The last annual compliance visit was conducted on 7/16/25. This facility operates with a 3-Star license issued in 2019. Upon arrival, I was greeted by Fatima Pass. I stated the purpose of today’s visit and we completed a walkthrough of the licensed spaces. There were two children present, both one years old. There are seven children enrolled. The children were observed watching a show on tv, eating lunch and receiving diaper changes. Your husband was also present in the home. Your adult grandson was also present, you stated he was just visiting, he left before I finished the visit. I reviewed a current menu and activity plans. Arrival and departure times were last documented in January. There was no current attendance documented. Fire and emergency drills were not current, the last fire drill documented was 1/20 and the last emergency drill was documented 10/15/26. I monitored the following child care requirements today; Supervision, Capacity, CPR / First Aid, • CBC Qualification • Storage of Hazardous Products • General Safety • General License Requirements • Program Records • License Posted • Fire Safety There were six items of non-compliance observed during today’s visit and the corresponding violations were cited. The last fire drill was documented 1/20/26. .0604(t); .0302(d)(5) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was a can of Febreeze air refresher on top of a hutch. Though out of reach it was not locked up. .2820(b) 1301 Center did not maintain a record of daily attendance. There were no attendance records available for 2026. GS 110-91(9) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. We discussed screen time today, screen time is only to be offered to children ages 3 and older. Screen time should part of the curriculum and be related to one of the developmental domains in the Foundations book. Today we began a discussion regarding the Pathways to the Stars. I will setup a time to come back a discuss the pathways in detail with you and Mrs. Wanda later this month. You will need to have a curriculum for Pathway 2, below are links to approved free resources. Other approved curriculums can be found on the DCDEE website. A checklist was used to note the requirements I monitored today. This facility currently operates with a Three Star license issued on March 12, 2019. This facility is a center located in a residence. Upon arrival to the facility, I was greeted by the caregivers, Ms. Wanda Pass. No children were present today, Ms. Pass informed me the facility was closed this week for vacation. A walk-thru of the indoor and outdoor spaces used by the children was monitored. A sanitation inspection was conducted on February 6, 2025. This facility received a superior rating with two (2) demerits. Staff and children files were monitored during today’s visit. The staff and training worksheet was updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. The following violations of child care requirements were cited: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Current health questionnaire was not available for the staff. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Health & Safety Requirements were also reviewed. **ANNUAL REMINDERS: --You stated your staff completed annual training from January-December 31st. As discussed during today’s visit the following items must be completed/updated annually. --Staff Records: Annual training, staff evaluations and development plan, fire safety training, fire safety training, aquatic policy, review of EPR Plan and Emergency Care Plan review. --Children Records: Updated emergency information and transportation forms --These documents will be monitored for compliance during each annual compliance visit. Sample forms can be found on the DCDEE website. STAFF & CHILDREN’S FILES & PROGRAM RECORDS: Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff & children’s files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff to ensure accuracy. As an administrator, it is your responsibility to review all employment documentation and verify that the required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A staff & children’s file checklist and a program record checklist is available on the DCDEE website under the Provider tab and provider documents links. Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements. PROFESSIONAL DEVELOPMENT/STAFF ANNUAL TRAINING REQUIREMENT: Because of the nature of their caregiving/teaching tasks, caregivers/teachers must attain multifaceted knowledge and skills. Professional development is crucial for early childhood educators as it enhances their skills, knowledge, and confidence, ultimately leading to higher quality care and education for young children, and improved job satisfaction for educators. Professional development also provides opportunities to learn about the latest research in child development, effective teaching strategies, and best practices in early childhood education. All staff members that have responsibility for planning and supervision of a childcare program; as well staff who work directly with children must complete in-service training annually. The required number of in-service training hours to be completed yearly is based on the education and/or experience of each individual staff member. --Please use the Record of In-service Training Chart to keep track of your training completed each year. (It is recommended that you use one sheet each year). Also have at least two years available for review (current year and previous year). --A sample chart was emailed to you to assist you with understanding the required number of training hours each staff must receive each year. HEALTH & SAFETY TRAINING: The CCDF Health and Safety trainings are now available in the DCDEE Moodle learning platform. The CCDF Health and Safety in Child Care Trainings, developed by the North Carolina Child Care Health and Safety Resource Center, are FREE and easy to access. The trainings will no longer be available on the Pro-solutions Website for free. You will still need to complete the Recognizing and Responding to Suspicions of Child Maltreatment training through Positive Childcare Alliance website (www.preventchildabusenc.org) within 90 days of employment. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A final review of the new QRIS system will be shared with licensed programs soon. CHANGE OF OWNERSHIP: You have been working on changing your facility to a LLC. During today’s visit you were able to show me that you have a current LLC listed on the NC Secretary of State website. During today’s visit I assisted you with completing the application and other required documents needed to complete a Change of Ownership. You will also need to get notarized document to me before I complete this process for you. You are hoping to have this change completed by August 1, 2025. Upon arrival to the facility, I was greeted by the caregivers, Ms. Wanda Pass. No children were present today, Ms. Pass informed me the facility was closed this week for vacation. A walk-thru of the indoor and outdoor spaces used by the children was monitored. A sanitation inspection was conducted on February 6, 2025. This facility received a superior rating with two (2) demerits. Staff and children files were monitored during today’s visit. The staff and training worksheet was updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. It is an important part of licensure. Review staff & children’s files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff to ensure accuracy. As an administrator, it is your responsibility to review all employment documentation and verify that the required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A staff & children’s file checklist and a program record checklist is available on the DCDEE website under the Provider tab and provider documents links. Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements. PROFESSIONAL DEVELOPMENT/STAFF ANNUAL TRAINING REQUIREMENT: Because of the nature of their caregiving/teaching tasks, caregivers/teachers must attain multifaceted knowledge and skills. Professional development is crucial for early childhood educators as it enhances their skills, knowledge, and confidence, ultimately leading to higher quality care and education for young children, and improved job satisfaction for educators. Professional development also provides opportunities to learn about the latest research in child development, effective teaching strategies, and best practices in early childhood education. All staff members that have responsibility for planning and supervision of a childcare program; as well staff who work directly with children must complete in-service training annually. The required number of in-service training hours to be completed yearly is based on the education and/or experience of each individual staff member. --Please use the Record of In-service Training Chart to keep track of your training completed each year. (It is recommended that you use one sheet each year). Also have at least two years available for review (current year and previous year). --A sample chart was emailed to you to assist you with understanding the required number of training hours each staff must receive each year. HEALTH & SAFETY TRAINING: The CCDF Health and Safety trainings are now available in the DCDEE Moodle learning platform. The children were observed playing in activity areas, transition routines and preparing for lunch. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in Cohort Two, the preparation year for Cohort Two begins July 1, 2024 and the reassessment year will begin July 1, 2025. I will reach out to you prior to the beginning of the preparation year of your assigned cohort and will provide guidance to assist you as you begin the process.
