Name of Operation: LIFELINE DEVELOPMENTAL CHILDCARE Facility ID: 41002555 Consultant: MARY MCMILLAN Operation Type: Family CC Home Case Number: Visit Date: 4/21/2026 Number Present: 8 Completed Date: 4/21/2026 Age: From 2 To 4 Total Minutes: 140 Time In: 10:30 AM Time Out: 12:50 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period visit. The Temporary license expires on 6/9/26. Ms. Shayla Boyd was present with the children. Sharon Williams, Child Care Consultant, accompanied me during today's visit. Upon arrival, 5 preschool children were running around the living room with no toys/activities, jumping off of couches and the living room was gated off from the rest of the house. There were 3 preschool children sitting at the table in the dining area of the kitchen, with no activities. Ms. Boyd was in the kitchen cooking lunch, at the stove, during the majority of the visit. Ms. Boyd could not see the children in the living room from the kitchen due to there being a wall between the kitchen and the living room. Limited items were monitored during today's visit: Capacity, Supervision, items posted, permit posted and restrictions met, approved space, hazardous items and medication storage, two children’s files were reviewed today, one substitute staff’s file, program records, age-appropriate activities and nutrition components. The last fire drill was recorded on 3/2/26 at 10:50am with 3-minute evacuation time. The following violations were cited today: Violation Number Comment Rule 601 Nutritious meals and snacks served did not meet Meal Patterns for Children in Child Care Programs. At lunch children were served: spaghetti with meat sauce, corn, bananas and fruit juice. No milk was served or offered. 10A NCAC 09. 1706(a) 720 Adequate supervision was not provided for children in the Family Child Care Home. Upon arrival there were 5 preschool children in the living room running around the room, jumping off of furniture with no activities available and 3 preschool children sitting at the kitchen table with no toys/activities while the provider cooked lunch at the stove, in the kitchen, and there is a wall between the living room and the kitchen and the children in the living room could not be seen. .1711(a) 929 Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. Upon arrival the children were in the living room and dining areas with no activities and/or toys/equipment to play with. They were running around the room and sitting at the kitchen table. G.S.110-91(12) & .1718(a)(6) 932 Operator did not have materials or equipment available indoors or outdoors to support the activities listed on the written schedule and activity plan. There were a few small ice cube sized blocks and no dramatic play- food/cups/plates/pots/pans- items available for the children. The dramatic play items that were available, were put up and inaccessible to the children and the other items were not in quantity for the amount of children that are present. .1718(6) 1712 Operator did not develop and adopt a written plan of care for completing routine tasks to ensure routine tasks did not interfere with the care of children during hours of operation. The Written plan did not include all required information. This is a repeated violation from 2/17/26. .1712(a) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. There was vaseline, toothpaste and hydrocortisone cream on the bathroom sink and under the bathroom sink cabinet that was all within reach of the children. This is a repeated violation from 2/17/26. .1719(a)(7) We discussed the following items of technical assistance during today's visit: -Supervision is very important to keep children safe and know what they are doing at all times. Unless they are laid down for rest time, all children should be adequately supervised at all times. -Children should always have access to age-appropriate activities, unless napping. This ensures that children are engaged with developmentally appropriate play and equipment and helps with discipline issues, inappropriate behavior, hitting, and so on. Children should have choices of different age-appropriate activities that meet minimum requirements available to them throughout the day. -As discussed during my last visit, you will need more age-appropriate activities/equipment, along with keeping the minimum required activities available for the children to play with and they must be accessible to the children. The children need to be taught how to play with the materials and you will need to have multiple activities being played with at the same time. This will be with the children split up into small groups playing with different things. -As we discussed during my last visit, you must lock or store all medications/creams/ointments, etc... out of reach, at least 5 feet from floor. -All children must have the required components for each meal, as stated by the USDA guidelines. Fluid milk is a required component for lunch meal. You can find all the lunch meal components on their website at https://www.fns.usda.gov/cacfp/nutrition-standards/lunch-supper-meal-pattern -All required forms and paperwork must be kept on file for all children and staff. The written plan of care must have the required information on it, even if you do not use the DCDEE form. Parents must know where their children are at all times, along with who is caring for them when you are not present and what those times are going to be. When they change at all, you will need to update the form. We discussed the following items of consultation during today's visit: -Compliance history: Your compliance history before today's visit is at 71%. You must have a compliance history above 75% to apply for a star license. When you received your Child Care License from the State of NC, you are agreeing to comply, at all times, with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn and maintain All Child Care Requirements that apply to your licensed facility. We discussed that your compliance history will be re-run after today's visit and that if it is still below 75%, a possible administrative action up to a revocation or denial of a star license could be taken, depending on what the Raleigh office decides. -You had questions about meal components and we discussed that you must have a meat/meat substitute, vegetable, fruit, grain and fluid milk. You had questions regarding things that your food program representative was telling you and we suggested that you referred to your manual and contact a supervisor from that program with questions. -You were given the opportunity to ask all child care related questions. You stated that you had been to a training on Pathways that was offered by the Health Nurse Consultants in Guilford County. You must correct all violations immediately and submit a compliance letter no later than 5/5/26, stating how the violations are corrected. The license was issued 12/9/25-6/9/26 but Ms. Shayla Boyd stated that she didn't want to start caring for children until January 2026. Upon arrival Kiara Mark, substitute was present and caring for the children. The children were all laying down for naptime. There were children laying on the 2 couches, individual cots, provider's bed and an infant bouncy seat (2 year old child.) Ms. Mark stated that the Ms. Boyd was out doing errands. Ms. Boyd called and stated that she was on her way back to the home. During today's visit I observed there were no sheets on the cots, the TVs were on in the living room and the bedroom where children were watching them as they lay down to rest. There were some age appropriate activities for the children but more are needed for the 9 children ages 2-4 years old to play with. No fire drills were recorded and the license was not posted. The following violations were cited during today's visit: Violation Number Comment Rule 542 When screen time was provided, it was not offered to stimulate a developmental domain in accordance with NC Foundations for Early Learning and Development. All children were watching cartoons while laying down for rest time. .1718(b)(1) 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. .1718(c) 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. There were no fire drills since the facility opened in January. .1721 (e )(2) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home, for 2 children. Children cannot us the couch or bed since these are not individual sleep spaces that are only used for that child for resting. And the item must be age appropriate, and a 2 year old sleeping in an infant seat is not age appropriate. -Linens must also be used, to help with sanitation and to keep children from laying directly on the rough surface/vinyl. -TV is screen time and is not allowed for children under 3 years old, and must be used only as part of a developmental activity for children 3 years and older. -Fire drills must be conducted monthly to help children learn and know what to do when they hear the sound of the smoke alarm and how to get out of the home. -Staff orientation must be documented to verify that all required items have been reviewed with the staff member. -Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff and children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. We discussed the following items of Consultation: -This must be your primary residence, as required by child care rules and the City of Greensboro to operate a home business. You stated that you and your boyfriend stay here and at his home with your daughter. After speaking with my supervisor, your boyfriend will also need a qualification letter for child care. -We discussed the pathway to the stars information for pathways 1 and 2, along with what is required for CQI and Family and community engagement. You can find all information in child care rules .3200 on our DCDEE website at ncchildcare.ncdhhs.gov and under the What's New tab, under QRIS modernization and scroll down to the information sheets. If you wish to keep DSS subsidy eligibility, you must be at least a 3-star license at the end of your Temporary Time Period. We discussed what the 3 star requirements are for both pathway 1 and 2. -Your education for the NCECC with 12 ECE semester hours is in our WORKS education system, and you stated that you do not have any experience in a licensed child care facility. Reminders: COMPLIANCE HISTORY must be above 75% to apply for a star rated license and to keep a license. When you receive your Child Care License from the State of NC, you are agreeing to comply at all times with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn, and maintain all Child Care Requirements that apply to your licensed facility. It is also your responsibility to ask Division staff questions for clarification of requirements you do not understand and to monitor your staff to ensure that all requirements are maintained at all times. Recognizing and Responding to Suspicions of Maltreatment: Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. All staff are required to complete the training within 90 days of hire. Recognizing and Responding to Suspicions of Child Maltreatment training is available on Prevent Child Abuse NC website, www.preventchildabusenc.org, free of charge, and counts toward Health and Safety Training. You must correct all violations immediately and submit a compliance letter no later than 3/3/26, stating how each of the violations was corrected. The license was issued 12/9/25-6/9/26 but Ms. Shayla Boyd stated that she didn't want to start caring for children until January 2026. Upon arrival Kiara Mark, substitute was present and caring for the children. The children were all laying down for naptime. There were children laying on the 2 couches, individual cots, provider's bed and an infant bouncy seat (2 year old child.) Ms. Mark stated that the Ms. Boyd was out doing errands. Ms. Boyd called and stated that she was on her way back to the home. During today's visit I observed there were no sheets on the cots, the TVs were on in the living room and the bedroom where children were watching them as they lay down to rest. There were some age appropriate activities for the children but more are needed for the 9 children ages 2-4 years old to play with. No fire drills were recorded and the license was not posted. The following violations were cited during today's visit: Violation Number Comment Rule 542 When screen time was provided, it was not offered to stimulate a developmental domain in accordance with NC Foundations for Early Learning and Development. All children were watching cartoons while laying down for rest time. .1718(b)(1) 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. .1718(c) 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. There were no fire drills since the facility opened in January. .1721 (e )(2) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home, for 2 children. G.S.110-91(10); .1727(a)&(b) 1601 License was not posted in a place in the home where parents are able to view daily. There was no license posted. G.S.110-99(b) & .1702(i) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file, for 2 children. GS 110-102 1712 Operator did not develop and adopt a written plan of care for completing routine tasks to ensure routine tasks did not interfere with the care of children during hours of operation. Upon arrival there was a substitute present, and Ms. Boyd stated that she was running errands. No written plan of care was on file for any of the children. .1712(a) 1828 The Family Child Care Home operator did not discuss the operational policies with parents on or before the child's first day of attendance, for 2 children. 10A NCAC 09. 1715(b) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. Multiple items including ointments were stored in low cabinets and cubbies within reach of the children. .1719(a)(7) 1891 Individuals providing care to children did not receive 16 hours of orientation prior to beginning caregiving activities, for the substitute caring for children. .1729( c) 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has, for 6 preschool children, the application did not contain Health needs information and characteristics information. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. .1719(a)(11) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. There were 4 children ages 2-4 that were sleeping on couches, the provider's bed and an infant seat and the other preschool children sleeping on cots did not have a sheet/linen on the cot. 10A NCAC 09 .1718(a)(2) The following technical assistance items were discussed during today's visit: -Cots or mats or an individual sleeping item must be used for each child. Children cannot us the couch or bed since these are not individual sleep spaces that are only used for that child for resting. And the item must be age appropriate, and a 2 year old sleeping in an infant seat is not age appropriate. -Linens must also be used, to help with sanitation and to keep children from laying directly on the rough surface/vinyl. -TV is screen time and is not allowed for children under 3 years old, and must be used only as part of a developmental activity for children 3 years and older. -Fire drills must be conducted monthly to help children learn and know what to do when they hear the sound of the smoke alarm and how to get out of the home. -Staff orientation must be documented to verify that all required items have been reviewed with the staff member. -Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff and children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. We discussed the following items of Consultation: -This must be your primary residence, as required by child care rules and the City of Greensboro to operate a home business. You stated that you and your boyfriend stay here and at his home with your daughter. After speaking with my supervisor, your boyfriend will also need a qualification letter for child care. -We discussed the pathway to the stars information for pathways 1 and 2, along with what is required for CQI and Family and community engagement. You can find all information in child care rules .3200 on our DCDEE website at ncchildcare.ncdhhs.gov and under the What's New tab, under QRIS modernization and scroll down to the information sheets. If you wish to keep DSS subsidy eligibility, you must be at least a 3-star license at the end of your Temporary Time Period. We discussed what the 3 star requirements are for both pathway 1 and 2. -Your education for the NCECC with 12 ECE semester hours is in our WORKS education system, and you stated that you do not have any experience in a licensed child care facility. Reminders: COMPLIANCE HISTORY must be above 75% to apply for a star rated license and to keep a license. When you receive your Child Care License from the State of NC, you are agreeing to comply at all times with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn, and maintain all Child Care Requirements that apply to your licensed facility. It is also your responsibility to ask Division staff questions for clarification of requirements you do not understand and to monitor your staff to ensure that all requirements are maintained at all times. Recognizing and Responding to Suspicions of Maltreatment: Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. All staff are required to complete the training within 90 days of hire. Recognizing and Responding to Suspicions of Child Maltreatment training is available on Prevent Child Abuse NC website, www.preventchildabusenc.org, free of charge, and counts toward Health and Safety Training. You must correct all violations immediately and submit a compliance letter no later than 3/3/26, stating how each of the violations was corrected.
