Name of Operation: Kiddos Happy Learners Childcare Facility ID: 41002534 Consultant: MARY MCMILLAN Operation Type: Family CC Home Case Number: Visit Date: 1/15/2026 Number Present: 2 Completed Date: 1/15/2026 Age: From 2 To 3 Total Minutes: 75 Time In: 11:30 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period visit. The First Temporary License was issued on 8/18/25. Ms. Elizabeth Lora was present and caring for children. Elizabeth Childress, Child Care Consultant, accompanied me during today's visit. Upon arrival the children were playing with playdough and napping. Limited items were monitored during today's visit: Capacity, Supervision, items posted, permit posted and restrictions met, approved space and hazardous items storage, one new child's file was reviewed today. The last fire drill was recorded on 12/18/25 at 3:50pm with 1 minute evacuation time. The following violation was cited today: Violation Number Comment Rule 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has, for one child. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) We discussed the following item of technical assistance today: You must have all items required to be on file for each child, including all items completed/addressed on the children's application in some way. We discussed the following items of consultation during today's visit: -You stated that you are using the Gee Whiz curriculum -You have 3 years of experience working in child care and at this time do not have any education. We discussed that you could complete the competency evaluation or CEU's and be a 2 star facility, if you wish to do so, however you will not be able to be eligible to receive child care subsidy vouchers unless you meet the requirements for at least a 3 star license. Currently you meet the 1 star education level. You stated that you plan to enroll in EDU 119. Therefore, after your completion, you can re-apply for additional stars with a rated license. You can find the FCCH education standards for star levels in rule .3217 You must correct any violations immediately and submit an emailed compliance letter stating how the violation is corrected by 1/29/26.
