An Annual Compliance Monitoring Checklist for Centers checklist was used to note the requirements I monitored today. The facility has an 18-month compliance history of 100%. Currently this center located in a residence operates with a Four-Star License, issued 12/18/24. Upon arrival, I was greeted by Ms. Yolanda Short, Ms. S. Whitney arrived during the visit. A full review of child care requirements was conducted during the visit. There were three children present. I observed the children napping on cots. A satisfactory fire inspection was conducted on 4/9/26. A sanitation inspection conducted on 2/27/26 resulted in a “Superior” rating. Documentation of fire drills, shelter-in-place drills, and playground inspections was reviewed and found to be current. One violation of child care requirements was cited during today’s visit. Violation Number Comment Rule 1328 Children's records were not made available for review. The children's files were in a locked file cabinet. The teacher did not have access to the files. G.S. 110-91(9) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before June 12, 2026. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If I do not receive your letter by June 12, 2026, a return visit to your program will be made to monitor for compliance of violations and to obtain compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. Pathways to the Stars We reviewed the updated rules in Section .3200 regarding the Standards for Two- through Five-Rated facilities (Pathway to Stars). As discussed, the facility should carefully consider the available QRIS options and determine which pathway it plans to pursue for its rated license reassessment. While the reassessment process may begin at any time, the facility should be prepared to submit the Application for Assessment for a Rated License between April 1, 2026, and September 30, 2026. Your facility’s transition will begin in 2027. Today you stated that you are still deciding which Pathway but considering Pathway 2: Classroom and Instructional Quality If you choose Pathway 2: Classroom and Instructional Quality, please complete the following steps: • Submit the “Application for Assessment for a Rated License for Centers” electronically to your child care consultant no later than September 30, 2026. • Confirm that all staff members have active WORKS accounts. • Complete the QRIS Staff Information and Education Worksheet electronically. • Identify Continuous Quality Improvement (CQI) goals and begin planning for implementation. • Develop strategies to strengthen Family and Community Engagement. • Select and implement a curriculum. (Four- and Five-Star centers must use an approved curriculum and a formative assessment tool for all age groups.) • Arrange coaching and/or training opportunities for administrators and lead teachers. Ericka Bailey, Child Care Consultant, accompanied me during today’s visit. Upon arrival the children were observed transitioning to naptime. Today we met with Salyna Whitney. We conducted a walk-through of all areas used by the children both inside and outside. The following items were monitored during today's visit: Supervision, Staff/Child Ratios, Required items posted, storage of hazardous items, safe environment, staff, child and program records. The last fire drill was recorded on 11/15/24 and the last playground inspection was recorded on 11/4/24. The following violation was cited during today’s visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. The Substitute that was present during the visit did not have a complete file available for review. G.S. 110-91( 9) We discussed the following item of technical assistance: -All staff must have TB tests, physicals and criminal background checks completed prior to employment. We discussed the following items of consultation during today's visit: -Your staff's education must all be at the 4 point level or higher to help you earn at least a three star license. You stated that the new teacher has enrolled in EDU 119 and will have it completed by December 2024. -We discussed several questions that you had about staff/child ratio and grouping children now that you have an infant enrolled. The infants and one year olds can be grouped together in the one classroom space with ratios being 5/1 and the 2 year olds and up can be grouped together in the 2nd classroom space with ratios being 7/1 for a total of 12 children. Infants cannot be grouped together with children over 2 unless there are no more than 5 children in the group. -Several resources including but not limited to Building Bright Futures program, https://www.buildingbrightfuturesnc.org/ -We discussed that when children either don' t sleep after 20 minutes or wake up early from naptime, must be given a quiet activity to do until rest time is over. . Reminders: In order to continue to receive DSS vouchers at the end of your Temporary License period, at the minimum: all staff members must meet 4 points in education, you must meet 2 points in program with meeting minimum requirements and either enhanced space or enhanced ratios and you must meet at least 1 quality point item. This is the first visit conducted since the license was changed to a CLIR on July 15, 2024. During today's visit the children were observed napping. I conducted a walk-through of all areas used by the children both inside and outside. The following items were monitored during today's visit: Supervision, Staff/Child Ratios, Required items posted, storage of hazardous items, safe environment, staff and program records. The last fire drill was recorded on 8/16/24 and the last playground inspection was recorded on 8/6/24. The following violations were cited during today's visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was a can of aerosol disinfectant in the bathroom in an unlocked cabinet. This was corrected during the visit. .2820(b) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One new staff member did not have a medical report on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One new staff member did not have any proof of TB results. .0701(a) 1301 Center did not maintain a record of daily attendance The attendance record was not completed for 9/4 or 9/5. This was corrected during the visit.
