    Name of Operation: DEVELOPING CREATIVE MINDS Facility ID: 41002545 Consultant: MARY MCMILLAN Operation Type: Center Case Number: Visit Date: 9/10/2025 Number Present: 26 Completed Date: 9/10/2025 Age: From 0 To 4 Total Minutes: 170 Time In: 11:50 AM Time Out: 02:40 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period visit. This is the first visit since the facility had a change of ownership on 8/1/25 and also this is a location change as you consolidated your previous center into this change of ownership on 8/1/25. Today I met with Maesha Parrish, Owner and Yalonda Moore, Director. I monitored the following items during today's visit: Supervision, Staff/Child Ratio, safe indoor and outdoor environment, required items posted, approved spaces, CBC records along with program records were reviewed. The sleep charts for the one infant were not completed for the last 2 days. The infant was not asleep during my visit. The classroom was observed with having 1 infant, 4 one year old's and 1 two year old present. The infant was awake and the other children were napping. We discussed that only 1 crib met the evacuation crib requirements with the reinforced legs/bottom and large caster wheels. You only currently need 1 evacuation crib, but you stated that you wanted to have 2 of them. The fencing on one side of the playground for younger children is right at 4 feet tall. We discussed that if you put more mulch on that side up against the fence it will shorten the fence below 4 feet tall. The fencing there and 2 sides of the playground for older children have protrusions on the top and are both less than 6 feet tall. The swing set does not have the required 6 inches of mulch surfacing as it has large holes under the swings and the fall zone for 2 of the swings does not meet the required area of space 2 times the pivot point. During today’s visit, the children were observed playing and napping. The last fire drill was recorded on 8/28/25 at 11:13am with 54 seconds evacuation time. The playground inspection was last recorded on 9/9/25. The following violations were cited during today’s visit: Violation Number Comment Rule 316 Children under one year of age were not kept separate from children two years and older. In the classroom for infants and one year old's, there was a 2-year-old present with an infant. 10A NCAC 09 .0713(a)(5) 807 A safe indoor and outdoor environment was not provided for the children. Indoors: Sleep charts were not completed for 9/8/25 and 9/9/25. Outdoors: The fence is lower than 6 feet and has protrusions around the top on 3 sides, there is rust on swing chains and some climbing equipment and mulch is not the required 6 feet deep under the swing set. 10A NCAC 09 .0601(a) We discussed the following items of technical assistance during today's visit: -suggestions of how to change the fencing and surfacing items of non-compliance along with the sleep charts -We discussed grouping of children and that children 2 years of age can be with the older children but not with the children under 1 year of age, except for the first hour and last hour of the operating day. We discussed the following items of consultation during today's visit: -We completed a Rules Review Training on 8/22/25. -You must have your approved Fire, Building and Sanitation inspections completed by 12/2025 and send them to me for review. -QRIS modernization for your stars and you can find more information about this on our website at ncchildcare.ncdhhs.gov under the what's new tab and then the QRIS tab You were interested in Pathway #2 for Pathways to the Stars and we went over all of this pathway in detail and you made copies of the requirements for each area of this pathway. You stated that you currently use the Mother goose approved curriculum for all children and you will start using the assessments that go along with that curriculum and sharing those with the parents 2 times a year. You stated that you wish to be a 5 star program and that is the level that we reviewed and all requirements that must be met. We discussed that I have invited you to a provider meeting for QRIS pathway information on September 30, 2025 at 12:30pm at the Guilford County Partnership for Children. Your compliance history affects the star rating. It is imperative that staff maintain compliance with child care rules and requirements at all times. Violations of child care requirements affect the facility’s compliance history. You must maintain a minimum compliance history of seventy-five (75) percent. At the end of your temporary time period, in order to qualify for DSS subsidy vouchers, you must have a 3-5 star rated license. Please ensure that all staff members have a WORKS Account and are updated with their correct educational information. The information on each staff member’s WORKS Qualification Letter and what is shown in the DCDEE WORKS system is what will be used to determine the points in education for your license. Instructions for the DCDEE WORKS system is located in the DCDEE website at https://ncchildcare.ncdhhs.gov. You must correct any violations immediately and submit a compliance letter to me at mary.mcmillan@dhhs.nc.gov before 9/22/25, stating how the violations were corrected. Please remember that it is your responsibility to know and follow all child care laws, rules and requirements even if they have not been reviewed with you. A follow-up visit will be conducted. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
