This center currently operates with a Three-Star License effective April 13, 2020. Upon arrival, I was greeted by Ms. T. Tufts. Ms. M. Johnson arrived to assist me with today’s visit. A partial assessment of child care requirements was conducted. This center is open with dividers for classrooms. I conducted a walkthrough of the center; I observed the children and gathered attendance. I observed ratios/children, interactions, and activities. There were twenty-one children present today; the children were playing in centers participating in group time, completing handwashing routines, eating lunch, and transitioning to rest time. The latest sanitation inspection was conducted on January 13, 2026. The center received a superior classification. Three items of noncompliance with child care requirements were cited during today’s visit. Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbent material and/or were not kept clean and in good repair. The carpet in the classroom was observed to be separating at the seams with loose strings. 15A NCAC 18A .2824(a)&(b) 705 Equipment and furnishings were not sturdy, stable and free of hazards. Toys in the classroom were stored in a white wicker basket that was broken with jagged edges. .0601(c) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB test for the staff member who began employment in January 2026 was more than 12 months old. .0701(a) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before February 26, 2026. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If I do not receive your letter by February 26, 2026 a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. TA: The carpet in the classroom was observed to be separating at the seams with loose strings. Floor covering must be kept clean and in good repair. The use of area rugs to cover worn or damaged portions of carpet was discussed. While rugs may provide temporary coverage, they must lie flat, be secured to prevent slipping or curling, and not create additional tripping hazards. Pathways to the Stars During today’s visit, information regarding the Pathway to Stars program and the updated rules in Section .3200 was provided. Instructions were shared for accessing the relevant resources on the DCDEE website. I informed you that the new rule rollout module is available on the DCDEE e-learning Moodle platform for review. Today you stated that you are considering Pathway 2: Classroom and Instructional Quality Pathway. Pathway 1: Program Assessment Focus: Overall program quality and classroom environment • Programs are assessed using a formal classroom observation by NCRLAP. I completed an Annual Compliance Monitoring Checklist for Centers during today’s visit and a completed copy was emailed to you. Jen Jones, Child Care Consultant accompanied me on today’s visit Upon arrival, we were greeted by M. Johnson. Ms. Johnson accompanied us on the walk-through of the facility. There were thirty-three children present today. Children were observed engaged in group times, playing in a variety of centers, outdoor activities and eating lunch. One infant was sleeping, toddlers were playing with soft toys and interacting with staff. Children were being attended to according to their individual needs. The staff were observed engaged in activities with the children and were positive in their interactions with the children. Your program currently operates with a three-star license, issued 4/13/2020. The last annual compliance visit was conducted 8/12/24. The sanitation inspection was completed 7/8/2025 with a “Superior” classification. The last fire inspection was conducted on 11/22/24 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety nine percent as of 7/28/2025. The NC Secretary of State website was reviewed on 07/28/2025 and Johnson’s Day Care was listed as current- active. We monitored the bus and no hazards were observed. The registration and proof of insurance were monitored and found to be current. The driver has a current driver’s license, and First Aid and CPR certification. Photos were not with the identifying information. We reviewed five children's files and three new staff files. One violation was observed during today’s visit. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. This center currently operates with a Three-Star License effective April 13, 2020. Limited licensing requirements were monitored. I was greeted by Ms. Richardson, LT for toddlers . New staff are working on their required training courses. Remember to complete the Orientation sheet as you complete the new staff members orientation. The newest hire does not have a current CBC Qualification. Ms. Johnson is helping her with the process. This center is open with dividers for classrooms. Ms. Johnson was preparing lunch. I conducted a walkthrough of the center; I observed the children and gathered attendance. I observed ratios/children, interactions, and activities. There were twenty-two children present today; the children were playing in centers and having group time. Supervision and staff/child ratios are in compliance. In the classrooms children have many activities to choose from. The classrooms are neat, organized and clean. The playground has activities, shaded areas and room for vigorous play. The latest sanitation inspection was conducted on January 17, 2025. The center received a superior classification. I observed one item of non-compliance during the visit today. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A new staff has a CBC qualification letter, but is has expired. G.S. 110-90.2(b) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a letter of correction by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by February 11, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. This program currently operates with a Three Star License, effective April 13, 2020. Upon my arrival I was greeted by you, Margie Johnson, Administrator. You assisted me with the documentation required during the visit. A fire inspection was conducted May 24, 2022. The facility received a rating of Satisfactory and is approved for day time care only. The most recent sanitation inspection was conducted on June 22, 2023 and received a Superior classification and ten demerits. I completed a walk-through of the indoor and outdoor areas of the facility that the children use. Eighteen children were present today. I observed the preschool children playing outside, engaged in free choice play in activity centers, completing handwashing and bathroom routines and eating lunch. In the space for infants and toddler-age children, I observed the caregivers providing routine care. I also observed both caregivers sitting in the floor and interacting with the children in a nurturing manner. A full assessment of applicable child care requirements was conducted during today’s visit as per the attached checklist. The following violations were observed and documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Last fire inspection was completed 05/24/2022. The updated inspection has been completed. 10A NCAC 09 .0304(a) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. .0802(h) 881 The evacuation crib or other approved device did not have wheels or a reinforced bottom. Provider replaced wheels to reinforce bottom on fire evacuation crib. .0604(r) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A letter stating how the violations were corrected and how you plan to maintain compliance in the future must be submitted to me by September 26, 2023. New Child Care Sanitation Rules Children’s Environmental Health has set up virtual trainings for child care center operators on the newly readopted child care sanitation rules. The dates for the trainings are September 20, 2023 and September 26, 2023; both scheduled from 9:00 am – 11:30 am. The changes are within the rules for: staff/child ratios and group sizes .0713, cooperative arrangements .2513, and enhanced space requirements .2809.
