Upon my arrival, I was greeted by Ms. Bethel The lead teacher. I introduced myself and informed me that Ms. Bethel , the purpose of today’s visit. Ms. Bethel gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Fifteen(15) children were present during today’s visit. They were observed playing outside on the playground, listening to a story, washing their hands in preparation for lunch, eating lunch and napping. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. There was an inhaler that was not in the original labeled container. .0803(2)(a) 1041 Prior to employment a Criminal Background Check was not completed. A criminal background was not completed prior to the substitute subbing in the Pre-K classroom. G.S. 110-90.2(b) The above violations must be corrected immediately, and a letter must be emailed to me no later than December 2,2025 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by December 2,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Bethel that the new staff member needs in-service training hours and health and safety training by August 15,2025. Rated License: I discussed with Ms. Bethal the Rated license assessment process for the hold harmless, is now over. The new QRIS has been adopted effective July 1,2025. I informed Ms. Bethel that the NCPre-K administration has chosen Pathway 1. We reviewed the new application process. I informed Ms. Bethel that she will not need to transition to the new QRIS System until May 2027. Thank you for your time. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
