Name of Operation: JONES ELEMENTARY ACES PROGRAM Facility ID: 41001533 Consultant: CHERISSA ALSTON Operation Type: Center Case Number: Visit Date: 11/24/2025 Number Present: 78 Completed Date: 11/24/2025 Age: From 5 To 11 Total Minutes: 60 Time In: 02:45 PM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance of child care rules during an annual compliance visit. The facility has an 18-month compliance history of 98%. The last annual compliance visit was documented on 3/6/25. Upon arrival, I was greeted by Roosevelt Dixon, Site Coordinator. I monitored the licensed space, noted attendance and monitored staff/child ratios. There were 78 children present today, ages 5-11 years old. Children were observed eating snack and completing homework or having quiet time at the table. Interactions observed were positive and nurturing in manner. I monitored arrival and departure documentation, current activity plans, schedules, a current menu and allergy list was posted in the space. Fire drills were documented monthly and other emergency drills were documented quarterly. Required items were posted and current. There was one item of non-compliance observed today and the corresponding violation was cited. Violation Number Comment Rule 205 Storage space was not available for each child's personal belongings. There was not enough cubby space for each child's belongings. There were 72 cubbies available, 78 children present and 87 children enrolled. G.S. 110-91(6) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 12/8/25, describing how and when the violation(s) were corrected. Please remember your letter should include facility name, facility ID#, and original signature. The letter can be emailed to cherissa.alston@dhhs.nc.gov. Today, I discussed the rated license process and timeline with you. We completed the pathway discussion template. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements.
