A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on June 10,2024. Upon my arrival, I was greeted by Ms. T. Hall, the lead teacher. I introduced myself and informed Ms. Hall the purpose of today’s visit. Ms. Hall gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Thirteen (13) children were present during today’s visit. During today’s visit, I observed the children participating in a literacy activity ,washing their hands in preparation for lunch, the finishing up lunch, napping waking up from napping and playing in centers. All indoor space used by the children were observed today. I was unable to monitor the playground due to the weather. All required items were posted. A sanitation inspection was conducted on September 11, 2025. This facility received a superior rating with nine (9) demerits. A fire inspection was conducted on June 17,2025. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Two(2) violations were cited during today’s visit. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan was not reviewed with all staff annually. The plan also needs to be revised. 10A NCAC 09 .0802(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) The above violations must be corrected immediately, and a letter must be emailed to me no later than January 28, 2026, stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by January 28,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: Technical Assistance: Today I reviewed with Ms. Hall when an incident report and log need to be completed. I explained to Ms. Hall an incident is when a child bumps or falls into another child or a piece of furniture then the incident form and log need to be completed. Also, if a child receives medical treatment from a physician, then you must submit an incident log to me within seven days. Rated License Assessment: I discussed with Ms. Hall license assessment process for the Hold Harmless is now over. The new QRIS has been adopted effective July 1,2025. I informed Ms. Hall that the NC Pré-K administration has chosen Pathway 1. This facility will renew their rated license by June 2027. WORKS: Please ensure the education information is in the W.O.R.K.S. database. Ms. Edmondson needs to complete the education process in W.O.R.K.S. to update her education. I reviewed the instructions on the DCDEE website. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed to ensure your staff meets minimum education requirements. This NC Pre-K program operated by Guilford County Schools, currently operates with a Five Star license issued on June 10, 2024. This elementary school has one NCPK classroom. I was greeted by Ms. K. Vanada, teacher assistant. Ms. T. Hall, lead teacher was absent today. Ms. Eleanor Drake, substitute was present. There were 11 children present today. They were observed listening to a story, completing bathroom routines, eating lunch and resting on cots. Interactions observed between the teachers and students were positive and nurturing in manner. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. The NC Pre-K Program Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Indoor areas, program requirements, and staff and children’s records were monitored. Current fire drills, shelter-in-place drills, and playground inspections were documented and current. The sanitation inspection was completed 9/11/24 with a “Superior” classification. The fire inspection was conducted and there were no violations cited. Required documents were posted and or accessible. I observed items of non-compliance during today’s visit. Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. E. Drake, substitute did not have the required Emergency Information Form on file on or before the first day of work. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance).
