    Name of Operation: DIANNA'S CHILD CARE II Facility ID: 41001825 Consultant: SHARON WILLIAMS Operation Type: Center Case Number: Visit Date: 4/17/2024 Number Present: 8 Completed Date: 4/17/2024 Age: From 0 To 1 Total Minutes: 160 Time In: 10:20 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance of applicable child care requirements during an annual compliance visit. A checklist was used to note the requirements I monitored today. I also monitored health and safety requirements. Currently this center located in a residence operates with a 3 Star License, issued 8/5/19 earning 4 points in the Education Component, 2 points in Program Standards (meeting enhanced space). This facility has also earned a quality point for having a 20% or less turnover rate for all staff members. I was greeted by Ms. Laverne Webster, lead teacher. Ms. Dianna Daye, the owner, arrived during my visit. There were 8 children present at the center. I observed the children being held, playing on the floor and participating in free choice activities. The staff/child ratios were the following: Infants 1:4 (L. Webster) 1’s 1:4 (S. Whitworth) A sanitation inspection was completed on June 21, 2023 resulting in a "Superior" classification with 6 Demerits. A satisfactory fire inspection was completed on September 15, 2023. The last documented fire drill was conducted on 3/28/24. The last documented lock drill was conducted on 1/28/24. I reviewed two staff files and a sampling of children's files. I reviewed the information on the staff and training worksheet. Today, I received from you the FCCH and Centers Located in a Residence Operator’s Statements of Responsibility. Your signature on these forms will serve as verification that the information provided is accurate and complete. Today, I checked the NC Secretary of State website for your corporation status. Your corporation status is current and active. During today's visit, one violation of child care requirements was cited. Violation Number Comment Rule 881 The evacuation crib or other approved device did not have wheels or a reinforced bottom. The old evacuation crib was replaced with a new crib. The new crib did not have the wheels or a reinforced bottom installed. The wheels and reinforced bottom were in a box waiting to be installed. .0604(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before May 1, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. Please email the compliance letter to: sharon.y.williams@dhhs.nc.gov. Licensed child care facilities must have an inspection for asbestos and lead-based paint. Child care center water testing is required every 3 years. It’s time for your facility to have the water re-tested for lead. In the same link you can get tested for asbestos. Facilities can partner with the Clean Classrooms for Carolina Kids program to fulfill North Carolina requirements (10A NCAC 41C .1000). The program is providing free facility-wide inspections without any out-of-pocket cost that meets all rule requirements from program launch to November 30, 2026. To enroll, facilities should go to www.cleanwaterforUSkids.org/carolina to sign up for a pre-enrollment webinar and participate in the free process. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
