    Name of Operation: HEAVEN SENT CHILD CARE Facility ID: 41000922 Consultant: SAMANTHA MCLEOD Operation Type: Family CC Home Case Number: Visit Date: 1/22/2026 Number Present: 5 Completed Date: 1/22/2026 Age: From 0 To 3 Total Minutes: 105 Time In: 10:15 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s routine unannounced visit was to assess compliance with applicable child care requirements for a Family Child Care Home (FCCH). This (FCCH) currently operates with a Four Star License reissued on August 28, 2018 Upon my arrival, I was greeted by the licensed provider Mrs. Peggy McLean. Mrs. McLean assisted me during my visit today. I completed a walk-through of the indoor and outdoor areas of the residence used for childcare. Five (5) children were present during today’s visit. The children were observed being cared for in the licensed space of the residence. During today’s visit I observed the children playing in the licensed space. A partial assessment of applicable child care requirements was conducted during today’s visit. Limited monitoring of operator files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Special Training including Health and Safety Training and Criminal Background Checks. The following violation was observed. Violation Number Comment Rule 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Linen was not provided for infants placed to sleep in the crib for sleeping. 10A NCAC 09 .1718(a)(2) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by February 5, 2026. Today SIDS requirements were reviewed with you: --All infants must have a signed SIDS policy on file their first day. Infants sleep charts must be completed daily. These charts must be completed until the infant reaches the one year of age. --Infants may only sleep in a crib, playpen or bassinet. A wall thermometer must be placed on the wall in the space for infants; it must read between 68-75 degrees at all times. --Children should not sleep in bouncy seats, car seats or swings. If a baby does not sleep for a long period of time or does not sleep at all it is recommended that you still document on the SIDS chart the time the baby went to sleep or write “child did not go to sleep today” on the chart. --When infants are placed in the crib for sleeping pacifiers, bibs and other items should not be in the crib. --Items such as pillows, wedges, blankets, toys etc. should not be in the crib. **As a reminder infants should only be placed in the cribs for sleeping. Once infants are awake they should be removed from the crib. DAILY CAREGIVING REQUIREMENTS: During today’s visit I reminded you that you must provide developmentally appropriate activities for the children in care. --You must have a daily schedule and current activity plan for the ages enrolled. --You must provide at least four different activities daily (including gross motor/outdoor activities). --The activity plan must also reflect four types of activities available daily including at least one hour of outside play daily (weather permitting). **The activities must include: --Art and creative play, children’s books, blocks and block building, manipulative materials and family living/dramatic play --These materials must be accessible to the children while in care and meet the development level of the children enrolled. --Music, science and nature & sand/water activities must also be offered at least once weekly. --Screen time (TV, computers & tablets) cannot be offered to children under the age of three. RATED LICENSE UPDATE/PATHWAYS TO THE STARS: (QRIS) The Pathways to the Stars transition has begun. I conducted a training for FCCH operators in September 2025, you were unable to attend. During today’s visit, an overview of section .3200 of the child care rules was provided. Each of the three (3) pathway options were discussed: Program Assessment Pathway, Classroom and Instructional Quality Pathway, and Accreditation and Head Start Pathway. Education requirements were also reviewed. Based on today’s conversation you stated you were interested in pursuing Pathway #2: Classroom and Instructional Quality Pathway. You also stated you wanted to receive additional information on the approved curriculum options for FCCH’s. I will send you via email some additional resources on approved curriculum and formative assessments. During your next visit we will discuss a timeline for your transition to the new system.
