    Name of Operation: CJ'S CHILDCARE CENTER #3 Facility ID: 41001732 Consultant: SHARON WILLIAMS Operation Type: Center Case Number: Visit Date: 1/24/2024 Number Present: 20 Completed Date: 1/24/2024 Age: From 2 To 4 Total Minutes: 180 Time In: 09:30 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance of applicable child care requirements including health and safety requirements during an annual compliance visit. A checklist was used to note the requirements I monitored today. Upon arrival at the center, I was greeted by Ms. Latrell Headen/Director. There were 20 children present during today's visit. I observed the children playing outside, completing bathroom routines, participating in free choice and teacher directed activities. The staff/child ratios were the following: 2's and 3’s - 2:10 (N. Downing and K. Mills) 3’s and 4’s– 1:10 (M. Pennix) A full assessment of child care requirements was conducted during today’s visit. The sanitation inspection was completed on August 1, 2023 with a "Superior" classification with 2 Demerits. A satisfactory fire inspection was conducted on September 12, 2023. Prior to today's visit, I checked the NC Department of the Secretary for the facility's corporation status. The corporation status is active and current. During today's visit, violations of child care requirements were cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the school-age playground, the monkey bars climbing tower was turned upside down. The handicap rail to the school-age building has four slats that are loose. 10A NCAC 09 .0601(a) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff member did not have a medical report on file. One staff did not have proof of tuberculosis test /or screening on file. .0701(d) The violations documented must be corrected immediately. A signed and dated letter of compliance must be received on or before February 7, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter You may email the signed compliance letter to sharon.y.williams@dhhs.nc.gov Rated License Process The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year won’t begin until July 1, 2024, the preparation year for cohort one began July 1, 2023. I will reach out to you prior to the beginning of the preparation year of your assigned cohort and will provide guidance to assist you as you begin the process.
