Upon my arrival, I was greeted by Ms. J. Jones, the owner/lead teacher. I informed Ms. Jones the purpose of today’s visit. Ms. Jones gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Nine (9) children were present during today’s visit. They were observed playing on the floor with available toys, being held, being diapered and working on colors. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions CPR and FA certification. There were (2) two violations cited during today’s visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One provider did not complete her qualification letter before the five year expiration date. G.S. 110-90.2(b) & .2703(n)&(o) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. Two child care provider did not complete ITS-SIDS training every three years. .1102(f) The above violations must be corrected immediately, and a letter must be sent to me no later than June 12,2024 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov If I do not receive your letter by June 12,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Technical Assistance: Today I explained to Ms. Jones that she needs to ensure the five year criminal recheck will need to be conducted before the expiration of the previous letter. Today we also discussed the requirement for a volunteer. A volunteer will need a Tb skin test, health questionnaire and emergency information. If this volunteer is being using in the staff/child ratio, then the volunteer all the same requirements as an employee. This volunteer will be an uncompensated worker. Rated license information: I also discussed with Ms. Jones the Rated license assessment process will begin July 1,2024 next year. I informed Ms. Jones this facility is in Cohort 2. I informed Ms. Jones that since she has enrollment that requires an additional caregiver. I explained to Ms. Jones any additional caregiver must meet the same educational point level as the previous caregiver. Your facility has earned four points in education. The additional caregiver needs to increase her education. I discussed with Ms. Chapman the option of testing out. I will email Ms. Chapman this information again since the process has changed. I discussed with Ms. Jones if your additional caregiver does not increase their education, then this facility will receive three (3) points in education. This will give you six (6) points which will be a Two star rated license unless you have the Environmental Rating Scales and score at least a 4.0 or higher in order the maintain your Three Star license. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). A checklist was used to note the requirements I monitored today. This facility currently operates with a Three Star license issued on March 23,2019. Upon arrival to the facility, I was greeted by Ms. J. Jones, the owner/lead teacher. Ms. Jones assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit the children were observed arriving, playing with available toys and listening to a story. Ten (10) children were present during today’s visit. A sanitation inspection was conducted on July 14, 2023. This facility received a superior rating with zero (0) demerits. A fire inspection was conducted on July 12, 2023. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 525 Menus for all meals and snacks were not planned at least 1 week ahead and dated. 10A NCAC 09 .0901(b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. .1102(d) The above violations must be corrected immediately, and a letter must be sent to me no later than November 17,2023 stating how each item was corrected and a plan to maintain compliance with the requirement. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Jones that the staff members need in-service training hours. It-SIDS, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. You need to email the training certificates once staff has completed the training hours. New rules: I explained to Ms. Jones the hold harmless that allows facilities to remain at their current star level without a reassessment will end June 30, 2024. I also discussed with Ms. Jones the Rated license assessment process will begin July 1,2024 next year. There is an observation assessment that you can contact the rated assessment project and a preparation assessment you with me. I informed Ms. Jones this facility is in Cohort 2. You must let me know if you ready to start the process before June 30,2024. Rated license information: I informed Ms. Jones that since she has enrollment that requires an additional caregiver.
