    Name of Operation: DIANNA'S CHILD CARE CENTER Facility ID: 41002165 Consultant: CHERISSA ALSTON Operation Type: Center Case Number: 0626-006L Visit Date: 6/3/2026 Number Present: 22 Completed Date: 6/3/2026 Age: From 2 To 9 Total Minutes: 75 Time In: 10:15 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Self Report Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to obtain information related to a self-report received by the Division. During today’s visit, Dianna Daye accompanied me during a walk-through of the facility. During the visit, I discussed the information with Mrs. Daye. Based on information obtained, the following was determined. On 5/28/26, a teacher and the children enrolled were on the breezeway. It is unknown what occurred before the incident, but the sibling of a 3 year old child witnessed the teacher thumping their sibling in the face. The incident was reported to the parent by the older sibling and the parent contacted Mrs. Daye. Mrs. Daye questioned the teacher about the incident reported and the teacher denied thumping the child. Mrs. Daye stated there were no visible injuries or bruises on the child's face. The parent requested a conference to speak with both Mrs. Daye and the teacher. Mrs. Daye questioned the teacher again on 5/29/26, prior to the meeting, and the teacher admitted to thumping the child in the face. Mrs. Daye reminded the teacher of the discipline and behavior management policy she signed and that any violations of that policy would be ground for immediate termination. The teacher was terminated effective immediately. Limited monitoring of child care requirements occurred during today’s visit. The following violation was confirmed during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A 3 year old child was thumped in the face by their teacher. G.S. 110-91(10) I spoke with Mrs. Daye about corrective measures to ensure the incident would not occur again. She stated she had a staff meeting on 5/29/26 and reviewed the facility’s discipline policy and reminded staff what they can and cannot do to manage behaviors. She reminded them that is never appropriate to put their hands on a child for any reason. If they need help, they can ask for assistance. I suggested that Mrs. Daye conduct periodic observations in classrooms to monitor how staff handle unwanted behaviors, monitor staff stress levels and provide relief if she notices any staff needing a break. I also suggested that Mrs. Daye contact local agencies about training for managing difficult behaviors, classroom management, and stress relief/self-care any other trainings that would be beneficial to the staff. No further corrective action is not required on your part at this time. You must continue to maintain compliance with applicable child care requirements. Thank you for your time today.
