The last annual visit was on 5/9/23. This facility has a compliance history of 95%. I was greeted by L. Hicks, Program Coordinator. Mrs. Hicks accompanied on a walkthrough of the licensed spaces and assisted me with required documents. During the walkthrough, children were observed arriving and putting away their belongings, washing hands and gathering for a group activity. Centers were set up in the cafeteria and there were a sufficient amount of materials available. I monitored ten children's files, medications, action plans and permission to administer forms. I monitored staff files as well as monitored required information for the two substitutes on site today. Mrs. Hicks stated that they are currently working on completing on-going training for this year. A sanitation inspection was completed on 10/16/23. Playground inspections and emergency drills were current and documented. I did not monitor the playground due to rain. There were three (3) items of non-compliance observed. There were 3 violations cited today, all three were corrected during the visit. Violation Number Comment Rule 539 When screen time was provided to school-aged children, it was not offered as a free-choice activity; not used to meet a developmental goal; was not limited to 30 minutes per day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Screen time was not documented on the cumulative log or the activity plan. .2508(e)(1-5) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Substitute providers Ms. Addie and Ms. Coles did not have the form on file. .0701(a) 1958 Substitute providers and volunteers did not have a health questionnaire on or before the first day of work and annually thereafter. Substitute providers Ms. Addie and Ms. Coles, did not have a health questionnaire. 10A NCAC 09 .0701(a) Today I reviewed rated license information and the following point have been proposed: RATED LICENSE ASSESSMENT 1) Components Information Program Standards: --The facility meets minimum licensing requirements and Enhanced Space --It appears that the facility may earn 2 points in this component. Staff Education: --Administrator: 7 points --Program Coordinator: 5 points --Group Leaders: 7 points --It appears the facility may earn 5 points in this component. Quality Point --You chose this Programmatic Option: Staff Benefits Package and Infrastructure of Parent Involvement --It appears the facility may earn 1 point on this component. 2) Points Summary --Program Standards: 2 --Staff Education: 5 --Quality point: 1 Total points: 8 - Three Star Rated License 3) Compliance Facilities are required to maintain an 18-month compliance history of at least 75% as a condition of receiving a rated license. As of April 8, 2024, the compliance history for this facility was 95%. 4) License Summary It appears the facility may be eligible for a Three Star Rated License. Authorization and final determination about the issuance of a license is made at the office of the NC Division of Child Development and Early Education, Raleigh NC. 5) Maintaining the Star Rating The NC Child Care Requirements for maintaining the facility's star rating are found in NC Child Care Requirement .2830. Please note that (b & c) states the length of time a facility has to correct non-compliance of any requirement related to the star rating. For more information, please refer in the NC Child Care Handbook to Chapter 17, the Star Rated License. Staff changes may affect the facility's star rating. If you hire a new staff person, you may contact me to find out if an education evaluation has been previously conducted on her/him. Depending upon the findings, you may need to submit the appropriate documentation and supporting documents to the Workforce Education Unit at the Raleigh office of the NC Division of Child Development. Upon arrival, I was greeted by Mrs. L. Hicks, Site Coordinator. I stated the purpose of my visit and went to the licensed space where the children enrolled in ACES were gathered. Children were observed arriving and waiting to put away their belongings, they were also observed using the restroom and completing homework or reading books. There were 94 children present and there were five (5) group leaders presents. There were four groups today, Group 1 had 25, Group 2 had 25, Group 3 had 24 and Group 4 had 20 children present today. There was one (1) item of noncompliance confirmed during today’s visit. The corresponding violation was cited. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Based on information received, there have been days when staff/child ratios and group sizes were not met. GS 110-91(7);.0713(a-d) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 9/26/23 describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. -Report regarding staff/child ratios – This facility meets enhanced space qualifications. The staff:child ratios for children ages 5 years and older is 1:25. During my visit, each group was found in compliance with 20 – 25 children in each group. I spoke with staff regarding staff:child ratios and it was confirmed that at times, due to a lack of group leaders, they have had more than 25 children in a group. I confirmed that there are two new group leaders hired but waiting on qualification letters. Based on information gathered today, I was able to confirm the report of violations regarding staff:child ratios. I will return within two weeks to verify continued compliance regarding staff:child ratios. Due to connectivity issues, I was unable to print during the visit. Thank you for your time today.
