were also observed eating lunch and napping. I monitored the outdoor spaces and there were no hazards observed. Required items were posted. You do not have any children enrolled with known allergies. There were no emergency medications, topical ointments or OTC medications on file today. Outdoor spaces were monitored and there were no hazards observed today. One randomly selected children’s file and staff files were monitored during today’s visit. Health and safety training and other annual requirements were current. A satisfactory fire inspection was completed on 2/12/25 and the facility was approved for day and night time care. A sanitation inspection was completed on 8/20/25 and the facility received a Superior rating. Outdoor inspections, fire and emergency drills were documented and current. There was one item of non-compliance observed today and the corresponding violation was cited. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). There was not a current qualification letter on file for S. Harris-Thompson. G.S. 110-90.2(b) & .2703(n)&(o) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 4/23/25, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. I provided technical assistance regarding your EPR plan today. I reminded you to login to the risk management portal at least once a year to make updates and review your plan and keep it current in the system. I gave you a phone number for IT regarding the portal for assistance with login. I reviewed the staff and training worksheet you provided. Your training year is January - December, I monitored the hours you currently have and S. Harris-Thompson needs 1.5 additional hours, J. Robinson needs 3.5 additional hours and I did not monitor M. Vanstory's file as she is out on medical leave. Ms. Vanstory will need a current qualification letter before she returns to work. Thank you for your time today. A checklist was used to note the requirements I monitored today. This facility currently operates with a Three Star license issued on June 28,2017. Upon my arrival to the facility, I was greeted by Ms. S. Harris-Thompson, lead teacher/owner. Ms. Harris-Thompson gave me the necessary paperwork I needed to complete today’s visit. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit, I observed the children reviewing letters, shapes, colors, washing their hands in preparation for lunch and eating lunch and napping. Eleven (11) children were present during today’s visit. A sanitation inspection was conducted on April 4,2024. This facility received a superior rating with two (2) demerits. A fire inspection was conducted on February 7, 2024. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Three(3) violations cited during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Seven children did not have arrival and departure times documented. 10A NCAC 09 .0302(d)(4) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. The provider did not keep the incident forms on file once children had an incident. The provider gave the forms to the parents. .0802 (e) 853 Incident logs were not completed and maintained as required. The provider did not complete the incident logs once a child had an incident at the facility. .0802(g)(1-6) The above violations must be corrected immediately, and a letter must be sent to me no later than May 6,2024 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov If I do not receive your letter by May 6,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Harris-Thompson the staff members need in-service training hours. It-SIDS, health safety, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. Today I explained to Ms. Harris-Thompson the requirements for the incident form and log. You must complete the form with every incident. This will allow you to have documentation in case something serious happens from the incident. You will only need to send the form to your consultant if the child receives medical attention. You will have seven days to submit the form to the consultant. Rated License Reassessment: I also discussed with Ms. Harris-Thompson the Rated license assessment process will begin July 1,2024. I informed Ms. Harris-Thompson this facility is in Cohort 1. There are two types of assessments. One is an observation assessment that you can contact the rated assessment project. The prep assessment you will contact the child care consultant to request the prep assessment. Ms. Harris-Thompson asked me what is the difference in the two assessments. I explained to her that the difference is the observation scores cannot count as your rated license assessment scores. The prep assessment scores you can count if the score is what you need to maintain your current star rated license. Your Prep assessment year is July 1,2023 to June 30, 2024. Your assessment year is July 1, 2024 to June 30,2025. Ms. Harris-Thompson informed me that she would contact EQUIP and then you will contact me to submit the request for the Prep assessment. I informed Ms. Harris- Thompson that she will need to contact me before June 30,2024. Thank you for your time.
