This Family Child Care Home currently operates with a One Star License, issued 2/3/25. A full assessment of child care requirements was conducted. Shannon Boyd, Owner/Operator, was present during today's visit. There were five children present. I observed four children napping and one child playing with a variety of toys. All indoor and outdoor spaces used by the children were observed today. All required items were posted. Outdoor inspections were documented and current. Emergency drills were documented and current. I reviewed four children’s files, the operator’s file and the program file. Today you stated that you do not transport the children enrolled in the family child care home. There is one household member and no pets. To verify compliance with specific requirements, you were given a Verification of Required Information for Operator and Additional Caregivers form and FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form to complete. During today’s visit, you completed and signed the forms. Your signature on these forms will serve as verification of the information provided is accurate and complete. There were two violations of Child Care requirements cited during this visit. Violation Number Comment Rule 920 Records were not made available for review. A file for the operator's grand child who was present was not available for review. G.S. 110-91(9) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. The operator has not completed Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1703(a)(5) The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before July 29, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation numbers. The date each violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If I do not receive your letter by July 29, 2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. Today we discussed the following: Emergency Preparedness and Response Training: you have until 8/19/25 to complete the training. Health and Safety and Safety Trainings: you have until 8/19/25 to complete the trainings. Today I showed you on the computer how to access the training on DCDEE Moodle. Shannon Boyd, Owner/Operator, was present during today's visit. Upon arrival the children were napping and playing. During today's visit I observed the following items: Supervision, Capacity, age appropriate activities, items posted, children's files and program records and approved space. There were no fire drills recorded. We discussed that fire drills must be conducted and recorded monthly and that you must use the smoke detector to signal the drills. The last playground inspection was recorded on 12/10/24. There were two children who had a bottle and sippy cup in their bed with them. The following violations were cited during today's visit: Violation Number Comment Rule 606 Each child was not held or placed in an appropriate feeding chair or other age appropriate seating apparatus to be fed. Two infants/toddlers were given a bottle and a sippy cup in their crib and cot. .1706(k) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment, for 1 child enrolled in October. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file, for one child enrolled in October. GS 110-91(1); .1721(a)(2) 1853 The operator did not conduct a monthly fire drill. There were no file drills recorded. .1719(a)(15) & .1721( e)(2) 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has, for 2 children enrolled. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) We discussed the following items of consultation during today's visit: -lesson plans should be individualized for each infant/toddler under 2 years of age, based on their individual developmental levels. -Each medication permission should be on it's own separate individual form. -You had a question about what a school-ager is and we discussed the definition of a school-ager: A 5 year old who has attended their first day of kindergarten. If a child is attending a school but in a pre-k program, that child is still a pre-school child. If that same child attends your facility too, they are still considered a pre-school child until they go to kindergarten. -You stated that you have not taken any educational courses or completed a self-study for your stars, and we discussed that you can be a 1 star license without any of these items. We discussed that once you complete EDU 119 and 6 additional ECE hours then your education points can go up to 4 points and once you complete your self study, you can move to 2 points in program and with a quality point earned, you can then move to a 3 star child care license. -We discussed that you cannot accept DSS vouchers without being at least a 3 star license and you stated that you understood this. -Once you have corrected your violations your license paperwork can be processed. You will then meet minimum requirements with: Education: 1 point Program: 1 point Total of 2 points for a One Star License can be issued once you have corrected your violations from today's visit. We discussed the following items of Technical Assistance during today's visit: -All preschool children must have a physical and shot records on file within 30 days of enrollment. This can be from last well child visit, as long as the visit was less than a year old. -All lines on the child's application must be addressed in some way: NA or none is fine if it doesn't apply. This is to ensure proper care for the children. -Fire drills must be conducted monthly and the smoke detector button must be used to signal the drills, so that the children know what the sound means and what to do when they hear the sound. - We discussed Rule .1706 The portion that states: Each child shall be held or placed in feeding chairs or other age-appropriate seating apparatus to be fed. Instead of placing infants/toddlers to sleep with bottles and sippy cups. You must know and follow all child care rules and requirements at all times, even if they have not been reviewed with you. You can find all of these on our website at ncchildcare.ncdhhs.gov You must correct your violations immediately and submit a compliance letter stating how they are corrected by 2/3/25.
