    Name of Operation: CLAXTON ELEMENTARY NC PRE-K PROGRAM Facility ID: 41002506 Consultant: WYTINA BLACKMON Operation Type: Center Case Number: Visit Date: 5/14/2026 Number Present: 31 Completed Date: 5/14/2026 Age: From 4 To 5 Total Minutes: 200 Time In: 09:35 AM Time Out: 12:55 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit with a license rated . A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on January 30,2025. Upon my arrival, I was greeted by Ms. L. Clark, one of the lead teachers. Ms. Clark informed me that Ms. Masterson was out today. I informed Ms. Clark the purpose of today’s visit. Ms. Clark gave me the necessary paperwork I needed to complete today’s visit. All indoor and outdoor spaces used by the children were observed today. All required items were posted. All medications were out of the children's reach. The children were observed finishing up lunch, participating in a literacy activity and napping. Thirty-one (31) children were present during today’s visit. A sanitation inspection was conducted on April 21,2026. This facility received a superior rating with seven (7) demerits. A fire inspection was conducted on December 16,2025. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Leftover medicines were not returned to one parent after the medication was expired. .0803(12) 1041 Prior to employment a Criminal Background Check was not completed. A criminal background was not completed prior to the substitute subbing in the Pre-K classroom. G.S. 110-90.2(b) The above violations must be corrected immediately, and a letter must be emailed to me no later than May 28,2026 stating how each item was corrected. If I do not receive your letter by May 28,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Clark that the staff members need in-service training hours, health and safety training, CPR and First Aid trainings by specific dates. Please refer to the staff/training worksheet. Rated License Assessment: We thoroughly discussed the results you received on the Early Childhood Environment Rating Scales with emphasis on the indicator items with scores less than 5. There were no items on the Facility Summary Report that could be a potential violation of child care requirement. We discussed strategies to improve some of the items that scored less than 5 points to increase the points earned on the next assessment. This facility has chosen Pathway 1. The facility met the foundational practices required to meet family and community engagement standards and 4 additional options, from different category. This facility also has completed the CQI and PD form. Education Standards: The administrator meets Level III NC Early Childhood Administration Credential and 24 years of early childhood education work experience, administrative work experience. The lead teacher meets minimum requirements ,BS/BA in ECE/CD and 19 years of early childhood work experience. The teachermeets completion of the NC Early Childhood Credential or its equivalent and completion of at least 12 semester hours in ECE/CD and 14 years of early childhood work experience. Program standards: The operational policies were approved during the original rated license assessment. The scales were conducted on April 14,2026. The center earned 4.88 on the Early Childhood Environmental Rating Scale (ECERS). You have agreed to meet enhanced space and ratios. Compliance History: An updated compliance history was printed on May 14,2026, which showed a current score of 98% for eighteen months. An updated compliance history will be printed to reflect violations cited during today’s visit. Based on the information reviewed ,this center may qualify for a Five Star License. Once I receive your compliance letter,then I will submit paperwork for a Five star license to be issued to your facility. Please keep in mind that the administration of the Division of Child Development and Early Education must approve this assessment and the star level earned prior to issuance of the license. Criminal Background As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please plan to complete your 5-year re-qualification before the expiration date. You will need to upload all your current to staff and new staff to the ABCMS Provider Portal. Failure to have your staff loaded in this provider portal will result in a violation being cited at your next visit. The facility had a location change, and the Temporary License was issued on August 28, 2024, to February 28, 2025. Upon my arrival, I was greeted by Ms. Masterson and Ms. Clark, the lead teachers. Thirty-three children were present during today's visit. They were observed painting, participating in a movement activity, listening to a story, and playing in centers. Limited licensing requirements were monitored during today's visit. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: group size: space capacity: proper storage of medications/hazardous supplies; use of approved space, permit posted, permit restrictions CPR and First Aid certification. There was one violation cited during today visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. There was a substitute who did not have a criminal background check completed prior to to employment. G.S. 110-90.2(b) The above violation must be corrected immediately, and a letter must be emailed to me with your signature no later than February 7,2025 stating how the one item was corrected. Rated License: The Rated license assessment process the hold harmless has been extended until the new QRIS is implemented therefore this facility will Keep their Five Star License from their previous location. Program standards: This facility had the environmental rating scales conducted on prior to the change of location therefore a new environmental rating will not be conducted. The facility scored 5.89 on the Early Childhood Environment Rating Scale. The average score is 5.89. The facility has chosen to meet enhanced all enhanced standards plus reduced ratios minus. The facility meets seven points in program standards. Compliance History: An updated compliance history was printed on January 24,2025, which showed a current score of 99% for eighteen months. An updated compliance history will be printed to reflect the violation cited during today's visit. Quality Point: The center has chosen center 75% lead teachers have a BA/BS or Higher in ECE/CD as their quality point. Based on the center having earned seven points in staff education at their previous , seven points in program standards and one point in Quality, your Child Care Center keep their Five Star Rated License. After I receive your compliance letter, then I will request a Five Star Rated License be issued and mailed to your Child Care Facility. Please keep in mind that the administration of the Division of Child Development and Early Education must approve this assessment, and the points earned in each component prior to issuance of the license.
