Name of Operation: ARNETTA'S HOME CHILD CARE SERVICES Facility ID: 41000842 Consultant: WYTINA BLACKMON Operation Type: Center Case Number: Visit Date: 6/10/2026 Number Present: 8 Completed Date: 6/10/2026 Age: From 2 To 9 Total Minutes: 305 Time In: 09:45 AM Time Out: 02:50 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for compliance during an annual compliance with rated license visit. A checklist was used to note the requirements I monitored today. This facility currently operates with Five Star license issued on December 12 ,2018. Upon my arrival, I was greeted by Ms. A. Edwards, one of the lead teachers and owner. I informed Ms. Edwards the purpose of today’s visit. Ms. Edwards gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Eight (8) children were present during today’s visit. They were observed participating in a writing activity, reviewing the letter P, playing with available toys, washing their hands in preparation for lunch, eating lunch, napping waking up from napping, washing their hands in preparation ror snack , arriving from school and eating snack. A sanitation inspection was conducted on October 28,2025. This facility received a superior rating with four (4) demerits. A fire inspection was conducted on May 21,2026. The original will be mailed to Raleigh with this documentation. Staff and children’s files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I monitored a Nissan Pathfinder which this facility uses for field trips. The insurance policy was current for the vehicle. The vehicle met the transportation requirements and was in compliance during today's visit. Four (4) violations were cited during today’s visit. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Identifying information, including children's photograph was not in the vehicle for six children being transported on a field trip. 10A NCAC 09 .1003(d) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff did not complete one of the topic for health and safty as apart of on going traing within five years of previous training. .1103(b) The above violations must be corrected immediately, and a letter must be emailed to me no later than June 24,2026 stating how each item was corrected. If I do not receive your letter by June 24,2026 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Rated License Assessment: I discussed with Ms. Edwards the QRIS requirements. Ms. Edwards stated that she was choosing Pathway 1. During today’s visit, Ms. Edwards decided that she wanted to do Pathway 2. I informed Ms. Edwards that in order to maintain her current Five then she would need to purchase an approved curriculum and formative assessment . She is looking at a few online and will make her decision by June 30,2026. I informed Ms. Edwards to contact me once she has purchased her curriculum. Clean Classrooms for Carolina Kids This facility completed the lead water on November 29,2023. This test should be completed every three years. The facility will need to complete the lead paint and asbestos testing. I informed Ms. Edwards these will be violations. You can check your facility status: https://data.cleanwaterforuskids.org/data/north-carolina Criminal Background As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please plan to complete your staff 5-year re-qualification before the expiration date. You have uploaded all your current to staff and new staff to the ABCMS Provider Portal. You need to upload all new staff to the ABCMS provider and previous staff as they renew their five year criminal qualification letter. Upon my arrival, I was greeted by Ms. A. Edwards, one of the lead teacher/ owner. I informed them the purpose of today’s visit. Ms. Edwards gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. (8) Eight children were present during today’s visit. They were observed sleeping, playing with available toys, washing their hands in preparation for lunch, eating lunch and napping. Limited licensing requirements were monitored. Emergency medications, topical creams, and permissions were observed, Epi-pens were in original packaging and had current permission to admission form and medical action plans. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions; CPR and First Aid Certification. There was one violation cited during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One child had medication that did not have the parent's authorization to administer the form. 10A NCAC 09 .0803(4)(6-9) The above violation must be corrected immediately, and a letter must be email to me no later than March 10,2025 stating how the one item was corrected. If I do not receive your letter by March 10,2025 /or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Edwards the staff members has health safety trainings, health questionnaire and emergency specific dates. Please refer to the staff/training worksheet. Rated License Assessment: I discussed it with Ms. Edwards the Rated license assessment process the hold harmless has been extended until the new QRIS is implemented. This means that you are not required to go any further with a rated license assessment unless you want to proceed. I explained to Ms. Edwards there is a new process that is still being developed. You can find information about the three pathways at https://ncchildcare.ncdhhs.gov under the what’s new tab. Beginning on February 1, 2025, child care facilities will use the Environment Rating Scale – Third Edition for family child care homes, infant/toddler classrooms and preschool classrooms, if requesting a program assessment or reassessment. Name of Operation: ARNETTA'S HOME CHILD CARE SERVICES Facility ID: 41000842 Consultant: SHARON WILLIAMS Operation Type: Center Case Number: Visit Date: 7/25/2023 Number Present: 9 Completed Date: 7/25/2023 Age: From 1 To 6 Total Minutes: 180 Time In: 09:45 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor applicable child care requirements compliance during an annual compliance visit. A checklist was used to note the requirements I monitored today. I also monitored health and safety requirements. This center located in a residence currently operates with a Five Star license issued December 21, 2018, earning 7 points in Education, 5 points in program, and 1 quality point, education option met for the administrator completing business training and a wage/hour training course (at least 30 contact hours) on October 27, 2007. The facility meets enhanced space and enhanced ratios. This center in a residence has capacity for 12 children and is licensed to operate on first, second and third shift. Upon my arrival, I was greeted by, Ms. Arnetta Edwards, the owner. Nine children, Ms. Arnetta Edwards and Ms. Germani Lindsay were present. I observed the children playing outside, completing bathroom routines, participating in teacher directed and free choice activities. The last annual compliance visit was conducted on August 1, 2023. As of July 25, 2023, the 18-month compliance history for the facility is 99%. A full assessment of child care requirements occurred during today’s visit. A sanitation inspection was completed on June 15, 2023 resulting in a "Superior" classification with 6 Demerits. A satisfactory fire inspection was conducted on 1/6/23. The last documented fire drill was conducted on 7/17/23. A shelter in place drill was conducted on 4/24/23. The last documented playground inspection was completed on 7/17/23. The center water was tested for lead on 10/29/20. During today's visit, one violation of the child care requirements was cited. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. A Medication Administration Permission Form was not on file for one child who's medication was at the center. 10A NCAC 09 .0803(4)(6-9) The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before August 8, 2023. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. Please email the compliance letter to: sharon.y.williams@dhhs.nc.gov TA: Rated License Reassessments: The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year will not begin until July 1, 2024, the preparation year for cohort one begins July 1, 2023. Your facility is in cohort one. Beginning in July and throughout the next few months I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. If possible, we will align the reassessment with your annual compliance visit timeframe. • Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) • Determine whether you wish to have Environment Rating Scale (ERS) Assessments [ITERS-R, ECERS-R, SACERS-U, FCCERS-R] conducted • Request technical assistance with your child care consultant and local partners • Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) • Reach out to your local Community College to discuss educational opportunities • Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. • Family child care home providers consider the FCCERS-R self-study process and timeline for completion since it must occur over a 3-month time period prior to your rated license reassessment • Consider requesting an ERS assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways • ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality • If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment • Assessment scores can be saved to use during the reassessment year • Facilities can request another ERS assessment (free of charge) in your reassessment year for any ERS assessment scoring less than a 5.0 If you have questions, please contact me. Sanitation Rules: 1) New Rules---The NC Commission for Public Health has readopted 32 rules in 15A NCAC 18A Section .2800. As a result, centers will have a revised set of Child Care Sanitation rules. The revised Child Care Sanitation Rules were effective beginning July 1, 2023. The information can be found on (https://ehs.dph.ncdhhs.gov/hhccehb/cehu/ccs/children.htm) 2) Lead Water Sampling-- RTI lead water sampling must be conducted every 3 years. For more information go to: Clean Water for Carolina Kids ( https://www.cleanwaterforuskids.org/en/carolina/) 3) New Inspection Requirement-[NC 10A NCAC 41C.1001‐1007]requires lead-based paint and asbestos inspections at all licensed childcare facilities and public schools in NC. In this year’s NC budget, Governor Cooper allocated $150 million dollars(ARPA funds) to address lead and asbestos in childcare and school facilities.
