A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on July 6, 2024. Upon arrival to the facility, I was greeted by Ms. Carolyn Pryor, Director. Ms. Pryor, Director assisted me during my visit today. A full assessment of child care requirements was conducted during today’s visit. The facility has three NC Pre K classrooms on site. There were 75 children present during the visit. During today’s visit, I observed the children playing outside, participating in free choice and teacher directed activities. Staff and child ratios were observed in compliance during today’s visit. Positive staff and child interactions were also observed. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. A selection of files was monitored for health assessments and developmental screenings. The center uses the Teaching Strategies instrument to document evidence of children's ongoing progress. A sanitation inspection was completed 11/7/24 with a “Superior” classification. A fire inspection was conducted on March 4, 2024. Monthly playground and fire drills have been conducted and documentation was available for review. Quarterly disaster drills have also been documented. A sample of children’s files and staff files were monitored during today’s visit. This facility’s corporation status was verified current on the NC Department of the Secretary of State on December 5, 2024. During today’s visit violations of childcare requirements were cited. Violation Number Comment Rule 536 Formula and other beverages which require refrigeration were not identified for each child or properly refrigerated. In the infant classroom, one unfinished bottle of milk was unlabeled in the mini refrigerator. 15A NCAC 18A .2804(d) 604 Lavatories were not kept clean, in good repair and kept free of storage. In NC PreK-3 bathroom, there there was a gap in the tile between the two toilets. 15A NCAC 18A .2818(a) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. In NC PreK-3 Classroom, the wood panel near the cozy corner is loose. 15A NCAC 18A .2825(a) 705 Equipment and furnishings were not sturdy, stable and free of hazards. In the school age classroom, the hockey table was missing 3 end caps. .0601(c) 807 A safe indoor and outdoor environment was not provided for the children. On 2 outdoor play areas several bolts holding the black borders down were sticking up. 10A NCAC 09 .0601(a) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before December 19, 2024. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date each violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If sufficient information is not received by the due date, a follow-up visit may be completed. Email the information to: sharon.y.williams@dhhs.nc.gov Provider Access Available to the Automated Background Check Management System Child care administrators can access ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Please complete the process for your facility as soon as possible. We discussed the following: 10A NCAC 09 .2703 (r) Operators shall notify the Division of all new child care providers, as defined in G.S. 110-90.2(a)(2), who are hired within five business days. Rated License Assessment Update: Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed.
