    Name of Operation: BRIGHTWOOD ELEMENTARY ACES PROGRAM Facility ID: 41001110 Consultant: SHARON WILLIAMS Operation Type: Center Case Number: Visit Date: 5/28/2025 Number Present: 34 Completed Date: 5/28/2025 Age: From 5 To 12 Total Minutes: 90 Time In: 02:00 PM Time Out: 03:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced A routine unannounced visit was conducted today to assess compliance with applicable childcare requirements for an ACES program. This program is operated by Guilford County Schools. This ACES program currently operates with a Four Star license issued on April 16, 2020. The primary space for this program is cafeteria. Upon arrival, I was greeted by the Program Coordinator Mr. Eric Price. Mr. Price assisted me during my visit today. I made you aware of the purpose of today’s visit. All approved spaces were monitored. A sanitation inspection was conducted on March 27, 2025. This facility received 6 demerits and a superior rating. Thirty four (34) children were present during today’s visit. All indoor and outdoor spaces used by the children were observed today. During today’s visit, I observed the arrival of the children, transition times, snack time and small group activities. I also observed the children completing bathroom and hand washing routines. The caregivers interacted with the children in a pleasant manner. Today you stated that the children do not play on the playground equipment. A variety of toys are taken outside. The children play in the area with the basketball goal. All required posted information was observed current and posted in the classrooms. One violation of child care requirements was cited today. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A documented playground inspection was not completed for April 2025. .0605(q) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before June 11, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If sufficient information is not received by the due date, a follow-up visit may be completed. Email the information to: sharon.y.williams@dhhs.nc.gov We discussed: ABCMS Provider Portal North Carolina Child Care Providers must obtain access to the ABCMS Provider Portal in order to meet the requirement to report new employees within five days of hire. Access to the Provider Portal is granted by CBC Unit staff after providers have completed the required Moodle training and submit the signed Powerform available at the conclusion of the training. Additional information and instructions are available in this ABCMS Provider Portal Guide. Once you have been notified that access has been granted, log into the provider portal to locate the unique provider code for the facility. Applicants and employees will use this code to create a connecting application to be associated with your facility. Clean Classrooms for Carolina Kids Due to the recent impacts of Hurricane Helene and other natural disasters, the Clean Classrooms for Carolina Kids™ enrollment deadline has been revised statewide, and schools and child care facilities that have not yet enrolled should complete the online process by May 31, 2025. Enroll now https://www.cleanwaterforcarolinakids.org/enroll The lead-based paint and asbestos enrollment surveys must be completed for ALL schools and childcare facilities within school districts prior to receiving on-site inspections. As this is a legislatively mandated effort, facilities are required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina Kids™ is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. This program is free, covers testing, inspections, and mitigation, and is intended to improve child-occupied facilities across North Carolina. Some programs have only completed the process for lead testing in water. Testing includes lead in water, lead based paint, and asbestos. You should complete the online process for all 3 by May 31, 2025. In addition, testing should also be completed every three years for childcare centers.
