A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star license issued on February 20,2020. Upon my arrival to the facility, I was greeted by Ms. J. Perez, the teacher assistant. Ms. Perez informed me Ms. D. Newkirk-White the lead teacher. I informed Ms. NewKirk-White the purpose of today’s visit. Ms. Newkirk-White gave me the necessary paperwork and assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit, I observed the children participating in a literacy activity, movement activity, washing their hands in preparation for lunch, eating, lunch, preparing for naptime and napping. Fifteen (15) children were present during today’s visit with one teacher. After ten minutes of my arrival, Ms. T. Smith came into the classroom until Ms. Newkirk-White returned to the classroom. I informed Ms. Newkirk-White this would be a staff/child ratio violation. A sanitation inspection was conducted on February 14, 2024. This facility received a superior rating with two (2) demerits. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. G.S. 110-91( 9) 1743 A minimum of two staff members were not on site at all times while children were present at the facility. There were fifteen children present with one teacher. .2902(g) The above violations must be corrected immediately, and a letter must be email to me no later than March 21,2024 stating how each item was corrected. My email address wytina.blackmon@dhhs.nc.gov If I do not receive your letter by March 21,2024/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Newkirk-White that the staff members need in-service training hours, health safety trainings, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. You need to email the training certificates once staff has completed the training hours. Rated License Assessment: I also discussed with Ms. Newkirk-White the Rated license assessment process will begin July 1,2024. I informed Ms. Newkirk-White this facility is in Cohort 2. Today I discussed with Ms. Newkirk-White about the Prep year assessment. Ms. Newkirk-White informed me that she would let me know when she was ready to request the assessment. Your prep year is July 1,2024 to June 30,2025 and your assessment year is July 1,2025 to June 30, 2026. You must let me know if you ready to start the process before June 30,2024. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
