I completed an Annual Compliance Monitoring Checklist for Centers during today’s visit, and a completed copy was emailed to you. Upon arrival, I was greeted by Donnishia Casterlow, Administrator, and Jaylin Wood, Assistant Administrator. Both assisted me with the visit. Sixty children were present today. During the visit, children were observed engaged in group time, teacher directed activities, playing in a variety of centers, transition activities, eating lunch and resting. Children were being attended to according to their individual needs. Infants were sleeping while others were engaged in floor activities and arrival transitions. Staff were observed engaged in activities with the children and were positive in their interactions with the children. Your program currently operates with a five-star license, issued 8/22/2024. The last annual compliance visit was conducted 4/8/25. The sanitation inspection was completed 10/29/2025 with a superior classification. The last fire inspection was conducted on 7/1/2025 and your facility was approved. The center's compliance history was reviewed with the administrator. The program’s compliance history was 98% as of 3/17/2026. Nine children’s files and three new staff files were reviewed. The Staff and Training Worksheets were also reviewed with the administrator. Required items were posted and current. Outdoor inspections were documented monthly. There were five items of non-compliance observed today. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A crib in the infant classroom was positioned in a location that obstructed the staff member being able to see that child. During the visit the teachers was at the diaper changing table and was unable to see the sleeping child. The teacher repositioned the crib during the visit. .1801(a)(1-5) 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. On the playground used for school-agers and pre-k children mulch did not measure nine inches in depth. The provider has roped off the area, and children are not permitted to play on he structure until mulch is delivered and installed. .0605(j) 807 A safe indoor environment was not provided for the children. A metal support pipe for an umbrella, approximately eight inches in height, was observed on the two-year-old playground. A gate on the toddler playground that leads to the parking lot was observed without a lock. 10A NCAC 09 .0601(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. A cabinet containing diaper creams was unlocked in the infant room. The assistant administrator added a lock to the cabinet during the visit. 15A NCAC 18A .2820(d) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Three staff were not linked to the facility. G.S. 110-90.2 & .2703(r) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 03/30/2026, describing how and when the violations were corrected and how compliance will be maintained in the future. Your letter should include facility name, facility ID, and original signature. Submit the signed letter electronically to my email address. If I do not receive your letter prior to 3/30/2026, a return visit to your program may be conducted to assess compliance with the violations. We discussed the following items of technical assistance today: - Linking Criminal Background Checks: New staff (within the last year) must be reported to the Division within 5 days of hire. Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way for staff at the CBC Unit to keep you updated about the status of out-of-state checks if a new employee has a Provisional Qualification. New staff need to link within five days of hire or transfer to your center. We discussed the following items of technical assistance today: - Linking Criminal Background Checks: New staff (within the last year) must be reported to the Division within 5 days of hire. Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way for staff at the CBC Unit to keep you updated about the status of out-of-state checks if a new employee has a Provisional Qualification. New staff need to link within five days of hire or transfer to your center. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. Three new staff were not linked to the programs facility. 10A NCAC 09 .2703 CRIMINAL HISTORY RECORD CHECK REQUIREMENTS FOR CHILD CARE PROVIDERS (r) Operators shall notify the Division of all new child care providers, as defined in G.S. 110-90.2(a)(2), who are hired or have moved into the child care facility within five business days. - Supervision: Supervision was discussed in detail, along with examples of inadequate supervision. Children shall be adequately supervised at all times. Adequate supervision shall mean that staff interact with the children while moving about indoor or outdoor areas and are able to always hear and see the children, except when emergencies necessitate that direct supervision is impossible for brief periods of time. A crib in the infant classroom was positioned in a location that obstructed the staff member being able to see that child. During the visit the teachers was at the diaper changing table and was unable to see the sleeping child. The teacher repositioned the crib during the visit. - Medications: A cabinet containing diaper creams was unlocked in the infant room. The assistant administrator added a lock to the cabinet during the visit. We discussed the following consultation items today: -NC WORKS- All staff need to have updated information in NC WORKS showing their most recent education. This information will be needed for the QRIS Rated License and will be determined by the information available in WORKS.     Name of Operation: CHILDCARE NETWORK #56 Facility ID: 41002494 Consultant: MARY MCMILLAN Operation Type: Center Case Number: Visit Date: 7/16/2024 Number Present: 44 Completed Date: 7/16/2024 Age: From 0 To 9 Total Minutes: 165 Time In: 02:15 PM Time Out: 05:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable childcare requirements during a Temporary Time Period visit due to a Change of Ownership due to paperwork on 5/1/24. Today I met with Donnishia Casterlow, Director. Ericka Bailey, Child Care Consultant, accompanied me during today’s visit. Ms. Casterlow walked us through the building today and we observed all classrooms of children. The children were observed during routine activities, teacher directed activities, eating and playing. Limited monitoring of child care requirements was done. The following child care requirements were monitored: Supervision, Staff/Child Ratio, Hazardous items storage, Age-Appropriate Activities, Appropriate Discipline, required items posted and license posted. A sampling of records were monitored: 10 new staff files, since the last visit in December 2023. The last fire drill was recorded on 6/21/24 and the last shelter in place drill was recorded on 6/15/24. Ms. Casterlow stated that they are no longer using the pool at the center and that it is covered. The following violations of child care requirements were cited during today's visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member had her medical statement after starting at the facility. This was corrected during the visit. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member had TB test several weeks after she was hired back in February. This was corrected during the visit. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks, for one staff member there was no record of orientation. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. There was no verification on file for one new staff member. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. There was no verification on file for one new staff member. .1102(d) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. In the classroom with school-aged children a staff member had a drink from a fast food restaurant. This was corrected during the visit. .0901(i) The following consultation was discussed: -You must have your new building, fire and sanitation inspections completed before October 2024. However, if you have them completed early, you may transition back to your star rated license once they have been received and submitted to me. I received your sanitation inspection report during today’s visit and you are waiting for fire and building inspectors to come back. -You will remain in the same cohort as with your previous license. -We discussed that we do not have your pre-service form for administrators, please email me this form. -Please submit your updated staff and training worksheets to me by Monday 7/22/24. We discussed the following Technical Assistance items today: -Staff must not have cups/bottles/cans from restaurants/stores unless it is water or other appropriate drink that the children are allowed to have. Otherwise it would need to be in an non-identifying container. -The fence had several areas where the ground has been washed away or the fence had a hole and will need to be fixed before it becomes an entrapment or other hazard. -Please update your alternate person on your EMC to list a current staff member. Violations must be corrected immediately; you are required to submit a compliance letter stating how the violations are corrected to me at mary.mcmillan@dhhs.nc.gov by 7/30/24.
