A checklist was used to note the requirements I monitored today. This is a Center Located in a Residence and currently operates with a Three Star license issued on February 19, 2019. Upon arrival to the facility, I was greeted by the caregiver, Ms. Camilla Burnett. Ms. Burnett assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was monitored. During today’s visit, I observed the children during center time, and group activities. Five (5) children were present during today’s visit. A sanitation inspection was conducted on July 24, 2024, and received a superior rating with eleven demerits. A fire inspection was conducted on May 1, 2024, and was approved for daytime and overnight care. Monthly playground and fire drills have been conducted and documentation was available for review. Quarterly disaster drills have also been documented. Staff and children files were monitored during today’s visit. The staff and training worksheet was updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. The bus onsite is use for summer field trips and has the required information, a fire extinguisher, and a first aid kit. Ms. Burnett stated the bus is not used during the school year, only during the summer months. I observed three violations of childcare rules. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The wooden picnic table is splintering and the paint is peeling. It must be sanded, all splintered wood removed and painted. The table is not safe for children. The fence has vines growing over the fence and through the fence. All of the vines must be removed. 10A NCAC 09 .0601(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Two children have emergency medication in the center without current or correct medical action plans. .0803(2)(a) 847 Parent's medication authorization did not include required information. Children with medical action plans for emergency medications did not have a medical authorization form completed and signed by a parent or guardian. 10A NCAC 09 .0803(4)(6-9) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by September 19, 2024. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. If I do not receive your letter by September 19, 2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. ANNUAL REMINDERS: As discussed during today’s visit the following items must be completed/updated annually. • Staff Records: Annual training, staff evaluations and development plan, fire safety training, aquatic policy, review of EPR Plan and Emergency Care Plan review. • Children Records: Updated emergency information and transportation forms yearly. These documents will be monitored for compliance during each annual compliance visit. Sample forms can be found on the DCDEE website. RATED LICENSE UPDATE: Hold Harmless has been extended until the new QRIS is implemented. This means that you do not need to schedule an assessment of your rated license during your cohort year. HEALTH & SAFETY CHILD CARE TRAININGS: The CCDF Health and Safety trainings are available on Moodle on the DCDEE website https://ncchildcare.ncdhhs.gov/. Moodle may be accessed https://www.dcdee.moodle.nc.gov/. The CCDF Health and Safety in Child Care Trainings, developed by the North Carolina Child Care Health and Safety Resource Center, are FREE and easy to access. The trainings are no longer available on Pro-Solutions Website for free. Moodle also has other trainings, and they are also free. Please keep in mind these trainings must be completed within 1 year of hire and once every 5 years. You will still need to complete the Recognizing and Responding to Suspicions of Child Maltreatment training through Prevent Child Abuse NC within 90 days of employment and again in five years. Recognizing and Responding to Suspicions of Child Maltreatment training is located on this website https://www.preventchildabusenc.org. TA for Medications: A suggestion for maintaining compliance with childcare rules pertaining to medications is monthly check all medications, action plans forms, permission forms, and expiration dates on the medications. Best practice - all medications should come to the office first to be checked for required information and that the forms are completed correctly. Check the medication expiration date and that the medication is for the correct child. Prescribed medications (1) shall be stored in the original containers in which they were dispensed with the pharmacy labels. (2) the child's name. (3) the name of the medication. (4) the amount and frequency of dosage. (5) the name of the prescribing physician or other health professional. (6) the date the medication was prescribed by the physician or other health professional. (7) shall be administered only to the child for whom they were prescribed; and (8) shall be administered according to the prescription, using amount and frequency of dosage specified on the label. A parent/guardian may give a caregiver standing authorization for up to six months to administer prescription or over-the-counter medication (as in Benadryl or fever reducer) to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. Fever reducer may have a standing authorization for children that have a history of febrile seizures. The authorization shall be in writing and shall contain: (1) the child's name. (2) the subject medical conditions or allergic reactions. (3) the names of the authorized over-the-counter medications. (4) the criteria for the administration of the medication. (5) the amount and frequency of the dosages. (6) the manner in which the medication shall be administered. (7) the signature of the parent. (8) the date the authorization was signed by the parent; and (9) the length of time the authorization is valid, if less than six months When a prescribed medication is given staff must complete the medication administration log. Make sure all prescribed medications are in the original container or have the prescription label directly on the medication, as in the Epi Pen. A picture of a label will work until the parent can get a box or label from the pharmacy. • OTC medications as in sunscreen, ChapStick, diaper creams, toothpaste are to have annual permission form completed by parent/guardian. • Emergency prescribed medications as in Inhalers, Epi Pens, Seizure medications are to have a completed Medical Action Plan, these plans must be signed by the medical provider and the parent/guardian. These are to be completed annually. The Prescription Medication Permission completed by the parent/guardian must be completed and signed every six months. Also, print the log along with permission form for staff to document when the medication is given. Best practice is not giving over-the-counter cold medications to any child. Child cannot have a standing permission for pain relievers for teething or other ailments. Medications that are prescribed to given two times a day should be given at home before attending childcare and in the evening after the child is back home. This facility currently operates with a Three Star license issued on February 19, 2019. This facility is a center located in a residence. Upon arrival to the facility, I was greeted by Ms. Camilla Burnett. Ms. Burnett assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was monitored. Three (3) children were present during today’s visit. The children were observed playing in activity areas, transition routines and completing a teacher directed art project. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. A sanitation inspection was conducted on January 31, 2024. This facility received a superior classification and fourteen (14) demerits. This facility received a superior fire inspection on May 21, 2023. This facility was approved for daytime and overnight care. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. The following violations of childcare requirements were cited today: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member did not complete a criminal background check prior to the expiration of the previous letter.(S.Foster) G.S. 110-90.2(b) & .2703(n)&(o) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Two children enrolled did not have complete, updated medical care information. .0802(c) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member did not have updated Health & Safety Training completed. .1103(b) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by May 9, 2024. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in Cohort Two, the preparation year for Cohort Two begins July 1, 2024 and the reassessment year will begin July 1, 2025. I will reach out to you prior to the beginning of the preparation year of your assigned cohort and will provide guidance to assist you as you begin the process. I also recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2025, and June 30, 2026. --Have all staff update their WORKS account to reflect all currently completed coursework (DCDEE-WORKS) --Determine whether you wish to have the Environment Rating Scale (ERS) Assessment conducted. --Request technical assistance with your child care consultant and local partners --Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) --Reach out to your local Community College to discuss educational opportunities To increase your points in the program component, you may wish to consider the Environment Rating Scale (ERS). If you would like to have the ERS conducted and need assistance with this preparation I am available to assist you with this preparation. NEW CRIMINAL BACKGROUND CHECK SYSTEM: The new Automated Background Check Management System (ABCMS) launched September 29, 2022 and can be found at https://ncabcms.nc.gov/DCDEE/Applicant/. The new Automated Background Check Management System (ABCMS) replaces the Criminal Background Check Portal as the site where applicants apply for their criminal background check to obtain a qualification letter to work in childcare. DCDEE has developed a user guide which is available at https://ncchildcare.ncdhhs.gov/Home/DCDEE-Sections/Criminal-Background-Check-Unit/Basic-Information. NEW CHILD CARE SANITATION RULES: The NC Children’s Environmental Health has re-adopted child care sanitation rules effective July 1, 2023. These rules can be found on the DCDEE website at https://ncchildcare.ncdhhs.gov/Portals/0/documents/pdf/S/Section_2800_Sanitation_of_Child_Care_Centers.pdf. REQUIRED testing for Lead & Asbestos: The new April 2022 rules require licensed child care facilities and public schools to test all drinking and cooking taps for lead at least once. They also require licensed child care facilities and public schools to inspect their buildings for lead-based paint and asbestos at least once. There is also a federal law under the Asbestos Hazard Emergency Response Act (AHERA) that requires public and non-profit schools to perform an inspection to determine whether asbestos-containing materials are present and then re-inspect asbestos-containing materials every three years. In addition to the one-time testing under the new rules, facilities must continue to meet ongoing state federal testing and inspection requirements. --Child Care Centers must complete this testing once every 3 years. Important: There is no cost for facilities to complete this testing, but you must complete these steps by May 1, 2024. You must register to attend a webinar to get reimbursement. To enroll, facilities should go to www.cleanwaterforUSkids.org/carolina to sign up for a pre-enrollment webinar and participate in the free process.
