A checklist was used to note the requirements I monitored today. This center currently operates with a Three-Star license, effective September 10, 2018. Upon arrival to the facility, I was greeted by the administrator Ms. Chonda Allred. Ms. Allred assisted me during my visit today. A full assessment of child care requirements was conducted during today’s visit. The last AC visit was conducted on 7/24/25. Twenty children were present today. I observed the children engaging in a variety of activities, including outdoor play, planting plants, completing bathroom routines, participating in free-choice activities, and taking part in teacher-directed learning experiences. A sample of children’s files and staff files were monitored during today’s visit. A sanitation inspection was completed 11/3/25 with a “Superior” classification. A fire inspection was conducted on June 5, 2026. Quarterly disaster drills, monthly playground and fire drills have been conducted and documentation was available for review. Today one violation of child care requirements was cited. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The wall near the back door had peeling paint accessible to the children. 15A NCAC 18A .2825(a) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before July 6, 2026. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. The date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. A sanitation inspection was completed 4/21/25 with a “Superior” classification. A fire inspection was conducted on July 23, 2025. Monthly playground and fire drills have been conducted and documentation was available for review. Quarterly disaster drills have also been documented. Today violations of child care requirements were cited. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before April 10, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. During the visit, I accessed the criminal background portal to gather information on Ms. Neal. Her details are currently being processed, and there is no prior information recorded in the system. Ms. Neal will not be permitted to return to the facility until the criminal background check is finalized and she receives the qualification letter. Childcare administrators can access ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. Due to the recent impacts of Hurricane Helene and other natural disasters, the Clean Classrooms for Carolina Kids™ enrollment deadline has been revised statewide, and schools and child care facilities that have not yet enrolled should complete the online process by May 31, 2025. As this is a legislatively mandated effort, facilities are required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina Kids™ is designed to ensure facilities meet all requirements, receive direct support throughout the participation process, and have access to communication resources. This program is free, covers testing, inspections, and mitigation, and is intended to improve child-occupied facilities across North Carolina. Some programs have only completed the process for lead testing in water. Testing includes lead in water, lead based paint, and asbestos. You should complete the online process for all 3 by May 31, 2025. In addition, testing should also be completed every three years for childcare centers. I was greeted by Ms. Chonda Allred, owner/director of the childcare center. The This center currently operates with a Three-Star license, effective September 10, 2018. An approved fire inspection was conducted on June 18, 2024, and I received a copy. The latest sanitation inspection was conducted on August 12, 2024, with a Superior score. The compliance history is 98% as of August 19, 2024. The NC Department of the Secretary of State list this center’s status as active and current as of August 4, 2024. A full assessment of applicable child care requirements was conducted, I used the Annual Monitoring Checklist during today’s visit. Today thirty-two children are attending. I walked through the center, I observed the children and gathered attendance. Children were participating in group activities, circle time and individual play. School age children were playing outdoors. Currently the center does not have infants enrolled. The classrooms are supplied with activities and toys for the children. I observed the outdoor play area. The playground is scheduled to receive new mulch later in the month. There are several items for the children to chose from for outdoor play. The school age children have a concreted area and a basketball goal. There are items they can take outdoors and there is a climbing structure. I reviewed staff files and a sampling of children files. Program files were current and in order. Your signature on these forms will serve as verification that the information provided is accurate and complete. All staff have CBC Qualification letters. Ms. Allred and I completed the Staff and Training Sheet, and we reviewed it. Staff are in various stages of completing their Health and Safety training and their Child Maltreatment training. I reminded Ms. Allred the Child Maltreatment is to be completed every 5 years also. Some staff need additional ongoing training hours. Staff have until September 11, 2024, to complete their training. By September 12, 2024, Ms. Allred will email me copies of training certificates and I will complete the staff training for the center file. I observed two items of non-compliance of childcare rules during the visit today. Violation Number Comment Rule 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. All medications are to have the correct form completed, and the child's name on the medication. 10A NCAC 09 .0803(2)(b)(i-v) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Not all staff have completed the Child Maltreatment training. This training is to be completed every 5 years. .1102(g) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by September 3, 2024. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. TA for Medications: A suggestion for maintaining compliance with childcare rules pertaining to medications is monthly check all medications, action plans forms, permission forms, and expiration dates on the medications. Best practice - all medications should come to the office first to be checked for required information and that the forms are completed correctly. Check the medication expiration date and that the medication is for the correct child. Prescribed medications (1) shall be stored in the original containers in which they were dispensed with the pharmacy labels. (2) the child's name. (3) the name of the medication. (4) the amount and frequency of dosage. (5) the name of the prescribing physician or other health professional. (6) the date the medication was prescribed by the physician or other health professional. (7) shall be administered only to the child for whom they were prescribed; and (8) shall be administered according to the prescription, using amount and frequency of dosage specified on the label. A parent/guardian may give a caregiver standing authorization for up to six months to administer prescription or over-the-counter medication (as in Benadryl or fever reducer) to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. Fever reducer may have a standing authorization for children that have a history of febrile seizures. The authorization shall be in writing and shall contain: (1) the child's name. (2) the subject medical conditions or allergic reactions. (3) the names of the authorized over-the-counter medications. (4) the criteria for the administration of the medication. (5) the amount and frequency of the dosages. (6) the manner in which the medication shall be administered. (7) the signature of the parent. (8) the date the authorization was signed by the parent; and (9) the length of time the authorization is valid, if less than six months When a prescribed medication is given staff must complete the medication administration log. Make sure all prescribed medications are in the original container or have the prescription label directly on the medication, as in the Epi Pen. A picture of a label will work until the parent can get a box or label from the pharmacy. • OTC medications as in sunscreen, ChapStick, diaper creams, toothpaste are to have annual permission form completed by parent/guardian. • Emergency prescribed medications as in Inhalers, Epi Pens, Seizure medications are to have a completed Medical Action Plan, these plans must be signed by the medical provider and the parent/guardian. These are to be completed annually. The Prescription Medication Permission completed by the parent/guardian must be completed and signed every six months. Also, print the log along with permission form for staff to document when the medication is given. Best practice is not giving over-the-counter cold medications to any child. Child cannot have a standing permission for pain relievers for teething or other ailments. Staff are to be mindful of how they speak to children and their tone. During the after lunch quiet time school age children are to be given books, or other quiet activities. If a school age child wants to nap that is acceptable, but children are not required to sleep or stay on a mat. If school age children request a mat, the mat must be long enough for the child. Hold Harmless has been extended until the new QRIS is implemented. This means that you do not need to schedule an assessment of your rated license during your cohort year. Recognizing and Responding to Suspicions of Child Maltreatment training is located on this website, https://www.preventchildabusenc.org. The most up-to-date childcare rules are on the DCDEE website. Moodle also has other trainings, and they are free. Please keep in mind these trainings must be completed within one year of hire and once every five years. You will still need to complete the Recognizing and Responding to Suspicions of Child Maltreatment training through Prevent Child Abuse NC within 90 days of employment and the training must be completed every five years. When the Suspicious of Child Maltreatment is completed you must print you certification of completion immediately. If you go out of the program your certification is not save and you will need to retake the training. We appreciate all you are doing to serve the children and families of NC.
