    Name of Operation: CLAXTON ELEMENTARY ACES Facility ID: 41002503 Consultant: WYTINA BLACKMON Operation Type: Center Case Number: Visit Date: 11/24/2025 Number Present: 72 Completed Date: 11/24/2025 Age: From 5 To 10 Total Minutes: 160 Time In: 01:35 PM Time Out: 04:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for compliance during an annual compliance visit. A checklist was used to note the requirements I monitored today. This facility currently operates with a Three Star license issued on June 1,2025. Upon my arrival, I was greeted by Ms. L. Swann-Grady, program coordinator. I informed Ms. Swann-Grady the purpose of today’s visit. Ms. Swann-Grady gave me the necessary paperwork I needed to complete today’s visit. Seventy-two (72) children were present during today’s visit. During today’s visit, I observed the children arriving, washing their hands in preparation for snack, eating snack, doing homework, playing in centers and departing. All indoor spaces used by the children were observed today. All required items were posted. All medication and hazardous items were out of the children's reach. A sanitation inspection was conducted on March 24,2025. This facility received a superior rating with zero (0) demerits. A fire inspection was conducted on June 17,2025. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Violation Number Comment Rule 851 When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. The group leader did not document when the child inhaler was administered. .0803(13)(a-e); .2318(3) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan was not documented that the plan was reviewed with the staff annually. 10A NCAC 09 .0802(a) 1812 The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The center did not a completed the EPR plan. .0607(c) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The trained staff did not review the EPR Plan annually with the staff. .0607(e) The above violations must be corrected immediately, and a letter must be emailed to me no later than December 8 ,2025 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov. If I do not receive your letter by December 8,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Swann-Grady the one new staff member who needs health and safety training ,in-service training hours, CPR and First Aid by specific dates. Please refer to the staff and training worksheet. Rated License Assessment: I discussed with Ms. Swann-Grady the Rated license assessment process for the hold harmless, is now over. The new QRIS has been adopted effective July 1,2025. I informed Ms. Swan-Grady that the ACES administration has chosen Pathway 1. We reviewed the new application process. I informed the teachers they will not need to transition to the new QRIS System until May 2028. Thank you for your time. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
