The facility has an 18-month compliance history of 99%. The last annual compliance visit was documented on 10/9/24. The facility’s corporation status was verified current with the NC Department of the Secretary of State on 10/6/25. Upon arrival, I was greeted by Brittany Jackson, Director. I stated the purpose of my visit and completed a walkthrough of the indoor and outdoor licensed spaces. There were 41 children present, ages 0-4 years old. Children were observed engaged in group time and playing outside with a variety of toys. Infants were observed walking around the classroom and playing with a variety of toys. Teachers were nearby singing and playing with them. A diaper change was also observed. There were current records of arrival and departure times, there were also documented safe sleep records on file. Current activity plans were posted, as well as a current menu. I reviewed nine children’s files, there were no new staff files, and I also reviewed four staff files during today’s visit. Staff had current qualification letters, CPR/FA certificates and IT’s SIDS for those required were on file. Health and safety training requirements were current. A sanitation inspection was completed on 7/11/25 and the facility received a Superior rating. Outdoor inspections, fire drills were documented monthly however quarterly drills were not current. No lead-based paint and no asbestos hazards were identified at this location. Lead in Water testing was completed - 1/31/25 Required items were posted and current. There were three items of non-compliance observed today, and the corresponding violations were cited. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). B. Jackson's qualification letter was expired. G.S. 110-90.2(b) & .2703(n)&(o) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. An emergency drill was not practiced every three months, the last documented drill was 4/2025. .0604(u);.0302(d)(8) Violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 10/20/25, describing how and when the violation(s) were corrected. Today I provided TA regarding: ABCMS and I sent you the instructions to get the training and roster completed. The Rated License Process and the Pathways: you requested an additional visit to get clarity for you and the other Directors. Allergy Postings - I advised you to identify allergies vs parental preferences on the posted list. Emergency Medications and Medical Action plans - I also reminded you that if a child requires emergency medications as indicated on the application, that medication and action plan must be on site on the child's first day of school. You were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Currently this center operates with a Five Star License, issued July 16, 2019 earning 6 points in the Education Component, 6 points in the Program Component (meeting enhanced space) and 1 Quality point for having a staff benefits package and an infrastructure of parent involvement. Ms. Brittany Jackson, Director greeted me. Ms. Brittany Jackson accompanied me during my walk through the facility. Ms. Deloris arrived during my visit. There were 61 children present during today's visit. I observed the children playing outside and participating in free choice and teacher directed activities, completing bathroom routines, eating lunch and napping. Limited monitoring of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. I reviewed three staff files. The sanitation inspection was completed on April 1, 2024 with a "Superior" classification with 9 Demerits. A satisfactory fire inspection was completed on February 28, 2024. During today's visit, violations of child care requirements were cited. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). S. Jackson (DOB: 12-22-69) did not complete and submit required forms to complete a criminal background check prior to the expiration on 5-7-24. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. The qualification letter for S. Jackson (DOB: 12-22-69) expired on 5-7-24. A valid qualification was not on file for review. G.S. 110-90.2(b) & (d) & .2703(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before May 29, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. Please email the compliance letter to: sharon.y.williams@dhhs.nc.gov. The qualification letter for S. Jackson (DOB 12-22-69) expired on 5/7/24. I checked the criminal background check in the ABCMS system and S. Jackson was previously qualified to work in child care on 5/7/19. A checklist was used to note the requirements I monitored today. Currently this center operates with a Five Star License, issued July 16, 2019 earning 6 points in the Education Component, 6 points in the Program Component (meeting enhanced space) and 1 Quality point for having a staff benefits package and an infrastructure of parent involvement. There were 44 children present during today's visit. I observed the children completing bathroom routines and participating in free choice and teacher directed activities. A satisfactory fire inspection was completed on September 12, 2023. The sanitation inspection was completed on April 11, 2023 with a "Superior" classification with 3 Demerits. During today's visit, violations of child care requirements were cited. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The fence on the school aged playground was lending and not secure. G.S. 110-91(6); .0601(b) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Rated License Process The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year won’t begin until July 1, 2024, the preparation year for cohort one began July 1, 2023.
