Applicable childcare requirements and the inspection were discussed with Administrator, Sequoya Moore. Currently this facility operates with a 3-star license effective 11/7/25. During the visit I monitored the indoor and outdoor spaces used for child care. I observed children during routine activities and rest time. Age-appropriate materials and equipment were accessible to all children. The facility was monitored for compliance using the annual compliance monitoring checklist. I also monitored permit restrictions, posting requirements, training requirements, use of approved space, program records, and nutrition. One child’s records was reviewed, and staff records were reviewed along with the staff and training worksheet. The last fire inspection was completed 4/16/26. The last fire drill was completed on 6/4/26. The last shelter-in-place drill was completed 6/5/26. The operation's EPR plan was last updated 06/24/26. Outdoor inspections were observed to be completed monthly. Sleep checks were observed to be completed. A report for 6/22/26 through 6/24/26 for all sleep checks was observed. Transportation is not provided by this facility. I observed 1 violation during this inspection. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A six foot balance beam was propped against the building on the playground. 10A NCAC 09 .0601(a) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by July 7, 2026, stating that all violations have been corrected. The letter can be emailed to elizabeth.childress@dhhs.nc.gov. Technical assistance was provided on the following: Intentional planning of the outdoor environment ensures a safe environment has been created which prevents and reduces injuries to young children. There was a six foot balance beam propped against the building on the playground. The administrator stated after mowing, the balance beam was not placed back on the ground. A gate was placed at the steps to prevent children from entering the area. We discussed the following consultation items today: During today’s visit, we discussed your transition to the new QRIS Rated License. Your next pathway transition will be in 2028. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Today I met with Sequoya Moore, Owner/Administrator. During today’s visit there were 5 children present and were observed during rest time. During today’s visit I observed the following limited items: Staff/Child Ratio, Supervision, Staff records, items posted, permit and restrictions met, age-appropriate activities, safe environment and approved space. The last fire drill was recorded on10/5/25 at 3pm with 1 minute evacuation time. Ms. Moore requested that I re-measure space #2 since she is removing the couch. The room measures 10x10 and will hold 4 children at the minimum space requirements and 3 children at enhanced space requirements. One violation was cited during today’s visit, and was corrected during the visit. Violation Number Comment Rule 1301 Center did not maintain a record of daily attendance for classroom #1. GS 110-91(9) We discussed the following items of consultation today: -Pathways to the Stars information. Ms. Moore stated that she understood more after the training that I conducted on the pathways, and she is interested in pathway 2: Classroom and Instructional Quality. -We reviewed the CQI information and use the curriculum training for staff CQI goals and purchasing the curriculum as the center goal. -I previously emailed Ms. Moore an update on the items in WORKS for both her and Jamaya Blanks, Lead Teacher. Ms. Moore: has education for 5-star level: Bachelors in other with NCECC and 26 hours in EC with at least 5 years of experience; Jamaya Blanks: has only high school education in WORKS but does have college transcripts to submit. Your volunteer, Ms. Sandra Moore’s education will not be looked at. You can find the education requirements in Rule .3217 on our DCDEE website at ncchildcare.ncdhhs.gov -We discussed family and community engagement and curriculum options that are requirements for this pathway and reviewed samplings of options available. This was along with having a mentor/coach and conducting child observations -You stated that you are looking at curriculums and will make a decision by then end of the week and purchase a curriculum with assessment. -You stated that you understand that you must have your rated license application with all required forms submitted to me no later than October 27, 2025. You will have to have all items under the star level that you wish to meet, in order to receive those stars. In order to receive DSS vouchers, you must be at least a 3-Star license at the end of your Temporary License, 11/17/25. No letter of compliance is required, since the violation was corrected during the visit. There are now two classrooms of children, Space #1 has children 2-years and older with 1 teacher and Space #2 has infants and 1-year olds present with 1 teacher. There was a volunteer present cooking lunch. The following items were monitored during today's visit: Supervision, Staff/Child Ratios, required items posted, storage of hazardous items, safe environment, staff records. Children were being cared for in a nurturing manner. The following violations were cited during today's visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference.
