This facility currently operates with a Four Star license issued on January 5, 2020. I arrived at the facility and was greeted by the Ms. Chimere Carr, assistant administrator. I introduced myself and stated the purpose of today’s visit. Ms. Carr assisted me during my visit today. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. A walk-thru of the indoor and outdoor spaces used by the children were monitored. Thirty-one children were present during the observation. In the infant room, one child was being fed while six children were actively engaged in free play and crawling on the floor. In the preschool classrooms, children were observed participating in both free-choice and teacher-directed activities, completing bathroom routines, and transitioning outdoors. A sanitation inspection was conducted on March 26, 2026. Monthly fire drills, playground inspections, and quarterly disaster drills have been documented. Two violations of childcare requirements were cited today. The violations were corrected during the visit therefore a letter of compliance is not required to be submitted. Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. In the preschool classroom the staff/child ratio was not posted. The assistant director corrected this during the visit by posting the staff/child ratio sheet. .0713(a)(10), (c) & (f)(3); .2818(e) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In Classroom 3B, a plastic shopping bag containing clothes was stored in a cubby that was accessible to children. The assistant director corrected this by removing the plastic bag from the cubby and placing it in the office. .0604(q) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Pathways to the Stars We reviewed the updated rules in Section .3200 regarding the Standards for Two- through Five-Rated facilities (Pathway to Stars). As discussed, the facility should carefully consider the available QRIS options and determine which pathway it plans to pursue for its rated license reassessment. While the reassessment process may begin at any time, the facility should be prepared to submit the Application for Assessment for a Rated License between April 1, 2026, and September 30, 2026. Today you stated that you are still deciding which Pathway but considering Pathway 1: Classroom and Instructional Quality If you choose Pathway 1: Program Assessment Pathway • Complete the “Application For Assessment for a Rated License for Centers” electronically and submit to your child care consultant no later than, September 30, 2026. • Complete the “Environmental Rating Scale (ERS) Assessment Request” form electronically and submit to your child care consultant no later than, September 30, 2026. • Confirm all staff have active WORKS accounts. • Complete the QRIS Staff Information and Education Worksheet electronically. • Begin planning for your three-month self-study using the applicable ITERS-3, ECERS-3, SACERS-U, books. • Prepare for Environmental Rating Scale (ERS) assessments. • Access resources, trainings, and outreach via ncrlap.org. • Identify CQI goals and begin planning for implementation. • Develop strategies for Family and Community Engagement. • If four-year-olds are enrolled, select and implement an approved curriculum. Clean Classrooms for Carolina Kids The lead-based paint and asbestos enrollment surveys must be completed for childcare facilities. As this is a legislatively mandated effort, facilities are required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina Kids™ is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. This program is free, covers testing, inspections, and mitigation, and is intended to improve child-occupied facilities across North Carolina. At this time, your facility has completed only the lead-in-water testing component. Required testing includes lead in water, lead-based paint, and asbestos. Please complete the online enrollment process for lead-based paint and asbestos testing no later than July 31, 2026. In addition, testing should also be completed every three years for childcare centers. Please remember that it is your responsibility to stay informed and comply with all child care rules and requirements. A checklist was used to note the requirements I monitored today. This facility currently operates with a Four Star license re-issued on March 15, 2020. I arrived at the facility and was greeted by the assistant administrator, Ms. Asya Settle. The administrator, Ms. Shyra Settle, was out today due to illness. I made you aware of the purpose of today’s visit. Ms. Asya Settle assisted me during my visit today. A walkthrough of the indoor and outdoor spaces used by the children was conducted during today’s visit. In the space for infants, the children were observed actively engaging in free play and one teacher was observed reading a story to the children. During today’s observation one diaper change was observed. Age-appropriate and sufficient materials were available and accessible within the classroom. The infants did not go outside during my observation today. In the spaces for toddlers and young preschool children, I observed the children as the played in the outdoor learning environment. The teachers were observed actively supervising the children. I also observed the children as they completed several transitions. In the preschool classrooms, observations took place during center free play, small group activities, and teacher-directed small group instruction. The children were also observed during transition periods, including transitions between indoor and outdoor spaces. Several handwashing routines were observed throughout these transitions. Age-appropriate and sufficient materials were available and accessible within the learning environments. During today’s visit I observed hand washing and bathroom routines. Positive interaction was observed between the staff and the children. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. Thirty-two (32) children were present during today’s visit. Program records and staff and children’s files were monitored for compliance. A sanitation inspection was conducted on November 20, 2025. This facility received a superior classification and fifteen (15) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. A satisfactory fire inspection has been completed on February 18, 2025. This facility was approved for daytime and overnight care. This facility’s corporation status was verified current on the NC Department of the Secretary of State on January 7, 2026. The following violations of childcare requirements were cited today. Violation Number Comment Rule 488 For children under three years of age, materials were not offered in sufficient quantity to allow all children to use them at some time during the day and to allow for a range of choices. In the space for two year old children a sufficient quantity of books and blocks were not available to the children; In the space for toddler children there was not a sufficient quantity of books accessible to the children. .0510(e)(3) 721 All equipment and furnishings were not in good repair. The fence separating the playground for school age children and infant children needs some repair at the bottom of the fence. G.S. 110-91(6); .0601(b) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In the space for two year old children plastic bags (Ziplock and store bags) were observed in the cubbies; these bags were accessible to the children. .0604(q) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by January 22, 2026. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. Health & Safety Requirements were also reviewed. INDOOR & OUTDOOR SAFETY: A safe, hazard-free indoor and outdoor environment is crucial in early childhood settings. During today’s visit, I discussed with you the importance of routinely inspecting both indoor and outdoor areas for hazardous items and broken equipment. It is also important that expectations are clearly communicated to all staff regarding daily safety checks, compliance with safety requirements, and the responsibility to promptly report any hazards, broken items, or equipment in poor repair. Indoor Environment: Staff must ensure that all hanging cords, electrical cords, and teacher supplies are kept inaccessible to children at all times. Items that could pose a strangulation, electrical, or injury risk must be properly secured or stored out of children’s reach. In addition, all cleaning supplies, including cleaning sprays and deodorizers, must be stored in locked storage areas and kept inaccessible to children to prevent accidental exposure or ingestion. Outdoor Area: The outdoor learning environment provides children with opportunities for exploration, freedom, and risk-taking. Because of this, it is essential that the space remains safe and well maintained. Proper and consistent maintenance is a key factor in ensuring a safe outdoor play environment for children. All outdoor play equipment must be maintained in good condition, free of damage, sharp edges, or broken parts. The grounds, including grassy areas, should be routinely maintained to prevent hazards such as holes, uneven surfaces, or debris. Staff should also ensure that trees and landscaping are properly maintained, with tree limbs trimmed and kept out of reach of the play space. In addition, the outdoor area should be routinely checked to ensure that fencing remains intact, secure, and in good condition, with no gaps or damage that could compromise children’s safety. Plastic Bags: Plastic bags pose a serious suffocation hazard for young children and must be kept inaccessible to children under three years of age. All plastic bags should be stored out of children’s reach, at least five feet above the finished floor. Trash bags should be secured and tucked away so that large sections are not accessible to children. **Access to important children, staff & program records: You have one enrolled child who receives additional food through a feeding tube. The information/instructions were available on your pro-care system (which the teachers have access to) but were not available in the classroom or in the child’s file. You must have printed instructions and all medical information in the file and accessible to the staff members in the classroom. The use of technology to store children and staff information is a valuable and efficient resource. However, please ensure that all state-required documentation can be printed and produced upon request. Additionally, keep in mind that in the event of a power or internet outage, electronic records may not be accessible to staff. It is important that critical documents, particularly those related to health and safety, are always readily accessible to staff. ANNUAL REMINDERS: --This facility completes annual staff training by November 1st of each year. --Staff Evaluations by August 31st of each year -Professional Development plans are completed by October 31st each year --Health Questionnaire/Emergency Information by October 31st of each year --Children’s emergency information is completed by July 31st of each year ABCMS PROVIDER PORTAL: (Automated Background Check Management System) Owners/Directors must notify DCDEE when a new employee is hired or moves into their facility by accessing the Provider Access to ABCMS . Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video. The CBC unit staff will grant providers access after completion of this training submit the signed Powerform available at the conclusion of the training. Please complete the process for your facility as soon as possible. Once providers are logged in, you can access codes to allow your staff and applicants to create a connecting application to link themselves to your facility. Additional information and instructions are available in the ABCMS Provider Portal Guide. I have emailed you additional information on how to complete this process and a portal guide for your reference. **This needs to be completed ASAP. **PATHWAYS TO THE STARS: (QRIS) The Pathways to the Stars transition has begun. During today’s visit we discussed each pathway. Based on today’s discussion you are trying to decide between Pathway #1 (Program Assessment) and Pathway #2 (Classroom and Instructional Quality). We discussed how each pathway would impact your facility and the methods needed to implement each pathway. I also shared with you the new approved curriculum and formative assessments list. --It is also suggested that you visit the NCRLAP (www.NCRLAP.org) website for upcoming trainings and view their resources for the new Infant/ Toddler & Early Childhood assessment tools and review the School-age assessment tool (ITERS-3 and ECERS-3 and SACERS-U). --As a reminder it is important to have all your staff use this time to submit updated educational information to our Workforce system. All staff will need to have a current Workforce letter on file. Additional information on the new QRIS system can be found on the DCDEE website including informational handouts and webinars: Click under the “What’s New tab” than “QRIS Modernization”. I was greeted by Ms. Chimere Carr; an administrative assistant of the childcare center she assisted me with the visit. This center currently operates with a Four-Star license on January 15, 2018. Ms. Shyra Settle, the executive director joined us for the visit. An approved fire inspection was conducted on May 2, 2024, and I received a copy. The latest sanitation inspection was conducted on July 23, 2024, with a Superior score. The compliance history is 98% as of January 15, 2025. The NC Department of the Secretary of State lists this center’s status as active and current as of January 14, 2025. A full assessment of applicable child care requirements was conducted, I used the Annual Monitoring Checklist during today’s visit. Today thirty-two children are attending When I arrived the children were playing in classrooms, and some classrooms teachers began preparing to transition to routines and lunch. There are many items and choices for the children in each center and on the playground. All the interactions I observed were warm and caring. I observed the outdoor play area. There is still snow in some areas of the playground, but there are some clear places for the children to play. The playground is shaded and has room for vigorous play and running. There are several items for the children to choose from for outdoor play. There is a large piece of climbing equipment with adequate mulch. I reviewed a sampling of staff files and of children files. Program files were current and in order. Your signature on these forms will serve as verification that the information provided is accurate and complete. I reviewed the Staff and Training Sheets. I observed one item of non-compliance of childcare rules during the visit today. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member CBC Qualification letter expired last week. G.S. 110-90.2(b) & .2703(n)&(o) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a letter of correction by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by January 31, 2025. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. Recognizing and Responding to Suspicions of Child Maltreatment training is located on this website, https://www.preventchildabusenc.org. The most up-to-date childcare rules are on the DCDEE website. Moodle also has other trainings, and they are free. Please keep in mind these trainings must be completed within one year of hire and once every five years. You will still need to complete the Recognizing and Responding to Suspicions of Child Maltreatment training through Prevent Child Abuse NC within 90 days of employment and the training must be completed every five years. When the Suspicious of Child Maltreatment is completed you must print you certification of completion immediately. If you go out of the program your certification is not save and you will need to retake the training. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements.
