This facility currently operates with a Four Star license issued on October 28, 2019. Upon arrival at the facility, I was greeted by T. Lyons and S. King. The owner/administrator Mrs. Judith Upchurch was not present upon my arrival but arrived later. I conducted a partial assessment of the childcare requirements and completed a walkthrough of the facility's indoor and outdoor areas. Today 10 children were observed. I observed finishing lunch and transitioning to nap. The sanitation inspection was completed 2/5/26 with a “Superior” classification. During today's visit, one violation of child care requirements was cited. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. On the outdoor playground, the plastic basketball goal and the cubed shaped play structure were broken. G.S. 110-91(6); .0601(b) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented was corrected during the visit by the administrator removing it from the playground. A compliance letter is not required. Annual License Fees Per North Carolina General Statute § 110-90(1a) child care facilities are assessed an annual license fee based on the licensed capacity as of October 1, 2025. The fee is based on the capacity printed on the child care license. Invoices were emailed to the facility email address on or before November 3, 2025. Today we discussed your annual fee which has not been paid. If fees are not paid, the Division may send notices of Revocation in April. Please submit your payment online at https://ncchildcare.ncdhhs.gov/Services/Licensing/Annual-License-Fee as soon as possible. Clean Classrooms for Carolina Kids The lead-based paint and asbestos enrollment surveys must be completed for childcare facilities. As this is a legislatively mandated effort, facilities are required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina Kids™ is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. This program is free until July 2026, covers testing, inspections, and mitigation, and is intended to improve child-occupied facilities across North Carolina. At this time, your facility has completed only the lead-in-water testing component. Required testing includes lead in water, lead-based paint, and asbestos. Please complete the online enrollment process for lead-based paint and asbestos testing. In addition, testing should also be completed every three years for childcare centers. ABCMS Child care administrators can access ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Please complete the process for your facility as soon as possible. Pathways to the Stars We reviewed the updated rules in Section .3200 regarding the Standards for Two- through Five-Rated facilities (Pathway to Stars), and I demonstrated how to access the relevant information on the DCDEE website. A new rule rollout module is now available on the DCDEE e-learning Moodle platform. As discussed, the facility should carefully consider the available QRIS options and determine which pathway it plans to pursue for its rated license reassessment. While the reassessment process may begin at any time, the facility should be prepared to submit the Application for Assessment for a Rated License between April 1, 2026, and September 30, 2026. The following steps are how you can begin preparing for the Rated License process: Today you chose Pathway 1: Program Assessment Pathway • Complete the “Application For Assessment for a Rated License for Centers” electronically and submit to your child care consultant no later than, June 30, 2026. • Complete the “Environmental Rating Scale (ERS) Assessment Request” form electronically and submit to your child care consultant no later than, June 30, 2026. • Confirm all staff have active WORKS accounts. • Complete the QRIS Staff Information and Education Worksheet electronically. • Begin planning for your three-month self-study using the applicable ITERS-3, ECERS-3, books. • Prepare for Environmental Rating Scale (ERS) assessments. • Access resources, trainings, and outreach via ncrlap.org. • Work with CCR&R or Smart Start for support in preparation. • Identify CQI goals and begin planning for implementation. • Develop strategies for Family and Community Engagement. • If four-year-olds are enrolled, select and implement an approved curriculum. Name of Operation: BABY'S WORLD #2 Facility ID: 4155201 Consultant: SAMANTHA MCLEOD Operation Type: Center Case Number: Visit Date: 5/31/2024 Number Present: 11 Completed Date: 5/31/2024 Age: From 3 To 5 Total Minutes: 60 Time In: 11:00 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced A routine unannounced visit was conducted today to assess compliance with applicable childcare requirements. This facility currently operates with a Four Star license issued on October 28, 2019. Upon arrival to the facility, I was greeted by the administrator Mrs. Upchurch. Mrs. Upchurch assisted me during today’s visit. A walk-thru of the indoor and outdoor spaces used by the children was monitored (11) children were present during today’s visit. The children were observed playing in various activity areas, during transition times and bathroom routines and mealtime. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. A sanitation inspection was conducted on March 26, 2024. This facility received a superior classification and six (6) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. The following violations of childcare requirements were cited today: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One employee did not have a medical assessment on file. 10A NCAC 09 .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One new staff member did not have documentation of completion of staff orientation on file. .1101(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Several staff members did not have complete/update Child Maltreatment training as required. .1102(g) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Staff members did not have current health and safety training on file. .1103(b) Child Care facilities must maintain compliance with applicable childcare requirements at all times. In order to maintain compliance with North Carolina childcare laws and rules, the above violation must be corrected immediately. Please send a letter of compliance stating how the above violation was corrected and what you will do in the future to prevent this violation from being cited. Please email me this letter by June 14, 2024. **You also have one employee whose criminal qualifying letter will expire in June, 2024. Please send me a copy of their updated qualifying letter once it has been received. CONSULTATION AND RESOURCES: Technical assistance regarding how to achieve and maintain compliance was offered. Health & Safety Requirements were also reviewed. RATED LICENSE UPDATE: The hold harmless legislation was extended and signed into law on Monday June 12, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in Cohort Two, the preparation year for Cohort Two begins July 1, 2024 and the reassessment year will begin July 1, 2025. I will reach out to you prior to the beginning of the preparation year of your assigned cohort and will provide guidance to assist you as you begin the process. It is recommended that you use your preparation months to determine steps you need to take to maintain or increase your current star rating. Make sure all staff have updated WORKS accounts. This is a great time to encourage staff to take additional college coursework. I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2025, and June 30, 2026. --Have all staff update their WORKS account to reflect all currently completed coursework (DCDEE-WORKS) --Determine whether you wish to have the Environment Rating Scale (ERS) Assessment conducted. --Request technical assistance with your child care consultant and local partners --Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) --Reach out to your local Community College to discuss educational opportunities --Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. --Consider requesting an ERS assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways. ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality. If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment. Assessment scores can be saved to use during the reassessment year. Facilities can request another ERS assessment (free of charge) in your reassessment year for any ERS assessment scoring less than a 5.0. UPDATED RULES: Effective July 1, 2023, we have new rules posted on the DCDEE website. The changes are within the rules for: staff/child ratios and group sizes .0713, cooperative arrangements .2513, and enhanced space requirements .2809.
