Name of Operation: ABG PROVIDER SERVICES CHILD CARE II Facility ID: 41002286 Consultant: WYTINA BLACKMON Operation Type: Center Case Number: Visit Date: 7/11/2023 Number Present: 68 Completed Date: 7/11/2023 Age: From 0 To 11 Total Minutes: 180 Time In: 09:30 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for compliance during an annual compliance visit. A checklist was used to note the requirements I monitored today. This facility currently operates with Five Star license issued on June 17,2019. Upon my arrival to the facility, I was greeted by Ms. B. Liles, the administrator. Ms. Liles assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medication and hazardous items were out of the children's reach. Sixty-eighty (68) children were present during today’s visit. I observed the children playing on the floor with available toys, eating morning snack, playing in centers, playing outside on the playground, washing their hands in preparation for lunch and eating lunch. A sanitation inspection was conducted on April 27,2023. This facility received a superior rating with two (2) demerits. A fire inspection was conducted on August 29, 2022. The original will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. I checked the NC Department of the Secretary of State for your corporation status. Your corporation status is active and current. I monitored two vans this facility uses to transport the children from school and on field trips. The school age children were observed being drop off from school. The first aid kit and the children's emergency information were on each van. The insurance policy was current for the vans. The vans met the transportation requirements and were in compliance during today's visit. One (1) violation was cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A safe outdoor environment was not provided for the children. There are trees roots exposed on the infant/toddlers' playground. This is a tripping. 10A NCAC 09 .0601(a) The above violation must be corrected immediately, and a letter must be sent to me no later than July 25 ,2023 stating how the one item was corrected. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Liles that the staff members need in-service training hours. It-SIDS, CPR and First Aid Trainings by specific dates. Please refer to the staff/training worksheet. You need to email the training certificates once staff has completed the training hours. Also there are several children's whose medication authorization will expire in August 2023. New rules: I also discussed with Ms. Liles the Rated license assessment process will begin sometime next year. There is an observation assessment that you can contact the rated assessment project. I told Ms. Liles to let me know when she was ready to start the process. Thank you for your time during today's visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
