A checklist was used to note the requirements I monitored today. This facility currently operates with Five Star license issued on October 3,2019. Upon my arrival, I was greeted by Ms. N. McNair, the director. I informed her of the purpose of today’s visit. Ms. McNair gave me the necessary paperwork I need to complete today’s visit. Ms. McNair accompanied me as I observed and documented the attendance. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medications were out of the children's reach. Sixty-six (66) children were present during today’s visit. I observed the children being diapered, playing on the floor with available toys, playing in centers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch, napping, waking up from napping, washing their hands in preparation for snack, eating snack and departing. A sanitation inspection was conducted on October 6,2025 This facility received a superior rating with zero (0) demerits. A fire inspection was conducted on February 6,2025. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I was unable to monitor the bus the facility uses on field trips due to the bus not being on-site today. I informed Ms. McNair that before the children are transported that i will need to monitor the bus before the children are transported. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as of January 8,2026. One (1) violation was cited during today’s visit. Violation Number Comment Rule 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Fourteen employee's personnel file did not contain an annual staff evaluation and a staff development plan. 10A NCAC 09 .0514(f) The above violation must be corrected immediately, and a letter must be emailed to me no later than January 22, 2026, stating how the one item was corrected. If I do not receive your letter by January 22,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Consultation: I discussed with Ms. McNair, that the staff members need in-service training hours, It-SIDS, health and safety training, CPR and First Aid trainings on specific dates. Please refer to the staff/training worksheet. Rated License: I discussed with Ms. McNair the Rated license assessment process for the hold harmless, which is now over. The new QRIS has been adopted effective July 1,2025. We reviewed the new application process. Ms. McNair stated that she was interested Pathway 2, but this will need to be presented to the daycare board before a final decision is made. I left a copy of the QRIS form that discussed the information for the Pathways to the Stars. WORKS: Please ensure the education information is in the W.O.R.K.S. database. Ms. McNair needs to complete the education process in W.O.R.K.S. to update her education in workforce as well as the new staff’s education. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed to ensure your staff meets minimum education requirements. Clean Classrooms for Carolina Kids This facility completed the lead water on March 29,2024. This test should be completed every three years. The lead water is due March 29,2027. Upon my arrival, I was greeted by Ms. L. Knight, office administrator. Ms. Knight informed me that Ms. N. McNair, the director, was on a tour with a potential family. I informed Ms. Knight the purpose of today’s visit. Ms. Knight accompanied me as I observed and documented the attendance. Later Ms. McNair joined me and assisted me for the remainder of my visit. Sixty-six (66) children were present during today’s visit. They were observed being fed, sleeping, being diapered, playing with available toys playing outside on the playground, washing their hands in preparation for lunch, eating lunch and napping. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions CPR and First Aid Certification. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. There was not enough mulch under the climbing equipment. There was less than one inch. .0605(j) 1043 All staff records, except financial records, were not made available for review. One staff file was not available for review. G.S. 110-91( 9) The above violations must be corrected immediately, and a letter must be emailed to me no later than July 1,2025 stating how each item was corrected. ABCMS Provider Portal North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. A sanitation inspection was conducted on July 2,2024. This facility received a superior rating with four (4) demerits. A fire inspection was conducted on November 11,2024. Each staff member must register in the WORKS system as soon as they are hired at this location. We reviewed the instructions on the DCDEE website. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed to ensure your staff is meeting minimum education requirements. There are two other education options. One options you may also want to consider having staff members complete the Early Educator Certification process to assist in getting higher education evaluations completed if their degree is not in early childhood education. The facility has a 100% compliance history. Upon arrival, I was greeted by Nikki McNair, Administrator. Ms. McNair accompanied me on a walkthrough of the facility and assisted me with required documents. Infants were observed napping on floor beds, in proper sleep positions according to your safe sleep policy. Children ages 1-5 were observed eating lunch and playing outside. Outside children were riding tricycles, playing in the sand, and playing together in small groups. Fire drills, emergency drills and playground inspections were current and documented. A sanitation inspection was conducted on 12/7/23. There was one item of non-compliance observed during today's visit. The corresponding violation was cited. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence closest to the parking lot has at least three (3) areas that are less than 4 feet in height. GS 110-91(6); .0605((i) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by June 6, 2024, describing how and when the violation(s) were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. During my walkthrough of the playground, I noticed the fence looked shorter than 4 feet in several areas. The Director and I measured three different portions of the fence and all three areas were below four feet. The Director stated that the playground is going to be updated and she is in the process of getting quotes to have the fence replaced, new structures for the sandboxes and new garden beds.
