Name of Operation: GILLESPIE PARK ELEMENTARY PRESCHOOL Facility ID: 41001261 Consultant: CHERISSA ALSTON Operation Type: Center Case Number: Visit Date: 1/16/2026 Number Present: 12 Completed Date: 1/16/2026 Age: From 4 To 5 Total Minutes: 105 Time In: 11:00 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance of child care rules during an annual compliance visit. The facility has an 18-month compliance history of 94%. The last annual compliance visit was documented on 2/7/25. Upon arrival, I was greeted by Kendra Robinson, Lead Teacher and Natoya Goode, Substitute Teacher. There were 12 children present today, ages 4 and 5 years old. Children were watching a cartoon about construction this goes with their learning unit this week about Construction. They were also observed washing their hands, eating lunch and napping. Mrs. Robinson was observed interacting with children in a positive manner. There was an Epi-pen and Albuterol on site today. Both had current permission to administer forms and medical action plans. The Epi-pen was not in the original packaging, I advised you that all medications need to be in the original packaging with the pharmacy label. Outdoor spaces were monitored and there were no hazards observed today. One child’s files and staff files were monitored during today’s visit. I monitored health and safety training and other annual requirements. There was no qualification letter on file for N. Goode. There were two items of non-compliance observed today and the corresponding violations were cited. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A substitute teacher N. Goode did not have a background check completed. G.S. 110-90.2(b) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. An Epi-pen for S. Osie-Okyer was not in the original packaging. 10A NCAC 09 .0803(2)(b)(i-v) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 1/30/26, describing how and when the violation(s) were corrected. The facility has an 18-month compliance history of 98%. The last annual compliance visit was documented on 2/27/24. Upon arrival, I was greeted by Tiffany Hunter, Lead Teacher and James Millner, Substitute Teacher. There were eight children present today, ages 4 and 5 years old. Children were observed playing in centers of their choice. They manipulated play-doh, built with blocks, and pretended to cook various foods. They also painted at the easel, played in sand and relaxed in a tent. Teachers were observed interacting with children in a positive manner. Medications with proper labels, current permission to administer forms and medical action plans were observed today. Outdoor spaces were monitored and there were no hazards observed today. Two children’s files and staff files were monitored during today’s visit. I monitored health and safety training and other annual requirements. There was a current qualification letter and emergency information on file for Mr. Milner. There were two items of non-compliance observed today and the corresponding violation was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There were several playground border spikes raised around the barrier on the playground. 10A NCAC 09 .0601(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. There were no documented drills for this school year. .0604(u);.0302(d)(8) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before February 21, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. The letter can be emailed to Cherissa.Alston@dhhs.nc.gov During the visit, we also discussed the following items: -Both teachers are enrolled in related courses. Please print unofficial transcripts and attach to your training log each semester. Please scan me a copy of your fall transcripts. -Shelter in place/lockdown drills must be conducted at least every three months. If the school does not practice a drill each quarter you should conduct the drill on your own in your licensed space. -There was an unplugged fan behind one of the desk. Fans must have a mesh covering to prevent children from being able to touch the fan blades. -There was a Keurig on top of a mini fridge, not in use. The last annual compliance visit was completed on 2/27/24. The facility has a 98% compliance history. Upon arrival, I was greeted by teachers, T. Hunter and K. Massenburg. There were 15 children ages 4-5 years old present. Children were observed playing in various centers of their choice. They were also observed gathering for group time and discussing the five senses. They listened to a book and also had music and movement time where they moved their bodies the way the song asked them too. The teachers observed participating and encouraging the children move their bodies safely. A sanitation inspection was completed on 2/28/24 an dthe facility received a Superior rating. Required items were posted including, First Aid poster, EMC plan and numbers, menu and activity plans. There was one item of non-compliance observed today and the corresponding violation was cited. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The administrator T. Bethea's qualification letter expired 8/2024. G.S. 110-90.2(b) & .2703(n)&(o) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 10/3/24, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. I monitored CPR/FA requirements and criminal background qualification letters. The administrator's letter expired 8/2024. During the visit you asked me about gross motor movement when the playground is wet because you do not have access to the gym this year. I advised that you can break up the time and have gross motor movement in the classroom. I observed the playground today and have the following consultation; you can take a small group at the time and play on the sidewalk playing hopscotch and having relay races by hopping on one foot, taking giant steps or baby steps, etc. Also, the area surrounding the play structure is mulched so you could dry the equipment and allow the children to play on the structure as well.
