Name of Operation: BRISTOL EARLY LEARNING CENTER Facility ID: 41001917 Consultant: SAMANTHA MCLEOD Operation Type: Center Case Number: Visit Date: 2/14/2025 Number Present: 47 Completed Date: 2/14/2025 Age: From 0 To 5 Total Minutes: 300 Time In: 10:00 AM Time Out: 03:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for compliance during an annual compliance visit. NC Pre-K requirements were also monitored for compliance during today’s visit. A checklist was used to note the requirements I monitored today. This facility is owned and operated by GenerationED. This center currently operates with a Five Star license reissued on May 28, 2024. Upon arrival to the facility, I was greeted by the Center Administrator, Ms. Rosalan Barnhill. Ms. Barnhill assisted me during my visit today. A walk-thru of the indoor and outdoor spaces used by the children was conducted. Forty-seven (47) children were present during today’s visit. During today’s visit I observed the children during center play, small group activities and outdoor play. Positive interaction was observed between the staff and the children. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. A sanitation inspection was conducted on October 15, 2024. This facility received a superior classification and two (2) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. A satisfactory fire inspection was completed on February 7, 2024. This facility was approved for daytime care only. This facility’s corporation status was verified current on the NC Department of the Secretary of State on February 10, 2025. The following violations of childcare requirements were cited today. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A large hole was observed on the playground #4, this is considered a tripping hazard. 10A NCAC 09 .0601(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member did not have current First Aid training as required. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member did not have current CPR training as required. .1102(d) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. One staff member working in the space for infants did not have current SIDS training. .1102(f) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by February 26, 2025. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. Health & Safety Requirements were also reviewed. RATED LICENSE UPDATE: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. As discussed with you today it is important to have all your staff use this time to submit updated educational information to our Workforce system. All staff will need to have a current Workforce letter on file. It is also suggested that you visit the NCRLAP (www.NCRLAP.org) website for upcoming training and view their resources for the new Infant/ Toddler & Early Childhood assessment tools (ITERS-3 and ECERS-3). MEDICATION: Medicines can be crucial to the health and wellness of children. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. Prevention is the key to prevent poisoning by making sure medications are inaccessible to children. Prescribed medications as well as over the counter (OTC) medicines must be labeled properly when brought to the childcare facility by the parent/guardian. The medication must be in the original container (with a label that includes the child’s name, date filled, prescribing clinician’s name, pharmacy name and phone number, dosage/instructions, and relevant warnings). It is important that all medications properly contain the child’s first and last name on the medicine and the box (if applicable) to avoid a child receiving the wrong mediation (this also includes OTC medication). During today’s visit I reviewed with you the importance of having a system to monitor the expiration dates of the action plans, mediation authorization and the expiration dates of medicines. We discussed several options for monitoring the classrooms for compliance. STAFF TRAINING: All staff members that have responsibility for planning and supervision of a childcare program; as well staff who work directly with children must complete in-service training annually. --The required number of in-service training hours to be completed yearly is based on the education and/or experience of each individual staff member. --Please use the Record of In-service Training Chart to keep track of your training completed each year. (It is recommended that you use one sheet for each year). Also have at least two years available for review (current year and previous year). --Also please make sure Health & Safety training is documented on the Health & Safety training log with the certificates attached for each staff member. These trainings should be kept accessible. --If you receive more than the required number of hours required, you may only carry forward half of what you are required for the next year. *For a complete review of the number of annual training hours required please review child care rule.1103 (On-going training and professional development requirements). A checklist was used to note the requirements I monitored today. This facility currently operates with a Five Star license issued on March 26,2020. Upon my arrival to the facility, I was greeted by Ms. K. Reid, the family advocate. Ms. Reid informed me that Ms. R. Barnhill the administrator was at a meeting. Ms. Reid contacted Ms. Barnhill and informed her of my arrival. I spoke with Ms. Barnhill on the telephone, and she informed that Ms. P. Barrett was the person in charge when she is not present in the building. Ms. Barrett gave me the necessary paperwork for me to complete my visit. Ms. Barnhill arrived before and assisted me during my departure and assisted me for the remainder of my visit my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. During today’s visit, I observed the children playing outside on the playground, washing their hands in preparation for lunch, eating, lunch, preparing for naptime, napping, waking up from napping and departing. Forty-four (44) children were present during today’s visit. A sanitation inspection was conducted on November 20, 23. This facility received a superior rating with seven (7) demerits. A fire inspection was conducted on April 26,23. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current as February 19,2024. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There were broken toys on the playground. There 3 broken hula hoops, white crack lid, and white bucket. These items had sharp edges. 10A NCAC 09 .0601(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There was Zyrtec in an unlock the medication bag. 15A NCAC 18A .2820(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One child's an Inhaler medication that was not returned to a parent. The authorization had expired. .0803(12) A corrective action letter is not required for this visit since three violations cited were corrected during today's visit. Consultation: I discussed with Ms. Barnhill that the staff members need in-service training hours. It-SIDS, CPR and First Aid Trainings by specific dates. Rated License Assessment: I also discussed with Ms. Barnhill the Rated license assessment process will begin July 1,2024. I informed Ms. Barnhill this facility is in Cohort 2. Today I discussed with Ms. Barnhill about the Prep year assessment. Ms. Barnhill informed me that she would let me know when she was ready to request the assessment. Your prep year is July 1,2025 to June 30,2025 and your assessment year is July 1,2025 to June30, 2026. You must let me know if you ready to start the process before June 30,2024.
