This NC Pre-K currently operates with a Five Star license issued on June 21, 2024. This NC Pre-K has two licensed spaces, room #517 & #523. Upon arrival to the licensed space, I was greeted by the lead teacher in room #523 Ms. Ebrahim and Ms. Douglas in room #517. Ms. Ebrahim assisted me during my visit today. I informed you of the purpose of today’s visit. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. A walk-thru of the indoor spaces used by the children was monitored. Thirty-two (32) children were present during today’s visit. I observed the children transitioning from lunch, completing bathroom routines, and resting on cots. A sanitation inspection was conducted on November 20, 2025. Monthly playground inspections and quarterly disaster drills have been conducted and documentation were available for review. The last documented fire drill was conducted on 3/25/26. All required posted information was observed current and posted in the classrooms. During today’s visit one item of noncompliance was cited. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last documented fire drill was conducted on 3/25/26. .0604(t); .0302(d)(5) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before June 4, 2026. The letter of compliance should contain the following information: 1. Pathways to the Stars During today’s visit, information regarding the Pathway to Stars program and the updated rules in Section .3200 was provided. Instructions were shared for accessing resources on the DCDEE website. I informed you that the new rule rollout module is available on the DCDEE e-learning Moodle platform for review. The GCS Central Office Administrators have selected Program Assessment Pathway for this program. Your last ERS was conducted on 4/18/24. Your facility transition will begin in January 2027 and must be completed by June 2027. Clean Classrooms for Carolina Kids Lead testing should also be completed every three years for childcare centers. Upon my arrival at the license space, I was greeted by the teachers in Room #523. I informed Ms. Ebrahim of the purpose of today’s visit. In the two classrooms there were a total of thirty-three (33) children present. During today’s visit, I observed the children completing bathroom routines, eating lunch and napping. The teachers interacted with the children in a pleasant manner. The school was having a special event for the parents. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Indoor areas, program requirements, and staff and children’s records were monitored. Current fire drills, shelter-in-place drills, and playground inspections were documented and current. A sanitation inspection was completed 11/12/24 with a “Superior” classification. A satisfactory fire inspection was completed on 4/18/24. All required documents were posted and accessible. Today violations of child care requirements were cited. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. G.S. 110-90.2(b) & (d) & .2703(e) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before December 26, 2024. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date each violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If sufficient information is not received by the due date, a follow-up visit may be completed.
