    Name of Operation: AFS CHILDCARE II Facility ID: 41002500 Consultant: ELIZABETH CHILDRESS Operation Type: Family CC Home Case Number: Visit Date: 2/24/2026 Number Present: 3 Completed Date: 2/24/2026 Age: From 2 To 3 Total Minutes: 75 Time In: 01:15 PM Time Out: 02:30 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance with child care rules. This Family Child Care Home operated with Two-Star License issued 11/14/2025. The last Annual Compliance was conducted on 8/12/2025. Upon arrival I was greeted by India Sturdivant, Owner/Operator. Ms. Sturdivant, Aniya Sturdivant, and three children were present during today’s visit. During the visit Ms. I. Sturdivant had to leave for an appointment. Ms. A. Sturdivant continued to assist me with the visit. A fire inspection was completed on 04/23/2025. I received a copy of the inspection report during the visit. Emergency drills and playground inspections were not completed. Lessons plans were posted but not current. I reviewed copies of CPR and First Aid Certifications, Infant and Toddler/Sudden Infant Death Syndrome Certifications, Criminal Background Qualifications Letters for the owner/operator, and household members. Four violations were observed today. One was corrected during the visit and the other must be corrected immediately. The violations are as follows: Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. Multivitamins and one prescription bottle were observed on the kitchen table. The provider corrected this during the visit. .1719(a)(5) 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. One child who was present during today’s visit had an inhaler at the Family Child Care Home. There was no authorization for the operator to administer the medication. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. The most recent dated sign in sheets were dated 11/22/2026. .1721(e)(6) 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. Monthly fire drills, quarterly lockdown or shelter-in-place drills and playground inspections were not available for review. .1721 (e)(7) The violations documented must be corrected immediately. On or before, March 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. We discussed the following items of technical assistance today: Medication- One child who was present during today’s visit had an inhaler at the Family Child Care Home. There was no authorization for the operator to administer the medication. Having the correct documentation on file when caring for children with health issues is vital in ensuring their health and safety. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were improperly stored on the kitchen table. Multivitamins and one prescription bottle were observed on the kitchen table. The provider moved these items to a locked storage area during the visit. It is extremely important that staff take the required time to lock doors, store hazardous items properly either in locked storage or five feet up from the finished floor. Outdoor Inspections- A designated staff member should verify daily that the area is safe for children to use. Each month, the outdoor play areas should be inspected and documented on the form provided by the Division of Child Development. Inspections were not available for review. Fire drills/emergency drills: Fire drills are required monthly. Lockdown or Shelter-in-place drills are conducted quarterly. Document dates that you will conduct fire drills and lockdown or shelter in place drills and have someone available for backup in case the person that usually conducts them isn’t present. Fire drills and Lockdown or Shelter-in -Place were not available for review. We discussed the following consultation items today: ACTIVITY PLANS - Complete weekly activity plans with the current week dated that include developmentally appropriate activities for enrolled children. Activity plans were posted, however, dated “Week One for December.” Activity plans for all children must meet the following minimum licensing requirements: *Include daily activities to stimulate the following 5 developmental domains, as listed in your NC Foundations for Early Learning and Development book: (social &emotional development, health & physical development, approaches to play and learning, language development/communication, and cognitive development). *Include a daily gross motor activity which may occur indoors or outdoors. *Reflect at least 4 activities daily (one of which is gross motor), one of which is offered outdoors, weather permitting. The remaining 3 activities should occur in the following areas (art/creative play, children’s books, blocks and block building, manipulatives and family living/dramatic play). *Include both free choice and adult-directed activities, that allow children to choose to participate with the whole group, in a small group or independently. *In addition, activity plans must include each of these 3 enrichment activities on at least a weekly basis: sand or water play; science and nature; and music and rhythm. A sample lesson plan form containing all required components was provided. *Materials and equipment must be available both inside and outside to support the activities on the activity plan and the daily schedule. At the completion of the visit, this visit summary was reviewed with you, and a copy was emailed to you. Upon arrival Aniya Sturdivant was present and caring for the 2 preschool children. The children were eating lunch. I observed the children eating and transitioning to nap time. The following items were monitored during today's visit: supervision, capacity, approved space, safe environment, program records, children's records, and items posted. India Sturdivant arrived before the end of the visit and we discussed the rated license paperwork, her education and she completed the 2-5 star application. The following violations were cited and corrected during today's visit: Violation Number Comment Rule 601 Nutritious meals and snacks served did not meet Meal Patterns for Children in Child Care Programs, apple juice was being served instead of milk and the menu did not reflect what the children were eating. 10A NCAC 09. 1706(a) 929 Developmentally appropriate activities for the ages of children in care, were not provided as planned on the written schedule and activity plan. We discussed the following items of consultation: -India is currently in school for Early Childhood (EC) courses and has completed EDU 119 in 2024 and also completed 1 additional course and is enrolled in another. As previously discussed, you will need to submit your EC courses to our WORKS unit for verification and to be entered into the system. We discussed that in order to increase to 3 stars you will need to have your education into WORKS with EDU 119 and your 2 additional classes, making a C or better along with completing your 3 month self-study. -We reviewed the proposed points and completed the rated license paperwork: Education: 3 points, with EDU 119 Program: 1 point, with meeting minimum requirements Quality: 1 point, with serving no more than 2 infants under age 1 year Total: It appears that you will receive 5 points for a 2 star FCCH License, total points will be determined by upper management. We discussed that you cannot be eligible to receive DSS subsidy funding without being at least a 3 star license and what you will need to do to earn that and you stated that you understand that.
