I completed an Annual Compliance Monitoring Checklist for Centers during today’s visit, and a completed copy was emailed to you. Upon arrival, I was greeted by the Director, Ms. R. Carroll. Ms. Carroll accompanied me on the walkthrough of the indoor and outdoor environments. There were fifty six children present today. Children were observed engaged in group activities, playing in a variety of centers, outdoor activities and eating lunch. Infants were sleeping and playing with toys, books and pulling up to stand. Children were being attended to according to their individual needs. The staff were observed engaged in activities with the children and were positive in their interactions with the children. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two percent as of 11/14/2025. The NC Secretary of State website was reviewed on 11/17/2025 and was listed as current- active. Your program currently operates with a five-star license, issued 7/6/2024. The last annual compliance visit was conducted 11/19/2024. The sanitation inspection was completed on 10/28/2025 with a “Superior” classification. The last fire inspection was conducted03/11/2025 and your facility was approved for daytime care only. Required items were posted and current. Seven children's records and six staff member's file were monitored. Required program records were also monitored during today’s visit. There were two items of non-compliance observed today. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Two infants were sleeping during the visit. No documentation was completed. .0606(g) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Three staff did not receive 16 hrs. of orientation. .1101(a) Violations documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 12/01/2025, describing how and when the violations were corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. - Orientation: Staff qualifications and training requirements are essential for adults who interact with and teach children. These requirements protect children in childcare facilities by ensuring these facilities provide a physically safe and healthy environment where the developmental needs of children are met and where children are cared for by qualified staff. Each staff member shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. Eleven staff have not received orientation. You can conduct orientation training as a group with new staff instead of one on one. Ensure you and applicable staff members reflect the date, hours, and signature on the orientation training form. We referred to adding any additional information to the bottom of the orientation form that is reviewed with the employees. -Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger. Two children were sleeping during the visit. We discussed: - Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way to for staff at the CBC Unit to keep you updated about the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility were provided today. In the future, new staff need to link within five days of hire or transfer to your center. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. - WORKS: Please ensure that all staff members’ education information is current in the W.O.R.K.S. database. Each staff member must register with the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment before the end of the temporary time period. You may also want to consider having staff members complete the Early Educator Certification process to assist in getting higher education evaluations completed if their degree is not in early childhood education. Early Educator information is available at www.ncicdp.org. WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their workforce information to support education requirements. The WORKS portal is available online and requires that the user maintain an individual North Carolina Identity Management (NCID) username and password. This facility currently operates with a Five Star license issued on July 6, 2024. Upon arrival to the facility, I was greeted by Ms. Rashanea Carroll, Director, who assisted me during my visit today. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. A walk-thru of the indoor and outdoor spaces used by the children were monitored. I reviewed five new staff files. During today’s visit, there were fifty-two (52) children present. I observed the children engaging in various activities such as playing outside, completing bathroom routines, and participating in both teacher-directed and free-choice activities. In the infant room, I noticed one infant and four one-year-olds playing on the floor with the teacher. The staff and child ratios were observed to be in compliance during the visit. I observed the teachers interacting with the children in a nurturing manner. A sanitation inspection was conducted on November 7, 2024, resulting in a superior classification. Monthly fire drills, quarterly disaster drills and playground inspections have been documented. During today's visit, violations of child care requirements were cited. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The peeling paint is accessible to children in several of the classrooms. 15A NCAC 18A .2825(a) 705 Equipment and furnishings were not sturdy, stable and free of hazards. In the classroom for two- to three-year-olds, the area rug in the dramatic play area was raised on one side, creating a potential tripping hazard. .0601(c) 807 A safe indoor and outdoor environment was not provided for the children. On the playground for two- to three-year-olds, there was a board that was coming loose from the wall border. 10A NCAC 09 .0601(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the four-year-old classroom, an outlet was left uncovered. The director promptly corrected the issue by placing a safety plug in the outlet to ensure the children's safety. 10A NCAC 09 .0604(c) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on July 6, 2024. Upon arrival to the facility, I was greeted by Ms. Keisha Dillard, Assistant Director. Ms. Kandace Nelson, Director assisted me during my visit today. A full assessment of child care requirements was conducted during today’s visit. There is one NC Pre K classroom on site. There were 52 children present during the visit. During today’s visit, I observed the children playing outside, participating in free choice and teacher directed activities. Staff and child ratios were observed in compliance during today’s visit. Positive staff and child interactions were also observed. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files was monitored for health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. A sanitation inspection was completed 11/7/24 with a “Superior” classification. A fire inspection was conducted on March 4, 2024. Emergency drills and outdoor inspections were documented and current. A sample of children’s files and staff files were monitored during today’s visit. This facility’s corporation status was verified current on the NC Department of the Secretary of State on November 25, 2024. During today’s visit violations of childcare requirements were cited. Violation Number Comment Rule 604 Lavatories were not kept clean, in good repair and kept free of storage. In one bathroom the soap dispenser was loose from the wall. Corrected during visit by assistant director tightening the screws. 15A NCAC 18A .2818(a) 721 All equipment and furnishings were not in good repair. A full assessment of child care requirements was conducted during today’s visit. There is one NC Pre K classroom on site. There were 52 children present during the visit. During today’s visit, I observed the children playing outside, participating in free choice and teacher directed activities. Staff and child ratios were observed in compliance during today’s visit. Positive staff and child interactions were also observed. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files was monitored for health assessments and developmental screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. A sanitation inspection was completed 11/7/24 with a “Superior” classification. A fire inspection was conducted on March 4, 2024. Emergency drills and outdoor inspections were documented and current. A sample of children’s files and staff files were monitored during today’s visit. This facility’s corporation status was verified current on the NC Department of the Secretary of State on November 25, 2024. During today’s visit violations of childcare requirements were cited.
