This facility currently operates with a Five Star license issued on January 4, 2021. Upon arrival to the facility, I was greeted by the administrator, Ms. V. Willis. Ms. Willis assisted me during today’s visit. A partial assessment of child care requirements was conducted. I completed a walkthrough of the indoor areas utilized by the children. The outdoor areas were not monitored today due to the weather. Sixteen (16) children were present during the visit. I observed the children engaged in group activities with the teachers. Supervision and staff-to-child ratios were found to be in compliance. Monthly fire drills, quarterly disaster drills, and playground inspections have been documented. A sanitation inspection was completed on 11/5/25 with a Superior rating. One staff file was reviewed. Two items of non-compliance were cited. Today the violations were corrected during the visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In the space for three and four year old current activity plan was not posted. This was corrected during the visit by the teacher accessing the activity plan from the computer and printing the activity plan. GS 110-91(12); .0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the space for three and four year old children one outlet was uncovered and accessible to the children. This was corrected during the visit by Ms. V. As discussed, the facility should carefully consider the available QRIS options and determine which pathway it plans to pursue for its rated license reassessment. While the reassessment process may begin at any time, the facility should be prepared to submit the Application for Assessment for a Rated License between April 1, 2026, and September 30, 2026. Upon arrival to the facility, I was greeted by the assistant director Ms. Vondelia Willis, I informed you of the purpose of today’s visit. Ms. Willis assisted me during my visit today. A full assessment of child care requirements was conducted during today’s visit. Forty-one (41) children were present today. I observed the children playing outside, completing bathroom routines, eating lunch, and resting. The school-age children were observed leaving for a field trip during today’s visit. A sample of children’s files and staff files were monitored during today’s visit. A sanitation inspection was completed 4/16/25 with a “Superior” classification. A fire inspection was conducted on 2/17/25. Monthly playground and fire drills have been conducted and documentation was available for review. Quarterly disaster drills have also been documented. This facility’s corporation status was verified current and active on the NC Department of the Secretary of State on July 29, 2025. Today violations of child care requirements were cited. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. Peeling paint was observed on the hallway walls. 15A NCAC 18A .2825(a) 721 All equipment and furnishings were not in good repair. On the outdoor lower playground, the bolts anchoring the border were protruding above the surface. G.S. 110-91(6); .0601(b) 847 Parent's medication authorization did not include required information. The duration of authorization for over-the-counter medication was not documented for two children, as required. 10A NCAC 09 .0803(4)(6-9) 1867 The depth of the loose surfacing was not based on critical height of the equipment. Mrs. Baouna assisted me during today’s visit. A walk-thru of the indoor and outdoor spaces used by the children was monitored. Twenty (20) children were present during today’s visit. During today’s visit I observed the children playing on the playground, during center play and during lunch time. I monitored staff/ child ratios and supervision as I walked throughout the facility and found both in compliance. Staff files were monitored for compliance for current Criminal Background qualifying letters and current CPR/First Aid Training as required. A sanitation inspection was conducted on November 26, 2024. This facility received a superior classification and zero (0) demerits. Monthly fire drills and playground inspections have been documented as well as quarterly disaster drills. This facility’s corporation status was verified current on the NC Department of the Secretary of State on February 24, 2025.
