A checklist was used to note the requirements I monitored today. This facility currently operates with a Notice of Compliance issued on November 18,2023. Upon arrival, I was greeted by Ms. L. Lasovsky, Preschool Director. Ms. Lasovsky greeted me and invited me into her office. I introduced myself and informed Ms. Lasovsky the purpose of today’s visit. Ms. Lasovsky gave me the necessary paperwork I needed to complete today’s visit. Ms. Lasovsky accompanied me on the walkthrough of the facility and assisted me for the remainder of the visit. Sixty- four (64) children were present during today’s visit. During today’s visit, we observed the children being fed, playing on the floor with available toys, painting, listen to a story, playing outside on the playground, washing their hands in preparation for lunch, eating lunch, napping and departing. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. A sanitation inspection was conducted on August 12,2024. This facility received a superior rating with nine (9) demerits. A fire inspection was conducted on January 3,2024. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. There were diaper creams and sunscreens not returned to the eight parents after the course of treatment was completed. .0803(12) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff did not obtained a valid qualification letter prior to the expiration date. The qualification letter expired October 10,2024. G.S. 110-90.2(b) & .2703(n)&(o) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. This facility did not have documentation of shelter-in-place or lockdown drills were practice every three months as required. The last documented practice drill was April 2024. .0604(u);.0302(d)(8) The above violations must be corrected immediately, and a letter must be sent to me no later than November 21,2024 stating how each item was corrected. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Lasovsky several staff members CPR/First Aid and criminal record rechecks, will expire by specific dates. Please refer to the staff/training worksheet for the expiration dates. During today’s visit, there were several medications that did not have authorization for the tropical creams, sunscreens and the creams were no longer being used. These topical creams have not been returned to the parents. You must return topical creams and sunscreens back to the parents within in 72 hours of the authorization expiration date or when the topical creams are no longer being used for the children. There are several children’s topical creams, and permissions to admission form and medical action plans and expirations of medications are due by specific dates. Please have each teacher to check their forms. Transportation: Today we discussed transportation requirements. The following requirements must be met and will be monitored: The driver must be: (1) 21 years old (2) have a valid driver's license of the type required under North Carolina Motor Vehicle Law for the vehicle being driven or comparable license from the state in which the driver resides; and (3) have no convictions of Driving While Impaired (DWI) or any other impaired driving offense within the previous three years. (4) have emergency information in the vehicles (5) have current insurance policy for the vehicles These requirements must be met by all drivers, and it is up to you to verify all these requirements have been met. SAFETY PROCEDURES (10A NCAC 09 .1003) (1) A first aid kit must be located in each vehicle used on a regular basis to transport children. The first aid kit shall be firmly mounted or otherwise secured if kept in the passenger compartment. (2) For each child being transported emergency and identifying information must be in the vehicle. (3) Children shall not be left in a vehicle unattended by an adult. (4) Before children are transported, written permission from a parent shall be obtained which shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. (5) When children are transported, staff in each vehicle shall have a functioning cellular telephone or other functioning two-way voice communication device with them for use in an emergency. SEAT RESTRAINTS (10A NCAC 09.1001) (1) Each adult and child shall be restrained with an individual seat belt or appropriate child restraint device when the vehicle is in motion. (2) Only one person shall occupy each seat belt or child restraint device. **You must follow all state and local driving laws. --North Carolina state law requires that all children who are not over age 8 or do not weigh at least 80lbs. must be restrained in a booster or car seat appropriate for the child's weight and height. For more information on how to meet NC child restraint requirements please visit www.buckleupnc.org Staff/Ratio: (section.1004): (1)When children are being transported, the same staff-child ratios required for a child care program applies. Ratios for the youngest child in the vehicle must be maintained at all times. Field Trip Permission form: The following requirements must be included on each permission slip: 1. Written permission from each child's parent to transport. 2. Parents notified when and where child is to be transported; and expected time of departure and arrival, and the transportation provider.
