The last annual compliance visit was conducted on January 11, 2024. An Annual Compliance Monitoring Checklist was completed during this visit. Upon arrival, I was greeted by Ms. Dacie Thompson-Hawkins, Owner/Operator, Ms. Thompson-Hawkins assisted me with the visit. Forty children were present today. Children were observed having a morning meeting as a group in the hallways of their classrooms, engaged in group times, playing in a variety of centers, and eating lunch. Children were being attended to according to their individual needs. The staff were observed engaged in activities with the children and were positive in their interactions with the children. Five children’s files were reviewed during today’s visit. All staff files were reviewed. We reviewed your staff and training worksheets together. A sanitation inspection was conducted on February 26, 2024, with an approved classification and 22 demerits. A fire inspection was conducted on September 16, 2024. Required items were posted and current. Compliance score is 100% The outdoor space was monitored, and one hazard was observed today. Outdoor inspections were documented monthly. Four violations were documented during today's visit. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. Mulch is needed for the four and five play area. .0605(j) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member had an expired certification. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member had an expired certification. .1102(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child was without a medical exam or health assessment. GS110-91(1) It is your responsibility to know and follow all of the child care rules and requirements, even if they have not been explained to you. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 12/19/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. We also discussed the process and requirements for lead and asbestos testing. I provided you with the information needed to complete the entire process. I also shared information with you about possible reimbursement. Dacie Thompson, administrator, accompanied me during a walk-through of the facility. During the visit, I spoke with Ms. Thompson. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. One staff member supervised a classroom of 20 children, combined ages 0 to 3-year-olds. 10A NCAC 09 .2818 Violations must be corrected immediately. An Annual Compliance Monitoring Checklist for Child Care Centers was used to note the requirements I monitored today. Your signature on the Visit Summary form will serve as verification that the information provided is accurate and complete. This facility operates with a Three- Star License issued 9/3/2020. The facility earned 4 points in Staff Education and 2 points in Program Standards, 1 Quality point (Infrastructure of Parent Involvement; Staff benefits package). The last Annual compliance Full visit was dated 1/24/2023. Upon arrival, I was greeted by Ms. P. Hicks who assisted me with the visit. D. Thompson, Administrator/ Owner, arrived later to continue assisting me during the visit. The last Sanitation Inspection was conducted on 7/24/2023 and the facility earned a "superior" classification with 8 demerits. The last annual Fire Inspection occurred on 7/21/23 and the facility was approved for daytime and night-time care. I walked through the indoor and outdoor areas of the facility that the children use during the visit. Forty-three children were present today. I observed the children engaged in free choice center play, eating lunch, and completing bathroom and hand washing routines. The teachers interacted with the children in a pleasant manner. There was one violation of Child Care requirements observed during this visit. The infant room had one table, one rocking chair and toys in front of the fire exit door. Mrs. Thompson-Hawkins corrected this violation during the visit. A correction letter is still required, documenting how you corrected the violation. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. There was a table, rocking chair and toys blocking the evacuation exit. All of these items were moved and the fire evacuation crib was placed closer to the exit. 10A NCAC 09 .0601(a) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. A letter stating how the violations were corrected and how you plan to maintain compliance in the future must be submitted to me by January 24, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. 5. Signature of the person preparing the letter. Please submit the signed letter electronically to my email address. If I do not receive your letter prior to January 24, 2024, a return visit to your program may be conducted to assess compliance with the violations. Technical Assistance provided: New Child Care Sanitation Rules Children’s Environmental Health has set up virtual trainings for child care center operators on the newly readopted child care sanitation rules. The dates for the trainings are September 20, 2023 and September 26, 2023; both scheduled from 9:00 am – 11:30 am. Dacie Thompson, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Thompson and an additional staff member. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Staff members did not record the arrival and departure time of a child on 11/27/23. 10A NCAC 09 .0302(d)(4) 303 Children were not adequately supervised at all times. A staff member was aware of children playing in a closet storage area. The staff member was not aware that the children closed the closet door. As a result, the staff member could not see or hear the children in the closet. .1801(a)(1-5) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Staff members did not have the child's parent sign the incident report. .0802 (e) 1952 A copy of the incident report was not given to the parent. A staff member did not give a parent a copy of an incident report on 11/27/23. .0802(e) Violations must be corrected immediately.
