The license was issued on 8/22/25, due to a Change of Ownership. Today I met with the Owner/Director, Rickeda Stephens. Upon arrival the children were observed napping and playing. Limited items were monitored during today’s visit: Staff/Child Ratio, Supervision, approved space, hazardous storage, 1 new staff file and required training. The last fire drill was recorded on 1-6-26 at 9:59am with 1 minute evacuation time. Mrs. Stephens stated that they are completing training on the Early Learning Matters Curriculum for the Curriculum and Instructional Quality Pathway. The following violations were cited during today’s visit. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference, there was no schedule for the classroom with infants. GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference, for the infant enrolled. GS 110-91(12); .0508(a) 843 A drug or medicine was administered after its expiration date. There was one child that has an emergency epi-pen that expired in November 2025. 10A NCAC 09 .0803(1)(d) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. .01102 (f) We discussed the following items of technical assistance today: -All classroom spaces must have a schedule for that group of children. -Each group of children must have lesson plans that are age-appropriate activities, and infants/toddlers must have individual lesson plans. -No staff member is allowed to be alone with infants without having ITS-SIDS training. If they are new or do not have the training, they must always be with another staff member until they receive the training within 2 months of working with the infants. -Emergency medication must be returned to the parent before it expires and replaced. The following items of consultation were discussed: -You will need to complete the staff and training worksheet with all current staff members listed and send to me by 1/21/26. -We reviewed the rated license application and all required forms on it for pathway #2, including the CQI center information and the family and community information. -We discussed that staff members must also complete their CQI forms, but those do not need to be submitted to me, please place them in their staff files for review. The rated license forms are needed by 1/31/26. -I received the legal designee form today and the following items are still needed by 1/21/26: -Operational and parent policies (handbooks) -Zoning letter The following items were confirmed as completed from the RTI website: -Lead Water test in September 2025 -Lead Paint testing in February 2025 -Asbestos testing in February 2025 You must correct all violations immediately and submit a compliance letter by 1/21/26, stating how the violations were corrected. The license was issued on 8/22/25, due to a change of ownership. Today I met with Rickeda Stephens, Owner/Operator. Upon arrival the children were observed during free play in centers. During today’s visit limited monitoring of child care requirements was completed: Staff/Child Ratio, Supervision, approved space, items posted, permit restrictions, age-appropriate activities, program records and a sampling of staff files were reviewed. The last fire drill recorded was on 9/9/25 at 10:45am with 1 minute evacuation time. You stated that currently you are not using side 1A of Building A, in which you wish to have 1 year olds in that space. I measured the two spaces (1A and 1B) and emailed you the space calculations. Side 1A can hold 8 children for minimum space requirements or 6 children for enhanced space. Side 1B can hold 16 children for minimum space and 14 children for enhanced space. You stated that you are currently not using Building B for child care. I monitored the building and it was not set up for child care. You stated that you are currently using the kitchenette in Building A. The menu was posted from September 2025. There were foam blocks and puzzle math pieces in the centers in Building A. The fencing was less than 6 feet tall and has protrusions on the top of it, along 3 sides. The following items of non-compliance were cited during today’s visit: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu that was posted was from the week of September 15, 2025. 10A NCAC 09 .0901(b) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. There were protrusions along the top of the fence that was less than six feet in height. GS 110-91(6); .0605((i) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. There were foam blocks and puzzles within reach of the children. .0604(q) We discussed the following items of consultation during today's visit: -You have been to the PATHWAYS TO THE STARS provider meeting and are looking to use pathway #2. We discussed curriculums and assessments must be approved if you wish to be more than 3 stars. We also discussed that staff education for 50% of lead teachers and 50% of all other staff must meet the star level that you wish to obtain. -You stated that you understood the other items for pathway #2 as well and didn't have any other questions at this time. -The sanitation inspection has been completed on 9/16/25 with 9 demerits and a Superior rating. -The fire inspection was completed and approved on 9/9/25. -You stated that you are still waiting on the building inspector and to go to the zoning office for your approved letter. You were given a NC Foundations book and an opportunity to ask child care related questions during today's visit. -You requested that you would like to change your age range to include 1 year olds. I received your email request during today's visit, and monitored the space for 1 year olds with age appropriate activities and items. Please post a diaper changing procedures poster near the changing table. Please remember that it is your responsibility to know and follow all child care laws and rules at all times, even if they have not been reviewed with you. Also you must insure that staff know and follow them as well. You must correct all violations immediately. You must submit a compliance letter, stating how the violations were corrected, by email to me at mary.mcmillan@dhhs.nc.gov by 10/17/25. A rules review will be scheduled. A more detailed documentation will be emailed to you. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
