This elementary school has two NCPK classrooms. An annual compliance visit was conducted on November 5, 2025. Upon arrival to the classroom, I was greeted by the lead teacher, Ms. Cynthia A partial assessment of child care requirements was monitored. I reviewed one staff’s file. Twenty children were present during today’s visit. I observed the children transitioning to lunch, completing bathroom routines, and napping. The children went to the cafeteria for lunch in preparation for their transition to kindergarten. Required documents were posted and accessible. Monthly fire drills, quarterly disaster drills, and playground inspections have been documented and documentation available for review. During today's visit, two violations of child care requirements were cited. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A criminal background check for the substitute E. Cobb was not completed. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not on file for E. Cobb, substitute. G.S. 110-90.2(b) & (d) & .2703(e) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before June 11, 2026. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. Date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If the letter of compliance is not received on June 11, 2026, a follow-up visit will be conducted. Today in Space #p9, the qualification letter for E. Cobb was not on file. I checked the criminal background check in the ABCMS system and no information was available for review. ERS summary was reviewed today and items scoring less than three could constitute a violation of child care requirements and therefore were monitored. Items are noted below: Item 8 Indicator 3:2 Score 2 Assessor comments: Donuts and juice were served for snack. Donuts are considered a grain-based dessert and do not count towards the grain requirement. In addition, a child was served water instead of juice. Observations today: Meals and snacks are prepared by the school’s cafeteria. The school menus are created by the school county office. Donuts were not served today. We discussed that because of the high sugar and solid fat content, a "Super Donut" cannot be credited as a healthy grain. Item 10 Indicator 3.2 When stored, the cots were stacked on top of one another with a blanket folded and placed in the middle of each cot; the blankets touched the bottom of the cots stacked above. Observations today: The cots placed out for nap time. They were not stacked. Rated License Information: Prior to today’s visit, I received the complete application packet for Pathway 1: Program Assessment. I received the updated staff information and education form via email on May 27, 2026. At this time the rated license packet cannot be processed until Ms. Sara Jones, principal, submits her education information to WORKS for evaluation. The facility 18th month compliance history is 95%. Your average score on ECERS-3 was 5.06 It appears that you may qualify for 5 Stars. A Five-Star Rated License is recommended for issuance to this facility upon completion of the education evaluation and the violations have been corrected. Please keep in mind that the administration of the Division of Child Development and Early Education must approve this assessment prior to issuance of the license. I used a checklist to note the requirements I monitored today. This NCPK currently operates with a Four-Star license, effective February 14, 2018. This elementary school has two NCPK classrooms. The school’s office staff greeted me and gave walked with me to the classrooms. I observed the NCPK classrooms and gathered attendance. Today a total of seventeen children are present between both classrooms. Ms. C. Monterrosa, lead teacher was in classroom #1 and the teacher assistant is Ms. D. Collins. I observed the children participating a gross motor group activity, completing bathroom routines, and going outside. I observed classroom # 2 as they going outside. Ms. M. Henning is the teacher and Ms. D. Outlaw is the teacher assistant All children eat in the classrooms. The latest Sanitation Inspection was conducted March 4, 2025, with a Superior score. I reviewed a sampling of children’s files. I received the staff and training worksheet via email prior to day's visit. The staff and training worksheet was updated during today's visit.
