I completed an Annual Compliance Monitoring Checklist for Centers during today’s visit, and a completed copy was emailed to you. Upon arrival, I was greeted by the Administrator Ms. Shenora Sprows-Stafford, and the Assistant Administrator, Ms. Tyanna Jacobs, both assisted me with the visit. There were fifty-four children present today. Children were observed engaged in group times, playing in a variety of centers, super bowl team building activities and eating lunch. Infants were sleeping and being held, Children were being attended to according to their individual needs. The staff were observed engaged in activities with the children and were positive in their interactions with the children. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety four percent as of 1/30/2026. Your program currently operates with a five-star license, issued 8/5/2024. The last annual compliance visit was conducted 2/11/25. The sanitation inspection was completed 12/11/2025 with a “Superior” classification. The last fire inspection was conducted, 3/3/2025, and your facility was approved for daytime care only. Outdoor inspections were documented monthly. Required items were posted and current. I reviewed Staff and Training Worksheets with the provider. Six staff member's file were monitored. Required program records were also monitored during today’s visit. There were no items of non-compliance. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. On the playgrounds used for school-age and pre-k children, the mulch underneath and around the structures must be at least 9 inches in depth. .0605(j) 807 A safe indoor and outdoor environment was not provided for the children. There were tree limbs hanging over the fence on the school-age playground. 10A NCAC 09 .0601(a) We discussed the following consultation items today: NC WORKS- All staff need to have updated information in NC WORKS showing their most recent education. This information will be needed for the QRIS Rated License and will be determined by the information available in WORKS. The Workforce Education Unit has recently updated and added resource documents to assist individuals with all Workforce Education Unit processes. Upon arrival at the facility, we were greeted by the assistant administrator, Ms. Tyanna Jacobs. I stated the purpose of the visit. Limited monitoring of child care requirements occurred during today’s visit. Today, a total of fifty-four (54) children were present, ranging in age from 1 to 10 years. I conducted additional observations in the school-age classroom to monitor compliance with requirements related to the reported allegations. In the school-age classroom, twenty (20) children and two (2) teachers were present. The children were observed lining up in preparation for a field trip. Three (3) teachers, along with the center administrator, Ms. Shenora Sprows-Stafford, were scheduled to accompany the children on the trip. I monitored two buses designated for transporting the children. During the inspection, one of the buses was found to have a shattered window at the bottom of the emergency door. Ms. Sprows-Stafford stated that she was unaware of the damage. As a result, the decision was made not to use the damaged bus for transportation. During my observations, I did not observe any violations related to staff-to-child ratios or concerns regarding the nurture, care, or treatment of children in the classroom. I conducted interviews with staff members and reviewed the allegations with the center administrator, Ms. Shenora Sprows-Stafford. The administrator stated that all school-aged children are allowed to participate in field trips. The facility makes efforts to accommodate the needs of all children by assigning additional staff when necessary. In cases where children have challenging behaviors or identified special needs, parents or other caregivers are encouraged to accompany their child on the field trip. Ms. Sprows-Stafford stated the facility takes thoughtful and proactive steps to accommodate children with special needs. I reviewed four (4) children’s files, including: Records of parent conferences Field trip calendars Off-premises permission forms Children’s sign-in and sign-out sheets Staff timecards Findings: During the visit we were unable to determine violations regarding staff-to-child ratios, or the nurture and care of children. Therefore, the complaint could not be substantiated. However, one (1) violation was identified and was not related to the complaint. This violation was cited and corrected during the visit. Violation Number Comment Rule 481 A list of all children participating in the off premise activity was not available at the center. A list of all children participating in the off-premises activity was not available at the center. This violation was corrected during the visit. There are two NC Pre-K classrooms at this facility. A checklist was used to note the requirements I monitored today. This center currently operates with a Five Star-Rated License effective August 5, 2024. Upon arriving, I was greeted by Ms. Shenora Sprows-Stafford, the Director. I then conducted a full assessment of the child care requirements and completed a walkthrough of the facility's indoor and outdoor areas. During the visit, 42 children were observed. I observed the children participating in free choice and teacher directed activities, completing bathroom routines, eating lunch and transitioning to rest time. The children enrolled in the NC PreK classrooms were not present due to the inclement weather policy followed by Guilford County Schools. Emergency drills and playground inspections were documented monthly. A sanitation inspection was completed on 8/29/24 and the facility received a Superior rating. Nine randomly selected children’s files and new staff member’s files were monitored during today’s visit. The vehicles were not monitored today due to inclement weather. These will be monitored during the next visit. The facility has an 18-month compliance history of 89%. The facility’s corporation status was verified as current and active on the NC Department of the Secretary of State today, 2/11/25. During today's visit, violations of child care requirements were cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the school aged outdoor playground, the wooden borders were partially obscured by grass, yet remained visible in certain spots, creating a potential tripping hazard. 10A NCAC 09 .0601(a) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. The medical health assessment for one child was on file 30 days after admission. The medical health assessment for two children did not have the required information. Upon arriving, I was greeted by Ms. Tyanna Jacobs, the Assistant Director. I then conducted a partial assessment of the child care requirements and completed a walkthrough of the facility's indoor areas. Shortly after, Ms. Shenora Sprows-Stafford, the Director, joined me to finish the walkthrough. During the visit, 68 children were observed. The children's activities included diapering, bathroom routines, lunchtime, and napping. In the NC Pre-K 2 classroom, one group of children engaged in a teacher-led activity while the other group participated in free-choice activities. In the NC Pre-K 3 classroom, the children took part in a teacher-directed activity. I reviewed four new staff files. During today's visit, I observed the following items to be in compliance: supervision, appropriate staff-to-child ratios, adequate space capacity, proper storage of medications, use of approved spaces, a posted permit, and adherence to permit restrictions. During today's visit, violations of child care requirements were cited.
