A checklist was used to note the requirements I monitored today. This facility currently operates with a four-star license issued 12/13/18. This facility has a 95% compliance score as of 6/15/26. The last annual compliance visit was on 07/11/25. As of 6/15/26 the facility was current and active according to the Secretary of State website. The last sanitation visit was 4/1/26 and resulted in a Superior rating with four (4) demerits. The facility tested negative for lead in water 2/3/26 and is exempt for lead in paint and asbestos. Upon my arrival, I was greeted by Natasha Whitaker, Director. I informed Ms. Whitaker, of the purpose of today’s visit. Ms. Whitaker gave me the necessary paperwork I needed to complete today’s visit. The last fire drill was conducted 6/12/26. The last emergency drill(lockdown) was conducted on 6/12/26. The last playground inspection was 6/12/26. The last fire inspection was 12/19/25. Fifty-seven (57) children were present during today’s visit. I observed the indoor and outdoor spaces. I observed both staff/child ratio and supervision and found both to be in compliance. During today’s visit, I observed infants napping and being fed and toddlers playing in centers and dancing at group time. In the preschool classes children were eating snack and transitioning to group time. All teachers interacted with the children in a pleasant manner. Outdoor there is ample room to move around and a variety of materials and equipment. All medication and hazardous items were out of the children's reach. Staff and children’s files were monitored during today’s visit. Seven (7) children files were reviewed. The staff and training worksheets were reviewed during today’s visit. Violation Number Comment Rule 893 A copy of the safe sleep policy was not given to and/or explained to the parent of each child on or before the first day the infant attended the center. One child did not have a signed sleep policy in their file. 10A NCAC 09 .0606(c) To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. On this license you received 4 points for program standards, 6 points for education and 1 point for the Quality point (staff benefit package/infrastructure for parent involvement). Restrictions on this license include: First shift (daytime care) only, enhanced ratio and enhanced space. Application • The application was originally submitted on 5/23/26. • It was corrected and resubmitted on 6/16/26. • The facility is applying for Pathway #2. • The facility uses the Early Learning Matters and the Desired Results assessment tool. Family and Community Engagement Standards • The facility is meeting the following items: All four choices in Communication Options, In Engagement and Leadership Options they have marked two family events annually, a family event that is culturally responsive, the administrator, Lead Teacher, families and service providers work collaboratively to ensure the children’s needs and goals are being addressed in the classroom. For Educational Opportunity Options, they marked choices: one educational opportunity annually for staff and families that includes the participation of a local community resource and paid planning time for lead teachers. • It appears the facility is eligible for five stars in this category. Facility CQI plan • The facilities goal for the current year is for all Teachers and Lead Teachers to complete at least 12 semester hours in ECE/CD. • Individual CQI plans are being maintained in staff folders. Staff Education • Ms. Whitaker and I completed the Staff Information and Education Worksheet together. • The facility has seven (7) Lead Teachers and six (6) staff in the Other category. • It appears the facility qualifies for a four star in education. • Ms. Whitaker has chosen for herself and all Lead Teachers to receive 5 additional training hours per year. Overall, this center may qualify for a Four-Star License. The Administration of the NC Division of Child Development and Early Education must approve this assessment prior to issuance of the license. I was greeted by Natasha Whitaker. This center currently operates with a Four-Star License since 12/13/19. As of 1/20/26, the facilities compliance score was ninety-five (95) percent. This facility’s corporation status was verified current and active on the NC Department of the Secretary of State on 1/20/26. I verified the lead in water was in progress as of 1/5/26 and the facility has started enrollment for lead-based paint and asbestos testing. The last fire inspection occurred 12/19/25 and the last sanitation inspection occurred 10/21/25 with a superior rating and zero (0) demerits. Fifty-six (56) children were present. Ms. Whitaker accompanied me on a walk-through of the spaces used by children, both inside and outside. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. Children were participating in free play activities, playing outside, and playing with table top toys. The staff/child interactions I observed were warm and caring. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. The last fire drill was 12/11/25. The last disaster drill was 9/30/25. The last playground inspection was 12/30/25. There was one violation cited during today's visit. Violation Number Comment Rule 1868 When equipment on the outdoor learning environment met one of the exceptions outlined in .0605(m)(1-4), the protective surfacing did not meet the specifications in rule. There was not enough protective surfacing (wood mulch) under the swings on the preschool playground. .0605(m)(1-4) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS110-90(4) to achieve and maintain an eighteen-month compliance history score of at least seventy-five (75) percent. Any violations documented may impact the compliance history score. To maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A checklist was used to note the requirements I monitored today. This facility currently operates with Four Star license issued on January 3,2024. Upon my arrival to the facility, I was greeted by Ms. N. Whitaker, the administrator. Ms. Whitaker assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Fifty-one (51) children were present during today’s visit. I observed the children playing on the floor with available toys, playing in centers, playing outside on the playground, washing their hands in preparation for lunch, eating lunch and napping. A sanitation inspection was conducted on January 30,2024. This facility received a superior rating with three (3) demerits. A fire inspection was conducted on October 23,2023. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as July 15,2024. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. There was a swing structure that has less than one inch of mulch under the climbing structure. .0605(j) 807 A safe indoor and outdoor environment was not provided for the children. There were broken containers and lids with sharp edges. and two plastic bags of sand on the playground. The plastic is a choking hazard. 10A NCAC 09 .0601(a) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. There were foam blocks in a two year old classroom. .0604(q) The above violations must be corrected immediately, and a letter must be email to me no later than July 29,2024 stating how each item was corrected. My email address is wytina.blackmon@dhhs.nc.gov If I do not receive your letter by July 29,2024 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Whitaker that the staff members need in-service training hours. It-SIDS, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. Rated License Assessment: I also discussed with Ms. Whitaker the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. Ms. Whitaker informed me that she needs to think about if she wanted to continue the rated license process. I informed Ms. Whitaker to contact me with her decision about the assessment. Education Standards: Today we discussed the education of the current staff. I was unable to verify the staff education due to our workforce database. Ms. Whitaker asked what education requirement did her current staff need to maintain the facility’s four star license without the environmental rating scales assessment. The lead teachers must meet seven points in education. This means that 75% of the lead teachers would need an associate degree or higher. The teachers meet seven points in education. This means that 50% of the teachers would need the credentials plus six additional hours or higher in education. Thank you for your time during today's visit.
