A checklist was used to note the requirements I monitored today. This center currently operates with a Five Star license reissued on June 13, 2018. This facility is owned by Moses Cone Health Care System and is located on the campus of Wesley Long Hospital. The center is managed by Bright Horizons. Upon arrival to the facility, I was greeted by the assistant center administrator, Ms. Diamond Brown. I was also greeted by the center administrator, Ms. Amy Love. I made Ms. Love aware of the purpose of today’s visit. Ms. Love and Ms. Brown assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was monitored. Eighty (80) children were present during today’s visit. During today’s visit I observed the children during center play, small group activities and outdoor play. I also observed mealtime and rest time. I observed one infant feeding and diaper change. Several transitions were also observed. Positive interaction was observed between the staff and the children. In each classroom space sufficient materials were observed accessible to the children. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. A sanitation inspection was conducted on November 26, 2024. This facility received an superior classification and six (6) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. A satisfactory fire inspection was completed on August 2, 2024. This facility was approved for daytime care only. This facility’s corporation status was verified current on the NC Department of the Secretary of State on April 7, 2025. The following violation of childcare requirements was cited today. Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. This facility's incident report did not included all the required information as required. .0802 (e) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by April 23, 2025. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. Health & Safety Requirements were also reviewed. INCIDENT REPORT: An incident report must be completed any time a child is injured as a result of an incident that occurs while the child is in care. The report must be signed by a parent/guardian and kept on file. The report must be submitted to your Child Care Consultant within seven (7) days if the child receives medical attention as a result of the incident. It is recommended that you use the DCDEE Incident Report form, but if you use your own version, it must contain all the required information. During today’s visit a review of your incident report revealed that all required information was not included in the report. This incident report must include: (1) facility identifying information; (2) the child's name; (3) date and time of the incident; (4) witness to the incident; (5) time the parent is notified of the incident and by whom; (6) piece of equipment involved, if applicable; (7) cause of injury, if applicable; (8) type of injury, if applicable; (9) body part injured, if applicable; (10) where the child received medical treatment, if applicable; (11) description of how and where the incident occurred, and the First Aid received; and (12) steps taken to prevent reoccurrence. --A copy of the form may be found on the Division's website at: http://ncchildcare.ncdhhs.gov/pdf_forms/DCDEE-0058.pdf --This report must be signed by the person completing the form and by the parent/guardian. A copy given to the parent or the parent declining a copy and the report must be maintained in the child's file. --Please keep in mind the importance of having a detailed account of what happened during the incident so the parent/guardian can have a complete account of the incident, care received and the steps the facility will take to prevent a reoccurrence of the incident. It is recommended that staff receive additional training on the thorough completion of this form. --A complete review of the rule can be found in section 10A NCAC 09 .0802 of the Child Care requirements. STAFF ANNUAL TRAINING REQUIREMENTS: You stated today that your staff completes training from January-December 31st of each year. HEALTH & SAFETY TRAINING: The CCDF Health and Safety trainings are now available in the DCDEE Moodle learning platform. The CCDF Health and Safety in Child Care Trainings, developed by the North Carolina Child Care Health and Safety Resource Center, are FREE and easy to access. The trainings will no longer be available on the Pro-solutions Website for free. You will still need to complete the Recognizing and Responding to Suspicions of Child Maltreatment training through Positive Childcare Alliance website (www.preventchildabusenc.org) within 90 days of employment. Moodle can also be accessed https://www.dcdee.moodle.nc.gov/ or thru DCDEE's homepage https://ncchildcare.ncdhhs.gov/ As reviewed with you today, all staff members must complete Health and Safety training within one year of employment, unless the staff member has completed the training within the year prior to beginning employment. Health and Safety training must be completed in addition to the new staff orientation requirements. --Complete training in 11 topic areas (not all may apply to your center) --Document on Health & Safety Training Record (available on DCDEE website) --After the first year, you must complete training in each topic area at least every five years, except for CPR/First Aid (every 2 years) and Safe Sleep Training (every 3 years) --Keep in mind this training will also count for annual training the year you complete the training --It is recommended that you print your certificates as you complete each course. --It is also important to note that if you don’t complete the training by the end of December you may not have access to the completed training courses in the new year. **For complete details of these requirements review section .1102 in the Child Care Requirements. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A final review of the new QRIS system will be shared with licensed programs soon. You completed the Environmental Rating Scale (ERS) last July 2024. Your scores will be good for three years. You stated that you may want to go ahead and complete a reassessment. I will verify that your license can be renewed. Please keep in mind that if we process your reassessment you will no longer be in the “Hold Harmless” classification and will have to maintain all staff education requirements. Please continue to make sure all have updated educational information submitted to our Workforce system. All staff will need to have a current Workforce letter on file. RECORD RETENTION: Record Retention is important for child care programs not only to keep good records, but also to know which ones to retain and for how long. We discussed the requirement of keeping these items for a minimum of at least 30 days to be viewed during any visit to the facility. Refer to Section .2318 Child Care Center Record Retention to keep required documents for the timeframe identified within the rule. A checklist was used to note the requirements I monitored today. I also monitored health and safety requirements. This facility currently operates with a Five Star License, effective June 13, 2018. The facility is owned by Moses Cone Health Care System and is located on the campus of Wesley Long Hospital. The center is managed by Bright Horizons. The facility operates from 6:00 am to 6:00 pm. Upon my arrival, I was greeted by you, Amy Love, the administrator for the program. You accompanied me during the walk-through of the indoor and outdoor spaces used by the children. A full assessment of applicable child care requirements was conducted during today’s visit. You informed me the email for the facility has changed. I updated the email in the Regulatory System. A fire inspection was completed on July 19, 2023. The report was marked Satisfactory, and the facility was approved for daytime care. The most recent sanitation inspection was completed on March 15, 2024 and received a classification of Superior with four demerits. Seventy-seven children were present today. I observed the preschool children engaged in various activities during free choice center time and playing outside. In the classroom for infants, I observed the one teacher holding an infant and one teacher with four infants were playing with available toys on the floor. Two infants were sleeping in cribs. Two violations of child care requirements were cited. One violation was marked as corrected. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The child care operator did not notify the Division of the new employee who were hired on 4-15-24 within five business days by submitting the Change of Information form provided by the Division. The Change of Information form has not been submitted. G.S. 110-90.2 & .2703(r) 1902 The professional development plan was not reviewed annually. The professional development plan for two employees should have been reviewed in November 2023 as documented from the previous consultant on the staff and training worksheet. The professional development plans were reviewed in January 2024. .1104 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed sharon.y.williams@dhhs.nc.gov on or before May 9, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Clean Classrooms for Carolina Kids All North Carolina licensed child care centers must enroll and complete the enrollment surveys for lead-based paint and asbestos of the Clean Classrooms for Carolina Kids™ program by May 1, 2024. The program is providing free facility-wide inspections without any out-of-pocket cost that meets all rule requirements from program launch to November 30, 2026. The webinar will provide information about lead and asbestos hazards, state requirements, personnel and facility information needed for the program, commonly asked questions, and tips for success. ABCMS Provider Portal Provider Access Available to the Automated Background Check Management System As of February 1, 2024 child care administrators can access ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Please complete the process for your facility as soon as possible. Rated License Re-assessment Process Your facility is in Cohort One. Your facility’s preparation year began on July 1, 2023 and will end on June 30, 2024. I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. • Today we discussed your staff’s education. Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS). • You stated that you would like to have a preparation ERS assessment.
